The primary aim of this non interventional study is to assess the independent prognostic role on overall survival of primary tumour 18F-FDG uptake value (SUVmax) measured on 18fluorodeoxyglucose positron emission tomography (18F-FDG-PET) in patients with non-metastatic non-small cell lung cancers treated with curative intent, taking into account the other conventional prognostic factors (performance status, age, sex, disease stage).

PubMed, ScienceDirect, Cochrane Database of Systematic Reviews will be used to search for articles published from January 1965 to July 2019 using the key words "renal cancer", "lymphocyte to monocyte ratio" and "prognosis". No restrictions to date, language, or article type will be applied. Cohort or observational studies in patients with non-metastatic renal cell carcinoma histopathologically confirmed, with hazard ratios (HR) and corresponding 95% confidence intervals (CI) that assessed association between LMR and overall survival (OS), cancer-specific survival (CSS), recurrence-free survival (RFS), and disease-free survival (DFS) will be analyzed.

A systematic review of the literature will be carried out using Patient, Intervention, Comparison and Outcome method (PICO), with the aim of answering the following clinical question: "Is pretreatment under lymphocyte to monocyte ratio (LMR) a prognostic factor in non-metastatic renal cell carcinoma ? "

All analyzes will be based on previously published studies. For this reason, patient consent and ethical approval will not be required.

An exhaustive search will be conducted in PubMed, Science Direct and Cochrane Database of Systematic Reviews for eligible studies that explored the prognostic role of LMR in patients with localized renal tumors, who underwent to partial or radical nephrectomy from January 1965 to July 2019. The terms search will include: "monocyte lymphocyte ratio", "renal cancer", "prognosis".

Cohort studies or observational studies will be included, patients with localized renal tumors who underwent partial or radical nephrectomy, with histopathologically confirmed neoplasms, who had access to the full text and without language limitation.

Inclusion:

Any observational study (cross-sectional, case-control, longitudinal with cross-sectional data) will be included.

Exclusion: Reviews, case reports, conference abstracts, letters, animal or cell studies.

Relevant articles will be identified in duplicate by two independent reviewers by first screening the titles and abstracts followed by the full text against inclusion and exclusion criteria. Any disagreement will be resolved by consensus with a third reviewer experienced in the renal cancer management.

The studies reflected the hazard ratio (HR) and corresponding 95% confidence intervals (CI), in which the overall survival (OS), specific cancer specific survival (CSS), recurrence-free survival (RFS), disease-free survival (DFS) and progression-free survival (PFP).

Information will be extracted for the first author, publication year, geographic location, study design, patient information (sample size, mean/median age, sex distribution, performance status), LMR, endpoint (OS, CSS, RFS, DFS, PFS), therapy, follow up duration, multivariate factors, hazard ratio (HR) and corresponding 95% confidence intervals (CIs), or exact P values. When univariate HR and multivariate HR were both reported, only the multivariate HR will be used.

OS is defined as the interval from the date of surgery in the primary tumor until death. CSS is defined as the interval from the date of surgery in the primary tumor to death for urological tumors. RFS is defined as the interval from the date of surgery in the primary tumor to local, regional or distant recurrence or death from any cause. DFS is defined as the interval from the date of surgery in the primary tumor to local, regional or distant recurrence. PFS is defined as the interval from the date of surgery in the primary tumor to the progression of the disease (including local recurrence or distant metastasis) or death.

The Newcastle-Ottawa Scale (NOS) will be used to assess the quality of studies. A maximum of 9 points can be given for each study in the categories of: selection of patients, comparability of the study groups, and assessment of outcomes. We will define high-quality studies with scores \>7.

If the necessary data are available, subgroup analyses will be performed to explore the potential sources of heterogeneity according to country, analysis type, tumor type, sample size and cut-off value.

Statistical study:

Data will be combined using random effect models. The Cochrane χ² (Cochrane Q) statistic and the I² test will be used to analyze heterogeneity.

Before calculating the combined results for all trials, statistical heterogeneity will be evaluated by using the I² statistic and p-value, which assessed the appropriateness of pooling the individual study results. The I² value provided an estimate of the amount of variance across studies because of heterogeneity rather than chance. I² values of 25%, 50%, and 75% corresponded to low, moderate, and high levels of heterogeneity, respectively. If p\> 0.05, the heterogeneity will be not substantial. Thus, a fixed-effect model will be used to calculate forest plots. If p \< 0.05, however, the heterogeneity will be considered substantial. Then a random effects models were used.

The publication bias will be graphically explored through funnel plot, and Duval and Tweedie's "trim-and-fill" test will be used to correct possible publication bias.

Statistical significance was defined as p less than 0.05.

Statistics will done using R 3.5.0 (R Core Team, 2018) and the meta (v4.1-5; Schwarzer, Guido, 2019) package.

Multi-center, non randomised, open label, non controlled pilot study. Evaluating the treatment of bevacizumab in association with pre-operative chemotherapy, followed by surgery, adjuvant chemotherapy and radiotherapy in Patients with inflammatory breast cancer.

Pilot study evaluating the safety and efficacy of adding Bevacizumab to neoadjuvant chemotherapy in patients presenting non metastatic inflammatory breast cancer (IBC). Patients will receive 4 cycles of chemotherapy FEC100 associating Fluorouracil (500 mg/m2), Epirubicin (100 mg/m2), Cyclophosphamide (500 mg/m2) and Bevacizumab 15 mg/kg every at day 1 of ecah 21 days cycle for 4 cycles. Six weeks after the end of neoadjuvant chemotherapy, patients will undergo mastectomy and 4 cycles of Docetaxel (100 mg/m2)as adjuvant chemotherapy +/-Trastuzumab 8 mg/kg for the first cycle then 6mg/kg every 3 weeks for 17 cycles if tumor overexpress Human Epidermal Growth Factor Receptor 2 (HER2).

RT will encompass bilateral internal and external iliac lymph nodes at a dose of 45 Gy and the bladder up to 63 Gy. Fractionation will be 1.8 Gy per fraction.

Cisplatin: 20 mg/m2/day through continuous iv perfusion for 4 consecutive days, from D2 to D5 and from D23 to D26 for the first part of the treatment, then from D2 to D5 if an additional treatment is decided.

A cystoscopy with a transurethral resection will be performed 3 weeks after the last day of the first part of radio-chemotherapy.

* In the absence of tumor cells, the 2nd part will start on the 4th week or at latest on the 5th week after the last day of the 1st part of radio-chemotherapy.
* In case of a microscopic tumor residue or of a local tumor progression, operability will be re-evaluated in view of a radical cystectomy and the patient will exit the study.

Radiation + cisplatin

Radiotherapy will encompass bilateral internal and external iliac lymph nodes at a dose of 45 Gy and the bladder up to 63 Gy. Fractionation will be 1.8 Gy per fraction.

Cisplatin: 20 mg/m2/day through continuous iv perfusion for 4 consecutive days, from D2 to D5 and from D23 to D26 for the first part of the treatment, then from D2 to D5 for the second part of the treatment if an additional treatment is decided.

Gemcitabine: iv injection for 30 minutes, twice a week at a dose of 25 mg/m2 on days 2, 5, 9, 12, 16, 19, 23, 26, 30, and 33 for the 1st part of treatment, then on days 2, 5, 9, and 12 for the 2nd part of treatment if an additional treatment is decided (cystoscopy with a transurethral resection). RT will be delivered between 2 and 6 hours after completion of the gemcitabine injection.

If radical cystectomy remains the standard of care for muscle invasive bladder cancer, consequences of this surgical procedure are often harsh. Over the past years, concurrent chemo-radiotherapy has imposed itself as an alternative treatment. Published data on concomitant radiochemotherapy (radiotherapy/cisplatin or radiotherapy/cisplatin/5-fluorouracil combinations) showed local control rates with bladder preservation at 5 years ranging from 40% to 65% according to the disease stage, and overall survival probabilities ranging from 40% to 50% at 5 years. In order to improve local and systemic prognosis, evaluation of other chemotherapy agents with higher radiosensitizing effect, such as gemcitabine, is justified. Gemcitabine possesses its own anti-cancer activities on urothelial diseases and has a synergetic activity with cisplatin. The investigators completed a monocenter phase I study combining radiotherapy, cisplatin, and twice-weekly gemcitabine, and determined a recommended dose of gemcitabine 25 mg/m². The objective of the present study is to evaluate the combination of radiotherapy + cisplatin + gemcitabine in terms of disease-free survival in non metastatic muscle invasive urothelial cancer patients.

A Randomized Phase II Trial Evaluating an Organ-conserving Strategy With Radiotherapy + CDDP + Gemcitabine vs Radiotherapy + CDDP in Muscle-infiltrative Bladder Cancer

The time to relapse is defined as the time from the date of randomisation to the date of the first event. Time to relapse for patients without any event (local, regional, distance, or death) will be censored at the date of latest information.

The time to death is defined as time from the randomization to the date of death from any cause, or to the date on which latest information is obtained.

Acute and late toxicities will be scored according to the NCI-CTC v4.0.

Questionnaires QLQ C30 + QLQ-BLM30 + QLQ-ELD15 + Oncodage

Before starting radiotherapy, 5ml of blood will be sampled in a 5ml heparinised tube to prospectively measure the rate of CD8 radio-induced lymphocyte apoptosis before any radiotherapy treatment. A correlation between the low rate of lymphocyte apoptosis and the severity of late toxicities will be studied to confirm the predictive power of this biological test on radio-induced side-effects.
