Penn State Milton S. Hershey Medical Center researchers are trying to compare the effects of two different forms of selenium (selenium yeast and selenomethionine) on blood selenium levels and biomarkers of oxidative stress as primary endpoints. One in six men are at risk of getting prostate cancer in their lifetime. Participants will be asked to take over-the-counter selenium yeast supplements and selenomethionine or a placebo for nine out of twelve months.

We will conduct a double-blind, randomized, placebo-controlled clinical study of selenium supplementation in the form of L-selenomethionine (SM)(200 µg/day) and selenium-enriched yeast (SY) (200 µg/day and 285 µg/day) for 9 months; the 285 µg/day SY is selected to deliver an equivalent selenium as in 200 µg/day SM to healthy men. The variability of SM in SY that will be used here is less than 3% and it accounts for 70.5% of the SM content in SY. As primary endpoints, we will determine the effects of these two forms of selenium on plasma levels of selenium and its metabolites as well as biomarkers of oxidative stress at several time points. As a secondary endpoint, the effect of these two forms of selenium on plasma PSA levels will be examined.

The objective of this study is to investigative the influence of different levels of glycaemia or insulinemia in vascular endothelium in ischemia/reperfusion lesion after myocardial infarction

The study will include patients consecutively admitted with acute myocardial infarction at State University of Campinas Hospital. Participants will be evaluated during hospitalization and, after they fulfill the inclusion criteria and sign an informed consent, it will be collected a brief medical history and recorded results from glycated hemoglobin (HbA1c), glycaemia, total cholesterol (TC), triglycerides (TG), HDL cholesterol (HDL-C), C-reactive protein (CRP), urea (U), creatinine (Cr), markers of myocardial necrosis (troponin T and CK-MB), Hemoglobin, Red blood cell, Leucocytes, Na, K, Ca and Mg.

In the first week after infarction, from admission until the realization of the clamps experiments (the medical procedure where blood glucose and insulin levels will be controlled), all medications prescribed, medical history and examination will be recorded, as well as follow-up during hospitalization.

At the day of the clamp, between 144 and 168 hours after infarction, volunteers will be randomized to one of the three situations with different glycemic and insulinemic conditions, i.e., a) high insulinemia and low glycaemia, b) high insulinemia and high glycaemia and c) low insulinemia and high glycaemia. During these experiments, endothelial vasomotor function will be recorded in four periods: i) before initiation of the procedure, ii) after 1h of the beginning, iii) 30 minutes after the second procedure and iv) 15 minutes after the simulation of ischemia/reperfusion lesion. In each time point, the investigators will collect plasma to dose inflammatory, oxidative stress and vascular activation markers.

Hospital care of subjects

The patients included in this survey will be treated according to actual guidelines. Briefly, will be enrolled patients who underwent primary angioplasty within 6 hours after symptoms onset, were introduced antiplatelet and anticoagulation therapy at the correct dose, respectively Acetylsalicylic Acid (ASA) 200 mg and Clopidogrel 300 mg, and enoxaparin 30 mg. In patients without contraindication, it will be administered in the first 24 hours beta-blockers and ACEi in those with ejection fraction (EF) \<0.4, previous MI or prior heart failure. Upon admission, if there is no contraindication, simvastatin 40 mg/day will be administered. As a maintenance dose, the investigators will administer ASA 100 mg, clopidogrel 75 mg, enoxaparin 1 mg/kg twice daily, simvastatin 40 mg, propranolol 40 mg or metoprolol 50 mg (beta-blockers available at the hospital) and captopril 25 mg (most used ACEi). Patients will be treated preferably in the Coronary Unit (CU). Researchers may suggest the physician responsible for the patient care to prescribe the above medications, as well as question why the drugs listed in the guidelines are not being used. The reasons and specific contraindications will be recorded.

The researchers are not directly responsible for the treatment of individuals, which does not absolve them of responsibility for any adverse effects related to the experiments. Also, the investigators emphasize that the study did not interfere with the flow of patients during hospitalization, i.e., will not influence the discharge of patients.

Randomization to one of the three clamps

As observed, the investigators will evaluate endothelial reactivity by FMD. On the day of the clamp, after fasting, they will be exposed to one of three situations: (i) hyperglycemia / hypoinsulinemia, (ii) euglycaemia / hyperinsulinemia and (iii) hyperglycemia / hyperinsulinemia.

The clamps will simulate the possible attenuation or increase in endothelial reactivity promoted by the independent variables during ischemia/reperfusion (I/R) injury. In total, the test will consist of realization of a basal FMD-1 (during 15 minutes), clamps (variable length, usually 60 minutes), FMD-2 (15 min) after the clamp, rest time (30 min), ischemia/reperfusion (20 min) injury, rest time (15 min) and FMD-3 (15 min). Throughout the experiment, blood glucose and insulin will be controlled and maintained at levels such as designed previously for each clamp situation. In all subjects, plasma samples will be collected before the start of the clamp (during FMD-1), at the end of the measurement of the second FMD during the clamp, prior to the I/R injury and 15 minutes after the end of the same test (during FMD-3). In these samples will be dosed insulin, glucose, C-peptide, C-reactive protein, tumor necrosis factor-α (TNF-α), nitric oxide, intercellular adhesion molecule-1 (ICAM-1), plasminogen activator inhibition-1 (PAI-1), isoprostane and interleucin-1 (IL-1).

In one group, the euglycemic/hyperinsulinemic consists of a situation in which insulin levels are above physiological levels and blood glucose levels maintained at approximately 100 mg/dL. Euglycemic/hyperinsulinemic clamp is similar to the way the hyperinsulinemic/hyperglycemic clamp is performed, although reaching a glucose level of approximately 150 mg/dL. During the clamp with hypoinsulinemia/hyperglycemia, will be infused a somatostatin analogue (octreotide) for 30 minutes (t = 0 min) and subsequently infusion of 20% glucose to achieve glycaemia of approximately 150 mg/dL (t = 30 min).

Ischemia/Reperfusion Injury The I/R injury is intended to simulate at the brachial artery a condition similar to coronary lesion resulting from infarction after reperfusion. This experiment will be performed 30 min after the measurement of FMD-2, positioning a pneumatic cuff at level of brachial artery, inflating it to 200 mmHg for 15 minutes. After this period of ischemia, the cuff is deflated and is subsequently performed the brachial reactivity test. This ischemia period is safe, as shown in other studies.

Statistical analysis Data will presented as mean ± standard deviation (SD) when normally distributed or median and interquartile range (IQR) for non-normal data. Categorical variables will be compared by chi-square. Continuous variables with normal distribution will be compared using analysis of covariance (ANCOVA) adjusted for age, gender, size of MI estimated by peak CK-MB and estimated by heart rate variability sympathetic activity. Prerequisites for the ANCOVA models (linearity, normality and equal variance) will be checked using histograms, normal probability plots and residual dispersion. Non-normal continuous variables may be transformed logarithmically. If, after the transformation, they persist not normal, will be analyzed by the Kruskal-Wallis test. Analysis will be performed using the SPSS 22 application, Mac version.

Jeffrey Chang MD is conducting a research study to learn more about the increased male hormone levels, otherwise known as androgens, seen in women with polycystic ovary syndrome (PCOS). Women with PCOS have ovaries that are comprised of many cysts, or follicles. They also have irregular or absent menstrual periods and symptoms of increased male hormones, such as facial hair or acne. In each part of the study (except part 4 which is for PCOS women only) we will be comparing responses of PCOS women to normal controls

The first part of this study looks specifically at the structure of the ovaries, which are the female sex glands where both androgens and estrogens (female hormones) are made. It involves imaging your ovaries with a technology called 3-D Ultrasound. We are interested in recording the number, size and arrangement of the follicles in your ovaries.

The second part of the study looks at how the ovary produces male hormones, or androgens, with and without follicle stimulating hormone (FSH) stimulation. Hormones are substances made by a gland in one part of the body which regulate another part. FSH is a hormone naturally produced by the pituitary gland located in the brain and it helps the ovary produce estrogens, or female hormones. LH is a hormone also naturally produced by the pituitary gland and it has the ability to stimulate the ovary to make androgens. We are interested to see how much androgen your ovaries will produce in response to LH with and without FSH. To accomplish this, you will be given FSH as well as hCG, a drug that acts like LH to stimulate the ovary; FSH and hCG are approved by the Food and Drug Administration (FDA) for this investigational, off-label use.

The third part of the study looks at how much the adrenal glands contribute to the increase male hormone levels seen in PCOS. ACTH is a hormone naturally produced by the pituitary gland located in the brain and it stimulates the adrenals to make hormones. The adrenal glands are above the kidneys. They are chiefly responsible for helping the body adjust to stressful situations and work by producing cortisol and adrenaline. The adrenal glands also produce androgens, or male hormones. Previous studies have shown that some women with PCOS produce more male hormones from their adrenals. We are interested to see how much androgen your adrenal glands produce. To accomplish this, you will first be given dexamethasone, a stress steroid, to temporarily suppress your adrenal glands. You will then be given ACTH intravenously over the course of 7 hours to stimulate your adrenal glands.

The fourth part of the study is for PCOS women only and looks at how much the role of insulin contributes to the increase male hormone levels. Insulin is a hormone naturally produced by the pancreas that stimulates all of the cells in your body to take up glucose, or sugar, from the blood. Previous studies have shown that PCOS women who are more resistant to insulin, or whose cells do not take up glucose from the blood in response to insulin, make more male hormones. We are interested to see how much androgen your ovaries produce in response to LH before and after we temporarily decrease the amount of insulin in your blood. To accomplish this, you will again be given hCG, a drug that acts like LH to stimulate the ovary, with and without diazoxide, a drug that decreases the amount of insulin in your bloodstream. These tests will all be done after you are on a diet that limits how much sugar you eat. To test how much insulin you make, you will also be given Oral Glucose Tolerance Tests before and after diazoxide. This test is done by drinking a sugary liquid and testing your blood over 3 hours.

Prolactin is suggested to influence insulin resistance, but scarce data is available on the metabolic profile of patients with prolactinoma.

The purpose of the protocol was to evaluate cardiovascular disease risk factors in women with prolactinoma treated with dopamine agonists and to study the influence of disease control and anthropometry on their metabolic profile.

A cross-sectional study was performed in non-obese premenopausal women with prolactinoma treated with dopamine agonists evaluated regarding glucose, insulin, Homeostasis Model Assessment (HOMA), lipid profile, C reactive protein (CRP), body mass index (BMI), waist circumference, prolactin, estradiol, testosterone, and sex hormone binding globulin (SHBG). They were compared with control women of similar age and BMI distribution.

This is a large nationwide population study, with 10 year follow-up, of the effect of diabetes, metabolic control and a large number of glucose-lowering medications, on total and site-specific cancer incidence and survival.

The study is based on electronic medical records from the largest Israeli health maintenance organization in Israel, Clalit Health Services. 2,301,990 insurees age 21 years old or above at study entry, January 2002 will be included. Four study groups will be established according to the prevalence of diabetes and/or cancer on that date: neither diabetes nor cancer; prevalent diabetes but not cancer; prevalent cancer but not diabetes; both diabetes and cancer prevalence. Subjects free of diabetes at study entry will be followed for diabetes incidence, and all four groups will be followed until December 2012 for study outcomes. The cohort data file will be linked to the Israel National Cancer Registry for cancer morbidity.

We will compare, after adjustment, all and site-specific cancer rates between individuals with and without diabetes; and investigate if metabolic control, as indicated by HbA1c and blood glucose levels, is related to cancer risk. Using time-dependent Cox proportionate hazard models, we will then evaluate differences in outcomes that associate with the use of one or a combination of glucose-lowering treatments, while stratifying by those who were already diagnosed with diabetes at study entry, and those diagnosed during follow-up. Data for a large number of potential confounding variables, including BMI, plasma glucose, HbA1c, hormone replacement therapy and comorbidities will help mitigate allocation bias. The accessibility and uniformity of the healthcare provided by Clalit Health Services, as well as data on cancer screening tests, will minimize the risk of surveillance bias.

All persons between 21 and 90 years and registered with Clalit on January 1, 2002 (the date of study entry), without a previous history of cancer, were included in a closed cohort that was followed until December 31 2012.

All individuals were followed for cancer incidence, which was ascertained by record linkage to the Israel National Cancer Registry, established in 1960. The registry has benefited since 1982 from a national law mandating registration of cancer, and has greater than 95% coverage of solid tumors, and approximately 85% coverage of hematologic cancers \[34\].

Definitions of diabetes: Incident diabetes was defined as fulfillment of at least one of the following six criteria during 2002-12: (1) diabetes recorded in the Clalit Chronic Disease Registry; (2) a physician's diagnosis of diabetes, and one plasma glucose test \> 125 mg/dL within 12 months; (3) one HbA1c measurement \> 6.5%; (4) 2-hour plasma glucose level during an oral glucose tolerance test ≥ 200 mg/dL; (5) 3 or more purchases of glucose-lowering medication within 12 months; (6) 2 plasma glucose measurements \> 125 mg/dL within 12 months. The date of the earliest defining criterion was considered the date of diagnosis.

Prevalent diabetes was defined as being recorded with diabetes in the Clalit Chronic Disease Registry on study entry, or fulfilling criterion #5 above (information on medication use was available from 1998) before study entry.

Types of cancer: The following cancers were investigated: liver (ICD-O-3 codes C22.0 and C22.1), pancreas (C25.0-C25.9), gallbladder (C23.9-C24.9), endometrium (C54.0-C54.9, C55.9), stomach (C16.0-C16.9), kidney (C64.9, C65.9, C66.9., C68.0-C68.9), benign brain and central nervous system (C70.0-C72.9 with behavior code 0), malignant brain (C70.0-C72.9 with behavior code 3), multiple myeloma (C42.1 with relevant morphology codes), colorectum (C18.0-C21.8), non-Hodgkin lymphoma (C02.4, C09.8. C09.9, C11.1, C14.2, C37.9, C42.2, C77 and all other sites for extra-nodal Non Hodgkin's lymphoma), lung (all: C34.0-C34.9), adenocarcinoma, squamous cell), leukemia (C42.0, C42.1, C42.2 with relevant morphology codes), ovary (C56.9), bladder (C67.0-C67.9), breast (C50.0-C50.9), thyroid (C73.9) and prostate (C61.9). Apart from benign brain tumors, all cancers included in the analysis were coded as invasive (behavior code = 3).

Diabetes, Glucose Control, Glucose Lowering Medications, and Cancer Risk: A 10-year Population-based Historical Cohort

The population will be followed historically for a period of up to 11 years whichever came first, cancer incidence, mortality, age 90, or end of follow-up.
