Gastric signet ring cell carcinoma (GSRCC) possesses unique epidemiology and pathogenesis in the field of cancer, but its incidence is low. Unfortunately, there is currently a lack of systematic research focusing on the prognostic proteomic features of GSRCC. Given this knowledge gap, this study aims to comprehensively characterize the proteomic landscape of GSRCC using a reliable and reproducible DIA-PCT method. This study objectives include characterizing the heterogeneity of GSRCC, performing molecular typing, identifying potential biomarkers and therapeutic targets, and providing a resource for stratified analysis of GSRCC. To achieve these goals, the investigators selected a cohort of 112 GSRCC patients from a pool of over 10,000 gastric cancer patients and conducted a proteomic analysis using the DIA-PCT method. This meticulous approach revealed four novel proteomic subtypes of GSRCC, each exhibiting unique molecular characteristics. Additionally, the investigators discovered that PRDX2 and DDX27 can serve as predictive biomarkers for GSRCC, which were further validated in an independent cohort of 75 GSRCC patients. Furthermore, the investigators paid particular attention to the MLT-GSRCC subgroup and identified three distinct proteomic clusters among MLT-GSRCC patients. Subtype 2 within this subgroup demonstrated the poorest prognosis. Through a rigorous screening process, the investigators determined potential targets for the treatment of GSRCC. In conclusion, these findings contribute to the investigators understanding of the heterogeneity of GSRCC and provide valuable resources for future clinical stratification and targeted treatment strategies.

Mechanism of Gastric Signet Ring Cell Carcinoma Based on Microproteomics

This study does not involve drug clinical trials but focuses on ensuring the confidentiality of patient information. To protect patient privacy, a coding system was used instead of real names when entering patient information such as names and ages. Each patient was assigned a study identification number upon enrollment. Throughout the entire study, only the main researchers in the research team have the authority to access the true personal data of patients obtained from the electronic database (such as names and ages), and it is strictly prohibited to circulate or share this information. During the publication of the research article, the main researchers in the research team will use sample coding to replace patient privacy information (such as names) to achieve de-identification. Additionally, the protection of personal information for study participants must be maintained throughout the entire research process, and it is strictly forbidden to circulate or leak such information.

The samples diagnosed as GSRCC by pathology were selected as the samples with signet ring cell content \> 70% as judged by 2 pathologists of associate chief physician or above and without previous radiotherapy and chemotherapy. Tumor tissues and paired nat were collected from the same patient at the time of tumor resection and stored in formalin-fixed paraffin embedding (FFPE). Liquid chromatography-mass spectrometry (LC-MS) was used to detect the proteomics in the samples. The proteins in the samples were extracted, and the proteomics in the samples was detected by LC-MS. Qualitative and quantitative analysis was performed to detect the types and content of proteins in the samples.

Patients diagnosed with Oligometastatic Cancer (OC) that will receive Stereotactic ABlative Radiotherapy (SABR) that has been prescribed per clinical protocol.

Individuals who have never been diagnosed with cancer or oligometastasis.

Analyze clinical data, medical images and molecular circulating features to propose which variables serve to select the patients that will achieve complete response after SABR.

To propose a diagnostic test to determine the viability of Stereotactic ABlative Radiotherapy (SABR).

Collect blood samples at five different time points before and after treatment and analyze circulating tumoral DNA, medical images, and metabolites to monitor the effect of SABR and its effectiveness.

To propose a diagnostic test to determine the effectiveness of SABR.

Analyze circulating tumoral DNA and metabolites from blood samples in search of the profile of OC.

To propose a diagnostic test for Oligometastatic Cancer (OC).

Metastases represent the most threatening challenge in cancer. One of the management strategies for patients with Oligometastatic Cancer (OC) is Stereotactic ABlative Radiotherapy (SABR). However, there are few studies, and there is no defined clinical standard, nor are the radiobiological mechanisms that contribute to treatment response well understood. The focus should be on generating evidence to guide the personalization of radiotherapy beyond solely technological and anatomical precision. This could be achieved by recollecting clinical and biological data from patients that undergo this treatment and analyzing them to ultimately predict, with the help of artificial intelligence, which patients will be the most beneficiary and improve their survival rate.

Metastases are the most threatening challenge in cancer. In patients with metastatic cancer, local radiotherapy treatment remains an essential tool with different goals that depend on numerous factors, especially on the number and extent of the metastases and whether disease control is feasible and desirable according to the expected quality of life. Oligometastatic Cancer (OC), i.e., a few metastases in a few organs, has been recently incorporated as a less aggressive state than widely disseminated metastatic disease. Consequently, OC is a serious candidate for aggressive treatments based on Stereotactic ABlative Radiotherapy (SABR). This treatment has shown promising results and is already incorporated into habitual clinical practices. However, OC is a complex and heterogeneous disease, and not all patients have improved their life quality and expectation. Identifying patients who would benefit from this treatment is an important area of research that needs factual information from a large sample provided by multiple centers. Therefore, this multicenter, retrospective, prospective, observational, and longitudinal study incorporates clinical data, medical images, and biological samples to feed artificial intelligence algorithms. The objective is to determine which patient profile achieves complete response after SABR. The secondary objectives are:

1. To analyze metastases by radiomics using computed tomography, magnetic resonance, or positron emission tomography images; and
2. To evaluate intratumoral metabolites released into circulation by multi-omics analyses.

These will contribute to determining the systemic molecular effects of treatment in search of biomarkers with predictive value.

Centralized storage of patient management data, clinical image analysis, and candidate biomarkers measured in blood samples may eventually meet the expectations of integrating data into clinical decision-making and influence evolution based on precision medicine.

The Impact of Radiotherapy on Oligometastatic Cancer

Stereotactic ABlative Radiotherapy (SABR) in Oligometastatic Cancer (OC): a Radiomics, Multi-omics, and Machine Learning Approach to Clinical Decision-making. The OC-SABR Multicentric Project

The Data Management Plan makes data fully findable, accessible, interoperable, and reusable, following the indication of the Horizon 2024 initiative of the European Union. The clinical team identified sensitive data, including epidemiological, anthropometric, and medical information. It is the only responsibility of the principal investigator to ensure that sensitive data are de-identified, implementing technical safeguards to guarantee anonymity.

Most data will be experimental and obtained from the analysis of column value and data format description (.txt or .csv) and syntax scripts (.R).

The external collaborators, especially those involved in validation cohorts, may have access to data upon request.

With the acceptance of the principal investigator, Rovira i Virgili University's institutional service will guarantee digital access to repositories with raw data generated in research analyses.

Images obtained by computerized tomography, magnetic resonance and positron emission tomography serve to evaluate the local control and response rate by:

* Complete response: absence of disease.
* Stable disease: no changes.
* Partial response: 50% or more injury reduction.
* Progression: 25% or more tumor size increase.

According to RECIST Criteria 1.1 and PERCIST.

To assess the impact of SABR on the progression over time, precisely at 1, 2, 3, and 5 years, regarding Progression-Free Survival (PFS) based on RECIST Criteria 1.1 and PERCIST.

To evaluate the progression of SABR toxicities over time, precisely at 1, 2, 3, and 5 years, according to the CTCAE 4.0 scale.

Background:

- The Chinese University of Hong Kong and the National Cancer Institute are conducting a breast tissue study in connection with another study on breast health. This larger study will look at breast health in women who live in Hong Kong. Women in the breast health study will provide a breast tissue sample during biopsy surgery. Researchers want to use part of the sample to study changes in breast tissue that are related to breast diseases. This sample study is part of the larger study.

Objectives:

- To look at changes in breast tissue that are related to breast diseases.

Eligibility:

* Women who are participating in the Hong Kong breast health study.
* Participants must be having a biopsy to remove breast tissue for study.

Design:

* Participants will have a biopsy to remove a breast tissue sample. Most of the sample will be used for diagnosis. A portion of the leftover tissue will be used for this study.
* No extra surgery will be required.
* Treatment will not be provided as part of this study.

Background: Breast cancer incidence is increasing rapidly among many Asian populations primarily due to the adaptation of a Westernized lifestyle. However, Asian women seem to have a distinctive profile of breast cancer that differs from that seen in Western populations, such as earlier age at onset, more advanced stage, and higher frequency of ER-negative tumors. The disproportionally high rates among younger women were also seen among U.S.-born Asian women, such as Chinese, Filipina, Indian, and Pakistani women. These findings suggest that Asian breast cancer may have a distinct etiology that is not fully explained by adapting a westernized lifestyle. Recent molecular studies have suggested that breast cancers comprise several biologically and clinically distinct tumor subtypes. Epidemiologic evidence suggests that associations with breast cancer risk factors may vary by tumor subtype. However, most previous studies were conducted in Western populations; large, population-based epidemiologic studies characterizing molecular subtypes of breast cancer in Asian women are limited. In this study, we would like to characterize the molecular profile of breast cancer among Chinese women in Hong Kong and associate the molecular features with clinical and etiologic factors. An improved understanding of the inter-relationships among age, breast cancer subtypes, and race/ethnicity is critical for the development of targeted prevention and the best and most efficacious therapy.

Objectives: To assess the technical feasibility of 1) collecting fresh frozen tissues and fixed tissue blocks from breast cancer cases in Hong Kong, and 2) conducting tumor profiling analyses in the collected tissues.

Eligibility: Breast cancer cases who are participants in an ongoing breast cancer case control study among Hong Kong women (Night shift and breast cancer: a case-control study among Hong Kong women, led by Dr. Tse at Chinese University in Hong Kong) and who will receive surgery at the North District Hospital, HK, in 2013.

Design: In this feasibility study, we plan to collect fresh frozen and fixed tissues (tumor and adjacent normal) from approximately 100 breast cancer cases who have given consent to participate in our breast tissue collection pilot study. DNA and RNA will be extracted from frozen tumors and assessed for quality and yield. We will then conduct expression profiling analysis in a subset of cases (N approximately 10) to determine the quality of RNA in molecular profiling analysis. We will also determine whether there are enough epithelial benign and tumor tissues for the construction of tissue microarrays (TMAs). The results from this feasibility study will inform our decision on whether or not to pursue a large scale study.
