Qualitative interviews

This interview and focus group study will explore patient, caregiver, and health professional opinions about the potential roles of digital technology to support effective cancer pain management. The views, opinions, and ideas expressed in this study will be used to design and develop a digital intervention which is likely to take the form of a measurement (pain and analgesic use) guided medication management intervention to improve cancer pain management in the community.

Cancer is increasing in incidence and prevalence in the United Kingdom. Pain is the most frequent complication of cancer and cancer pain control is frequently suboptimal. Previous research has established several potentially modifiable reasons for suboptimal cancer pain management. These include: under-reporting of pain; inadequate communication about pain between patients and health care professionals ; inadequate assessment of pain by health care providers; inadequate analgesic prescribing; suboptimal adherence to prescribed analgesic regimens by patients; and failure to re-assess pain timeously.

Information and communication technology (ICT) is a term used to describe all digital technologies that facilitate the electronic capture, processing, storage, and exchange of information. ICT could offer innovative solutions to suboptimal cancer pain management. Technological solutions might include electronic diaries, available as applications or "apps", which could encourage pain self-monitoring by people with cancer, and prompt medical assessment when pain is poorly controlled. Accurate patient reported pain data could help professionals to adequately assess pain and might influence the patient-professional consultation. Feedback of adherence data to patients and carers might improve the way that medications are used. Electronic medication adherence devices are currently available which can capture data on analgesic utilisation and timing. This data might be useful for health care professionals to inform safe and timely adjustments of analgesic doses (i.e. is pain control poor because of inadequate analgesic dose or because of non-adherence to the prescribed regime?). The aim of the proposed study is to explore the desirability, feasibility, and practicalities of using ICT to improve cancer pain management The proposed study would seek the views of patients with cancer pain, their caregivers, and health care professionals, about using technology to help with cancer pain management. The study will investigate the acceptability of electronic pain diaries and electronic medication adherence devices to patients and health care professionals, barriers to using such technology, and how data generated from technology might be used by patients, caregivers, and health care professionals to improve cancer pain management. This study will form part of a programme of research and data from the proposed study will be used to inform the potential development of a technological intervention to improve the management of cancer pain in the community.

Participants will receive a geriatric assessment which consists of validated questionnaires that are used to assess health status of older adults with cancer. Based on participant responses, tailored recommendations will be provided.

The Delphi method, a process used to arrive at a group opinion or decision by surveying a panel of experts, will be used with experts in geriatric oncology to develop a supportive care management intervention for older caregivers of older adults with cancer.

Older caregivers of older adults with cancer (described here-on as "older caregivers") are at high risk for increased burden and negative physical and psychological outcomes. Despite this, identification of older caregivers at risk for burden remains a challenge and evaluation of supportive care interventions and guidelines for this population remains limited. Health outcomes of older adults with cancer and their caregivers are inter-related and it is important to evaluate and support the health of both. The geriatric assessment (GA) is a valuable tool to evaluate health and age-related conditions (e.g., physical and cognitive function) of older adults with cancer and guide supportive care. Identifying older adults with age-related conditions can help to identify caregivers who are at increased risk for the negative impacts of caregiving. This project seeks to gather preliminary data on the health of older caregivers and to develop and pilot high priority supportive care interventions for this population. The investigator will examine the feasibility of utilizing the GA as a tool to identify distressed older caregivers. The investigator will engage key stakeholders including, older caregivers and older adults with cancer, experts in geriatric oncology and primary care physicians to develop and pilot a supportive care intervention to improve outcomes in older caregivers.

Development and Feasibility of a Supportive Care Intervention for Older Caregivers of Older Adults With Advanced Cancer

A complete and final study protocol will be made publicly available through the University of Rochester Cancer Center Community Oncology Research Program Research Base Protocol and Data Sharing Committee. The full protocol and data will be made publicly available no later than the publication date of the study findings from the final dataset. The protocol will include a detailed description of the study population, hypotheses tested, measurement and assessment information, data definitions and codes, and the analysis plan utilized. We will be collecting identifying information. The final dataset will be stripped of identifiers prior to release for sharing. Published papers will be made available in portable document format.

Percentage of participants who were approached, enrolled in the study and completed the GA will be determined.

Time to complete the GA per caregiver will be determined by summing the time to complete self-administered and clinic portions of GA.

We will conduct semi-structured interviews with target patients and patients' representatives to assess their decisional needs. Decisional needs including difficult decision type /timing, unreceptive decisional stage, decisional conflict (uncertainty), inadequate knowledge \& unrealistic expectations, unclear values, inadequate support \& resources, and Personal \& clinical needs. All interviews will be conducted one-to-one or one-to-many (including families) and be audio-recorded for further analysis. Written informed consent will be obtained prior to each interview.

Nonrandom purposive sampling will be used to select key respondents to conduct in depth interview. Snowball sampling, where potential participants are asked to identify others who may be willing to participate, as well as convenience sampling, will be utilized. For patients, aiming for diversity regarding age, sex, education level and annual household income. All participants should be Chinese-speaking.

The purpose of Alpha test is to test the understandability of Decision Aid for Lung Cancer Molecular Testing version 1.0 (DA_LCMT 1.0). All debriefing interviews will be conducted one-on-one with structured probing questions. Participants will be encouraged to comment on DA_LCMT 1.0 and give recommendations to replace any unclear wording. At the start, the interviewer will explain the aim of the study and the procedures of the cognitive debriefing to participants. Then a paper-based DA_LCMT 1.0 will be given to participants, and sufficient time will be guaranteed to allow them to read DA_LCMT 1.0. The cognitive debriefing interview will start afterwards, and participants will answer questions probing questions asked by interviewers about DA_LCMT 1.0. Each cognitive debriefing interview will last for about 20 minutes. DA_LCMT 2.0 will be generated after the completion of the cognitive debriefing (Alpha test).

Then the investigators will conduct the field testing (Beta test) with patient and clinician facing the decision in real time. The purpose of Beta test is to test the usability of DA_LCMT 2.0 in "real-world setting". Clinician and patients will use the DA_LCMT 2.0 for real-time decision making. The conversation of decision progress will be audio-recorded. After the decision is made by patients, structured interview will be conducted and audio-recorded separately between clinician and patients. All the field testing interviews will conducted one-on-one and written informed consent will be obtained prior to each interview. Each field testing interview will last for about 20 minutes.

We will conduct semi-structured interviews with target patients, patients' representatives and other stakeholders to assess their decisional needs. The objective of this study is to develop a Decision Aid for Lung Cancer Molecular Testing (DA_LCMT) and to facilitate shared-decision making in patients who were diagnosed with lung adenocarcinoma by intraoperative frozen pathology about whether to conduct the molecular testing.

Under the guidance by Ottawa Decision Support Framework (ODSF), International Patient Decision Aid Standards (IPDAS) and FDA guidance for Identify what is important to patients, the investigators use semi-structured interviews qualitative research methods to generate the decision making needs items of lung cancer patients who were diagnosed with lung adenocarcinoma by intraoperative frozen section pathology and other stakeholders (including thoracic surgeons, nurses, hospital administrators, molecular testing company staffs and insurance company staffs). Then modified Delphi will be used to develop the Decision Aid for Lung Cancer Molecular Testing version 1.0 (DA_LCMT 1.0). One-to-one structured interviews qualitative research methods will be used in the Cognitive debriefing (alpha tests) and Field testing (β tests) to revise and improve the DA_LCMT from version 1.0 to finally version 3.0. Descriptive statistics will be used to summarize the baseline characteristics of patients and other stakeholders. Qualitative data will be analyzed by the three steps proposed by grounded theory.

We will use a prospective, observational cohort design, we will invite a sample of individuals who have indicated willingness to be re-contacted for future studies (from existing protocols involving cancer survivors and their unaffected relatives employing mixed methods -qualitative interviews coupled with validated measures - to assess: the proportion of participants experiencing psychological distress from Whole genome/exome sequencing (WGS/WES) results.

We will also recruit an additional group of participants from the general public (with or without a cancer history) who have not had their genomes or exomes sequenced to participate in focus groups to inform us about their perceptions of the hypothetical utility of learning of incidental results from their genome or exomes. For our sampling purposes, this group of participants is referred to as the 'focus group participants (sample #3-hypothetical group)

A week before the participants return to the clinic to learn of their results, the RSA will call each participant to complete the Hospital Anxiety \& Depression Scale (HADS), revised Impact of Events Scale (IES-R), \& a questionnaire about their health behaviors, to establish baseline distress levels \& health behaviors. A week later, participants will return to the Clinical Genetics Service to review their results with the genetics provider \& discuss resultant therapeutic \& management recommendations for the participants \& their relatives. A week later, the RSA will call each participant to complete the HADS, IES-R again, to establish the safety of receiving these results. Participants will also be asked to complete the revised Multidimensional Impact of Cancer Risk Assessment (MICRA) measure. The RSA will also invite participants to complete an in-depth telephone interview.

This study uses new methods called "genome sequencing" that allow the investigators to study part or all of a person's genome. The genome is the collection of all of a person's genes. Genes carry the instructions that our bodies need to develop and function. Genes are passed on from one generation to the next. Genome sequencing can study all of a person's genome (whole genome sequencing) or just parts of their genome (whole exome sequencing). In the study, the investigators refer to all these research methods as 'genome sequencing'. Genome sequencing typically shows a large number of gene changes, known as "variants." Some (but not all) of these genetic variants may be linked to increased risks of diseases other than cancer.

The purpose of this study is to learn what kinds of genetic variants the patient wants to learn about from their genome.

Participants will be recruited from cohorts of patients whose DNA samples are being examined with WGS/WES as part of efforts to identify novel cancer susceptibility alleles. Participants will be derived from the following protocols: 09-068 and 96-051.

Subjects will participate in a qualitative interview lasting up to 90 minutes. The interviews will include a concept elicitation component to explore details of the subject's perceived disease condition, functional limitations and impacts on daily life and health-related quality-of-life (HRQoL). The objective of this study is to conduct in-person qualitative interviews of subjects who have different tumor types to identify and assess relevant PROMIS physical function items that can be utilized in future studies. Health Research Associates (HRA) will conduct qualitative interviews in subjects with a variety of cancer types. PROMIS is a set of self-report measurement tools, developed by the United States (US) National Institutes for Health (NIH). The most recent version of PROMIS Physical Function Item Bank contains 165 items assessing a range of abilities and its subset, a 45-item PROMIS Function Cancer Item Bank which contains questions relevant to subjects with cancer. Approximately 150 subjects with five tumor types will be recruited. HRA will conduct the interviews that will be audio-recorded for transcription and analysis. Five tumor types that will be focused on are Breast, Prostate, Non-Small-Cell Lung Cancer (NSCLC), Multiple Myeloma, and Diffuse Large B-Cell (DLBCL) or Follicular Lymphoma. Interviews will be conducted in English language in private areas within clinic site or rented meeting facilities and will last approximately for 60-90 minutes.

The Patient-Perspective of Complications After Colon and Rectum Surgery: A Qualitative Analysis

Information gathered from these interviews will form the basis for the conceptual model of surgical complications. Qualitative data will be obtained from patients who have directly experienced a surgical complication. These complications are defined and categorized by the Common Terminology Criteria for Adverse Events (CTCAE) version 4.0.

This is a qualitative interview study that aims to understand treatment burden in individuals who have experienced prostate or colorectal cancer treatment within the past five years. We intend to use patient and caregiver experiences to co-design interventions to optimise cancer aftercare.

Treatment burden is the workload of healthcare for patients and the consequences of this workload on patient function. Treatment burden has been associated with negative outcomes in stroke, heart failure, diabetes, and renal failure. Cancer is increasingly becoming a chronic condition, and involves a variety of self-management tasks for patients and their caregivers. In this study investigators will investigate treatment burden in people after prostate and colorectal cancer. Investigators will seek to understand patient and caregiver perceptions about cancer aftercare, and ways that services could be redesigned and improved to reduce treatment burden, and improve patient outcomes.

We will undertake a qualitative interview study, recruiting patients from general practices and oncology outpatient clinics who have completed potentially curative treatment for prostate or colorectal cancer, or who are on active surveillance or hormonal therapies for localised or locally advanced prostate cancer. We will purposively sample, to ensure that participants with comorbidities, those from lower socioeconomic groups, and rural dwellers are adequately represented. We will conduct interviews according to a schedule, informed by conceptual models of burden of treatment, Schwarzer's Health Action Process Approach, and Normalisation Process Theory.

Interviews will be filmed and/or audio-recorded and transcribed. Framework and thematic analysis will be used to analyse and synthesise the data. Participants will be given the chance to comment on outputs and findings (triangulation).

Investigators plan to use the results of this study, and excerpts from video interviews during co-design events, and to create new interventions to optimise aftercare for patients with prostate and colorectal cancer.

The number of individuals surviving for more than five years following a cancer diagnosis is steadily rising. Cancer is often a long-term condition. Individuals living beyond a cancer diagnosis are required to participate in a range of self-management activities, including (and not limited to) managing symptoms and comorbidities, self-monitoring for recurrence, adjusting to life beyond cancer/managing psychological sequelae, taking medications, attending follow-up appointments, and addressing unhealthy behaviours.

Treatment burden is the workload of healthcare for patients, and the effect that this work has on patient functioning and well-being. In the 1960s, Thomas Creer recognised that patient participation in their own healthcare was important in achieving disease control in paediatric asthma. Since then, "self-management" has been extensively researched and conceptualised. In the 1980s, Corbin and Strauss characterised three strands of "work" for people with chronic conditions: medical management, for example adhering to medication regimens and attending appointments; behavioural management, such as adopting healthier lifestyle practices; and the emotional management of adapting to a long-term condition.

Increasing patient participation in self-management has mostly been viewed as a positive strategy through which to improve health outcomes, reduce medical paternalism, and delegate some tasks away from over-burdened health and social care systems. Treatment burden is a relatively recent concept which highlights the potentially deleterious effects of increasing patient workload. Treatment burden is becoming increasingly relevant as more people survive into old age, often with combinations of comorbidities (including cancer) for which there are multiple available therapeutic agents and complex self-management regimens.

In diabetes, renal failure, heart failure, and stroke, there is increasing evidence that increased treatment burden is associated with reduced quality of life, non-adherence to therapeutic regimes, and wasted resources. Treatment burden is modified by patient capacity: those with lower self-efficacy, low levels of social support, financial difficulties, and competing personal workload can experience increased treatment burden. In cancer, certain large and distinct demographic groups, particularly rural dwellers and the socially deprived, have poorer survival after a cancer diagnosis. The mechanisms of these inequities have not been adequately explained.

It is feasible that treatment burden could modify interactions with health services, engagement with survivorship activities, and ultimately, cancer outcomes.

Treatment burden is a relatively recent concept which is under-explored in in cancer survivors.

The objectives of this study are:

1. To investigate patient perceptions of treatment burden after prostate and colorectal cancer.
2. To determine sources of treatment burden in prostate and colorectal cancer survivors, their interactions, and the perceived consequences of treatment burden.
3. To utilise key concepts from the interviews to build a video narrative, featuring video excerpts from participants to ensure that their voices reach a wider audience, including professionals. We will use these narratives to improve cancer services through co-design.

Qualitative interview will be conducted with approximately 40 patients, with or without linked caregivers. They will be transcribed verbatim, and analysed using Framework and thematic analysis.
