This is a randomized controlled trial (RCT) to evaluate the influence of different doses of vitamin D3 (800 IU/d versus 400 IU/d), on serum levels of interleukin (IL)-6, TNF-alpha and C- reactive (CRP) in premature infants with clinical evidence of late-onset sepsis and to assess its influence on clinical outcomes of these infants.

Vitamin D has an important role in the regulation of both the innate and adaptive immune systems. There are very few studies of such roles in the neonatal population. It is potentially an attractive therapeutic agent for sepsis given its low cost and low risk of toxicity and side effects. There is no consensus regarding to the dose of vitamin D supplementation required for preterm infants given the paucity of evidence. AAP and ESPGHAN have recommended different dosages of vitamin D ranging from 400 IU to 1000 IU per day. The influence of different doses of vitamin D on immunological status and clinical outcomes of preterm infants with late-onset sepsis has not been evaluated before. This RCT will evaluate the influence of different doses of vitamin D3 (800 IU/d versus 400 IU/d), on serum levels of interleukin (IL)-6, TNF-alpha and C- reactive (CRP) in premature infants with clinical evidence of late-onset sepsis we will also evaluate their safety and influence on clinical outcomes of these infants

* Limiting toxicity as defined by a grade 3 adverse event that does not resolve to grade \< 3 within 7 days; adverse event grade 4 or 5.
* Subject decision.

Regorafenib Oral Product

The present protocol (STRAT-aHCC trial) aims to prospectively evaluate the tolerability, quality of life and efficacy of an alternative regimen of regorafenib in patients with advanced hepatocellular carcinoma (HCC) after progression to first-line. Patients will receive increasing dose of regorafenib in the first 2 treatment cycles (initial dose of 80mg, with weekly increments of 40mg up to 160mg in the first 2 treatment cycles). From the 3rd cycle on, the maximum tolerated dose during the first 2 cycles will be maintained. The maximum tolerated dose will be considered the highest dose in which the patient does not present grade ≥3 adverse events. The primary endpoint is the proportion of evaluable patients completing cycle 4. Radiologic response rate, quality of life, time to progression and overall survival will be evaluated as secondary endpoints.

In patients who are refractory to first-line treatment with sorafenib, regorafenib was tested in the phase III RESORCE study. In this study, a standard dose of 160mg/day was used (in cycles of 4 weeks, with 3 weeks on treatment and 1 week off) until tumor progression or limiting toxicity. Regorafenib was superior to placebo with a significant increase in overall survival and became a sequential treatment option.

However, regorafenib is associated with relevant adverse events such as fatigue, hand-foot reaction, diarrhea and hypertension. Such events are more frequent in the first 2 cycles. In the RESORCE trial, 54% of patients had adverse events requiring interruption or dose reduction, and treatment discontinuation was required in 10% of patients. This toxicity profile limits the wide adoption of this drug in clinical practice.

Dose escalation strategies for regorafenib have been evaluated in patients with colorectal cancer and have resulted in better tolerability with comparable efficacy. However, there are no prospective studies with alternative doses of regorafenib in patients with advanced HCC.

Medical visits will be carried out weekly in the first 2 treatment cycles and every 4 weeks after the 3rd cycle. Laboratory tests will be performed every 2 weeks during the first 2 cycles and every 4 weeks after the 3rd cycle. Response assessment by imaging exams will be performed every 8 weeks. Quality of life assessments will be performed every 4 weeks using the EuroQol EQ-5D-5L7 questionnaire. The planned sample size is 28 patients with an expected duration of 25-30 months.

Human epidermal growth factor receptor 3 (HER3) expression is seen across a wide variety of solid malignancies and is associated with poor prognosis. Up-regulation of HER3 expression and activity is also associated with resistance to multiple pathway inhibitors. GSK2849330, a monoclonal antibody (mAb) targeting HER3, is a new agent for subjects whose tumors express HER3. This study aims to characterise the biodistribution and dose-receptor occupancy relationship of GSK2849330 in patients with advanced HER3 expressing solid tumours via the use of PET imaging. This study will be conducted in two parts. Part 1 will be the imaging phase where each subject will receive two doses of GSK2849330 containing both Zirconium-89 (89Zr) labelled GSK2849330 and unlabeled GSK2849330. The amount of unlabeled GSK2849330 present in each dose will be varied to explore the effect on target mediated uptake of 89Zr into HER3 expressing tissues and tumors. Subjects will then proceed to the continuation phase (Part 2) for continued treatment with unlabelled GSK2849330. The study is planned to enroll approximately 12-15 subjects.

This is a single center, non-randomized phase IIa study to determine the tolerance and safety of Abraxane (ABX) in combination with cisplatin (DDP) in patients with advanced nasopharyngeal carcinoma (NPC). Patients in whom the standard therapy had failed or had been infeasible will be eligible.The safety and efficacy will be evaluated according to NCI-CTCAE V4.0 and RECIST 1.1 respectively.

Nasopharyngeal carcinoma (NPC) is most commonly seen in Southeast Asia, especially in southern and southeastern China, where the incidence rate has been documented between 10 and 150 cases per 100,000 population per year. For advanced or metastatic NPC, chemotherapy remain the mainstay of treatment. The 130-nm albumin-bound formulation of paclitaxel (\[Abraxane, ABX \];Celgene,Summit,NJ) is a promising new agent with more efficient entry to the tumor microenvironment via caveolae-mediated transcytosis and preferential uptake by cancer cells. Superior activity of ABX-based regimens without the necessity for antianaphylactic pretreatments been shown in various solid tumors compared with the traditional solvent-based paclitaxel-based ones. However, the safety and efficacy of combination of ABX and cisplatin (DDP) has not been determined in patients with advanced NPC. In this single center, non-randomized phase IIa study, investigators seek to determine the maximum tolerated dose (MTD) and dose limiting toxicity (DLT) of ABX-DDP, and perform an exploratory study of its efficacy as measured by tumor response.

3-D conformal radiotherapy offers the opportunity to reach higher tumor doses with acceptable complication rates compared to conventional radiotherapy. There are retrospective and preliminary prospective reports of a better local control / disease free survival (including PSA control) using higher tumor doses. A prospective randomised phase III study is required to validate these reports. The purpose of this randomized phase III study is to investigate and compare the tumor control and toxicity in prostate cancer patients treated to 68 Gy and 78 Gy;

Patients were stratified by hospital, treatment group, age and (neo)adjuvant hormonal therapy. Four treatment groups with specific radiation volumes were defined depending on the estimated risk of tumour involvement of the seminal vesicles (based on T-stage, initial PSA and Gleason Score or Differentiation Grade).The dose was delivered with daily fractions of 2 Gy, five times a week. The total duration of the treatment was 7 weeks for patients treated to 68 Gy and 8 week for 78 Gy.

Objectives:

* To test in a phase III randomised study the hypothesis that higher radiation doses lead to a higher freedom from failure rate for localized prostate cancer.
* To test the hypothesis that a higher freedom from failure rate leads to a longer disease-free survival and overall survival.
* To limit acute and late toxicity with respect to gastro-intestinal and urological toxicity.
* To evaluate different PSA related endpoints for local failure and distant metastasis.
* To establish a database of patients treated to escalated doses with Dose Volume Histograms of the normal tissues at risk for each individual patient and common toxicity endpoints.
* To validate a model to estimate normal tissue complication probabilities (NTCPs) of rectum and bladder, based on the above-mentioned database, and to obtain reliable parameter values for NTCP estimations.

The situation in Italy regarding radiation dose to patients undergoing Multidetector row Computed Tomography (MDCT) scans is unclear despite Italian law require that each centre check the Reference Dose Level (RDL) every two years. RDL in CT were first suggested in a document of 1998 published by the European Commission (EUR16262/1998) based on the results of an older survey in the UK from the late 1980s. Those RDLs, expressed in terms of Computed Tomography Dose Index (CTDI) and Dose Length Product (DLP), are reported in the Italian law (D.Lgs.187/00). However, in Italy there is no governmental authority comparable to the UK's National Radiological Protection Board, deputed to dose evaluation and protection.

An overview of the Italian situation is relevant in order to know the actual dose levels for MDCT administered to patients both adults and paediatrics.

CT imaging is rapidly growing and now represents an indispensable medical procedure for many clinical conditions. The widespread availability and expanding use of multidetector CT (MSCT) due to its high performance and very short acquisition times, has resulted in an increase in both frequency of procedures and level of patient exposures from CT scan examinations, but issues related to radiation, including potential risks, have been relatively neglected. Particular attention should be given to CT scans in children, as they represent about 10% of all the X-ray studies performed on them, accounting for about 75% of the total dose administered. CT in children should be performed only when absolutely necessary as their longer life expectation and sensitivity to X-ray radiations may increase the risk of causing radiation-induced neoplasm's in later years.

According to the 2000 report of the United Nations Scientific Committee on the Effects of Atomic Radiation (UNSCEAR) the frequency of CT examinations in countries with high healthcare levels has increased by a factor of 8; with the mean effective dose increasing from 1,3 mSv in the 1970s to 8,8 mSv per year per 1000 persons in the early 1990s. In Switzerland, Germany and United Kingdom the increases in the number of reported CT examinations ranged between 39% and 70% over just the period 1998-2002. The 2000 ISTAT report's estimated that 1479 CT apparatus were installed in Italy performing a total of 3.327.750 CT examinations \[3\]. Considering an increase of 40% every 5 years, comparable to other European countries, 6.000.000 CT examinations can be estimated for 2010.

The primary objective of the study is to obtain a snapshot of Italian CT practice in adults and children that is up-to-date in regards to information concerning multidetector 16- and 64- row CT. A second goal is to update the national reference dose levels.

The study should further establish a sustainable database for the ongoing collation of further data from local surveys of CT. This will facilitate the analysis of trends and periodic review of national reference doses for CT in a way similar to other European countries.

Radiology departments dealing with adult and paediatric CT in Italy will receive an invitation to participate to the survey and a questionnaire regarding some important features of their CT equipment. Four common protocols will be used for dose calculation: CT scan of the head, lumbar spine, chest and abdomen. Cardiac CT data will be collected only for adults, as in children other examinations, such as MR are commonly preferred to CT just for radioprotection reasons.

All radiology departments using a 16 or 64 multidetector CT will be sought for enrolment in the study. Centres that perform at least 2000 MDCT examinations in either adults or paediatrics in one year and do a regular quality control on their CT scanner will be included in the survey list and divided according to their geographical site, namely the four macro-areas of Northern Italy, Centre, Southern Italy and Islands.

Each enrolled centre will receive a standardized form to fill in. The main technical parameters included in the questionnaire will be: CT scanner manufacturer, model and type, characteristics of automatic modulation system (modulation directions and chosen image quality level e.g. Noise Index and effective mAs). Tube voltage (kV), current (mA), tube rotation time(s), slice thickness, total acquisition time (s), pitch, filtration should also be given. For each patient examination Volumetric Computed Tomography Dose Index (CTDIvol) and Dose Length Product (DLP) should be collected from the dose report. For the cardiac protocol, a dedicated questionnaire will be distributed including typical cardiac parameters in addition to those mentioned above. For the protocols performed on adults, data are requested for normal patients (excluding those who were excessively small or large). For children, the age at examinations and weight are included. Paediatric studies will be those of persons less than 16 years old, as established by the Italian regulation.

In order to verify the accuracy of the console displayed values a medical physicist should measure before starting the survey the CTDIvol following the European Guidelines EUR16262/1998. Errors less then 10% between the measured and reported value will be accepted.

Each protocol will be analysed separately and descriptive statistical analysis of the main parameters will be performed. These include sample size, range, median, mean value, third quartile and percentage coefficient of variation.

The survey will be conducted over a period of twelve months. The data will be treated in an anonymous way, with the centres being coded by alphabetical letters. The investigators calculate that a sample size of 2,421 MDCT examinations for each protocol will be sufficient to guarantee a confidence of 90% and an absolute error of 5% of the average dose, with simple random sampling and assuming a conservative hypothesis that the population standard deviation is five times that estimated in a similar German survey \[4\].

In order to increase the accuracy of the estimation the investigators will adopt a stratified random sampling scheme that consists of stratification into the four geographical Italian macro-areas and the random selection of a number of MDCT examinations proportional to the population living in each macro-area. Stratification facilitates the tasks of data collection and survey, and aides in removing a possible source of bias related to the different frequency of MDCT apparatus, as MDCT services will be clustered into internally homogeneous strata.

Value of Mean Volumetric Computed Tomography Dose Index (CTDI Vol) for Spine CT Examinations.

A total of 65 radiology departments, with 70 MDCT scanners, collected data of 5942 adult patients, randomly chosen, who underwent to CT examinations for common clinical for 5 common different protocols including also new examination that will be in the near future common practise as cardiac CT.

A prevalence of multiphasic study was documented in many chest abdomen and pelvis (CAP) studies and abdominal studies.
