Intravenous normal saline 2 ml is given slowly as a placebo before the anesthesia induction and after finishing anesthesia.

Intravenous nefopam 20 mg is given slowly before the anesthesia induction and after finishing anesthesia.

Saline

Nefopam will be administered to the patients undergoing laparoscopic gastrectomy under the total intravenous anesthesia (TIVA).

The investigators will evaluate whether the nefopam can decrease the total amount of remifentanil administered during the operation, which will reduce the acute opioid tolerance and following consumption of fentanyl during postoperative period.

Breast cancer surgery is known to cause severe acute postoperative pain, which can persist for a long time. The investigators administered nefopam preventively to patients undergoing total mastectomy or modified radical mastectomy with axillary lymph node dissection or sentinel lymph node biopsy, and evaluated its efficacy on acute and chronic postoperative pain.

Effectiveness of Nefopam in Breast Cancer Surgery

20 mg nefopam in 100 ml normal saline is infused before starting operation.

Nefopam is administered as a preemptive analgesic drug to the patient undergoing breast cancer surgery. It will be evaluated whether postoperative acute and chronic pain can be reduced.

The nefopam group received 120 mg nefopam (Acupan, Pharmbio, France) by continuous iv infusion over the study period (48h)

The Control group received 120 mg placebo infusion (saline solution) by continuous iv infusion over the study period (48h).

Nefopam will not be administered in this group.

Intravenous nefopam 20 mg will be administered twice, immediately after induction of anesthesia and at the end of surgery.

Nefopam 20 mg is mixed to 100 mL of normal saline, and is infused intravenously over 15 minutes.

20 mg of nefopam is administered intravenously for two times, after induction and at the end of surgery.

Acute postoperative pain after video-assisted thoracoscopic surgery is very severe. Because pain after thoracic surgery prevents early recovery, and increases postoperative complications, its is important to prevent pain after video-assisted thoracoscopic surgery. In the present study, we will evaluate whether nefopam reduces pain after video-assisted thoracoscopic surgery.

Acute postoperative pain will be assessed using numerical rating scale from 0 (no pain) to 10 (worst pain ever).

Fentanyl consumption administered thorough the patient-controlled analgesia machine.

Additional analgesic use during postoperative period.
