This project will build upon prior, successful research on the effectiveness of an interactive computer disk with readable only memory (CD-ROM) for educating women about breast cancer and genetic risk. Prior studies found that an interactive CD-ROM for educating women about breast cancer risk and genetic testing was effective. The program was well received by lay persons and professionals. In a randomized controlled trial conducted at multiple sites, the CD-ROM intervention was highly effective in increasing knowledge, especially among women at low risk of carrying a breast cancer susceptibility gene 1/2 (BRCA1/2) mutation.

Researchers now propose to expand the use of the interactive CD-ROM in two innovative ways. The overall goal of this proposal is to evaluate the program as a first-line educational approach for Hispanic women on the Texas-Mexico border, where educational resources about cancer genetics are limited. First, researchers will modify and adapt the program for a primarily Hispanic population, in order to make it culturally and linguistically appropriate to the needs of that audience.

Second, researchers will evaluate the program as a first-line educational method among women with a personal or family history of breast cancer. Researchers will compare the effectiveness of the CD-ROM when implemented with and without the guidance of a trained promotora, and in comparison with standard educational materials (usual care condition).

The specific aims of this study include:

Aim 1: To modify the interactive CD-ROM to: a) make it culturally and linguistically appropriate for Hispanic women residing along the Texas-Mexico border; b) reflect current knowledge about breast cancer genetics; c) add a module to help women prepare to discuss their family history and cancer risk with a health care provider (n=50 participants).

Aim 2: To conduct a randomized, controlled evaluation that compares the efficacy of the modified CD-ROM when used alone vs. when used with a promotora-assisted approach vs. standard written materials (n=414 participants).

This study will test ways to help improve Hispanic women's knowledge and understanding about breast cancer risk and genetic testing for inherited cancer risk. These methods include providing printed educational materials, using an educational computer program, and using the computer program with the assistance of a lay health worker (a promotora).

If you agree to take part in this study, you will complete a questionnaire that will ask about your personal health as well as your knowledge and attitudes toward cancer and genetic testing. After the questionnaire is completed, you will be randomly assigned (as in the roll of dice) to 1 of 3 study groups. One group will be asked to read educational materials. Another group will use an educational computer program to learn about breast cancer risk and genetic testing without guidance. Another group will use the computer program with the guidance of a promotora.

Each of the 3 study groups should take between 30 and 60 minutes to complete.

Immediately after participating in your assigned group, you will be asked to complete another questionnaire that will ask about your knowledge and attitudes toward cancer and genetic testing. You will also be contacted later by telephone to complete similar questionnaires at about 1 month and 6 months after your study visit. Some women will be asked to complete an additional questionnaire by telephone 1 week after reading the educational materials or the computer program, and researchers will ask for permission to audiotape this interview. It will take about 20 minutes to complete each of the questionnaires used in this study.

This is an investigational study. Up to 414 women will take part in this study.

Patients will be randomized to receive either culturally adapted AI materials or non-culturally adapted SI materials. Intervention materials will be mailed to patients every 2 weeks. Based on the results of the fitness tests, a specific exercise program will be presented. The research staff will call about every 2 weeks to check that materials were received and have been sent back whatever materials are due. The phone call should take no more than 15 minutes. At 6 months after the follow-up visit, a packet of follow-up questionnaires about physical activity and quality of life will be mailed or given over the phone. The questionnaire packet will be mailed with a postage-paid return envelope. It should take about 10 minutes to complete the questionnaires.

Patients may choose to participate in the exercise program after 16-week assessment. After 4 months of exercise, all the follow-up tests repeated. Exercise recommendation then given, and all of the intervention materials.

Complete fitness tests to measure strength, flexibility, balance, and aerobic capacity. All of the baseline fitness tests will be repeated at the end of the program (at about the 16th week) during a follow-up visit.

Baseline Fitness Test

At the baseline visit, complete questionnaires about demographic information taking about 35 minutes to complete. At 6 months after the follow-up visit, a packet of follow-up questionnaires about physical activity and quality of life will be mailed or given over the phone. It should take about 10 minutes to complete these questionnaires. Patients also be asked to fill out a questionnaire every week that asks about level of physical activity. The questionnaire should take about 5 minutes to complete.

The exercise programs will be individualized based on results from baseline fitness assessments and individual medical history using ACSM guidelines. For resistance and flexibility exercises each patient will receive an exercise prescription and instructional session prior to beginning the program. Invitation extended to attend monthly Group Exercise sessions with a family member. Resistance tubes, pedometer, exercise book, and video issued for use at home.

Individualized Exercise Program

Either culturally adapted AI materials or non-culturally intervention materials will be mailed to patients every 2 weeks. The 7 themed sessions are: Getting Started;Goal Setting;Effective Energy Balance;Getting Confident;Uncovering Barriers;Lapsing, Relapsing, Collapsing;and Continuing to be Active.

The goal of this research study is to test an exercise program that is designed for Hispanic breast cancer survivors. Researchers want to learn the most effective strategies for promoting exercise.

Study Summary:

If you are found to be eligible to take part in this study, you will come in for 2 visits to complete fitness testing and questionnaires at the Behavioral Research Treatment Center (BRTC) at MD Anderson. During the week before your first visit to the BRTC, you'll wear an accelerometer at home. An accelerometer is a small device that you wear on your waist during the day. It records how active you are. The accelerometer will be mailed to you or given to you in clinic ahead of time by a staff member. You will bring the accelerometer with you to your first visit to the BRTC. This will be repeated at the end before your follow-up assessment visit. The visits should last about 2 hours and 30 minutes. You will be asked to exercise about 3-5 times every week for 16 weeks at home for at least 30 minutes. You will also be invited to group exercise sessions every month. You should attend at least 2 of these sessions. A family member or friend is invited to attend the group sessions with you. Six (3) months after the follow-up visit, you will complete follow-up questionnaires.

Study Visits:

At the baseline (first) visit, your blood pressure, heart rate, height, and weight will be measured. Your waist and hip will be measured. A tool that measures your skin thickness will be used on the back of your arm, above your hip, and on your thigh. Your arms will be measured by placing your arm in a tool used for measuring arm volume.

You will complete questionnaires about your demographic information (such as your age), medical history, any drugs you may be taking, your current level of physical activity, your quality of life, and your environment and exercise (such as how many people you see exercising daily). The questionnaires should take about 35 minutes to complete. You may complete these questionnaires either when you sign the informed consent form or at your first visit.

Blood (about 1½ tablespoons) will be drawn during your baseline and 4-months visits after you fast (have nothing to eat or drink but water) for 12 hour. These samples will be stored for future testing on biomarkers. Biomarkers are found in the blood and may be related to breast cancer risk.

You will complete fitness tests to measure your strength, flexibility, balance, and aerobic capacity (how effectively you breathe in air while you are exercising).

For the strength tests, you will pull on a bar that is connected to a sensor that shows how much force you are using. You will pull on the bar as hard as you can for 3 seconds. You will have at least 10 seconds of rest between efforts. You will do pulls to measure the strength of your arms, shoulders, and upper body. To measure your hand and forearm muscle strength, you will squeeze and release your hand on a hand tool. To measure your leg strength, you will stand and sit back down in a chair as many times as you can in 30 seconds. You will be shown how to do each of these tests.

For flexibility, you will perform a "sit-and-reach" test and a "stand-and-reach" test. Your hip and lower back flexibility will be measured using a "sit-and-reach" flexibility box. You will sit on the floor with your feet placed on the edge of the box. You will reach forward with your hands as far as possible 3 different times. The farthest reach will measure your lower body flexibility. To measure your upper body flexibility, you will do a "stand and reach" test. You will stand next to a wall with your arm stretched forward. Then you will reach forward as far as possible without your feet lifting off the ground. The distance reached forward will be the measure of your upper body flexibility. Range of motion of your arm will be measured by asking you to lift your arm upward with your thumb pointing up. This will be done on the side and in front of you.

For aerobic testing, you will be asked to walk on a treadmill. Before you exercise, you will have an electrocardiogram (ECG) to check your heart function and your heart rate and blood pressure will be measured. A nose clip and mouthpiece will be put on you. During exercising, you will have an ECG. Your blood pressure will also be measured every few minutes. You will be asked at several times to point to a scale that measures your overall level of effort. You will walk on the treadmill at increasing speeds and inclines (hills) until you reach a point of about 85 percent of your maximum heart rate. At that point, the treadmill will be slowed down and you will be allowed to walk slowly and recover. You can ask to stop the treadmill at any time. The reason for doing this test is to see how your body reacts to hard exercise at an intensity higher than will be recommended for you during the study when you will be exercising on your own. You will be asked to perform a 6-minute walk test as a warm-up. This test measures the distance that you can walk quickly on a flat, hard surface for 6 minutes.

All of the baseline fitness tests will be repeated at the end of the program (at about the 16th week) during a follow-up visit. At the follow-up visit, you will also be asked to complete a feedback questionnaire and you will be interviewed for your feedback on the newsletters. This will help researchers design future research studies. The questionnaire and interview should take about 15 minutes to complete. You will also be asked to answer a Food Questionnaire at the follow-up visit. This questionnaire will include questions about your usual food intake during the last year. It will take about an hour to complete this questionnaire.

Study Groups:

After your baseline fitness tests and questionnaires, you will be randomly assigned (as in the flip of a coin) to 1 of 3 study groups. Groups 1 and 2 will be given an individualized exercise program, have the same amount of contact with research staff, and receive written materials about exercise. The only difference between the groups is that they will be given different written materials. Group 3 will be a "waiting list" group who choose to participate in the exercise program after their 16-week assessment. After 4 months of exercise, they will repeat all the follow-up tests described above, and receive an exercise recommendation and all the intervention materials. You will have an equal chance of being assigned to one of the three groups. You will know if you are in the waiting list group (3) or one of the exercise groups (1 or 2), but if you are in an exercise group you will not know which one.

Exercise Program:

Based on the results of the fitness tests, a specific exercise program for you will be presented to you. The comprehensive program will include aerobic (cardiovascular) training, strength training, and flexibility training to be completed at home. A set of resistance tubes will be given to you to keep and use at home. You will also be given a booklet and DVD video that contain the exercise. If you would like, after the follow-up visit, a long-term suggested exercise program will be given to you.

You will be given an exercise log to keep track of your exercising. You will also be asked to fill out a questionnaire every week that asks about your level of physical activity. The questionnaire should take about 5 minutes to complete. Every month, you will be asked to fill out a questionnaire about any swelling you may have had. This questionnaire should take about 5 minutes to complete. Every month, you will mail your completed questionnaires and exercise logs back to the study staff in postage-paid envelopes that will be provided to you.

Newsletters and Study Contact:

Every 2 weeks, you will receive a newsletter on a different theme related to exercise. One of the newsletters will be about healthy eating. The wording of the material will be different between groups, but the overall theme will be the same. The research staff will call you about every 2 weeks to check that you have received the materials and have sent back whatever materials are due. The phone call should take no more than 15 minutes. However, you can ask research staff any questions about the program at any time using the contact information that will be given to you.

Follow-up Questionnaires:

At 3 months after the follow-up visit, a packet of follow-up questionnaires about your physical activity and quality of life will be mailed to you or given over the phone. The questionnaire packet will be mailed with a postage-paid return envelope. It should take about 10 minutes to complete the questionnaires. The research staff will call you to check that you have received it and encourage you to complete it. If you are coming to MD Anderson for any appointments, you will receive the packet in person instead.

Length of Study:

Your study participation will be over after you complete the follow-up questionnaires. Your study participation may be stopped early if you become injured, if you become ineligible (for example, if you need to have treatment), or if the research staff thinks it is in your best interest.

Part II: Exercise in Hispanic Breast Cancer Survivors

Exercise behavior measured as frequency of exercise as assessed by the IPAQ score. Multiple linear regression used to test for differences in the changes between groups adjusting for relevant covariates, such as baseline activity level (IPAQ score) and BMI. Linear mixed model analysis also performed for weekly change in exercise behavior. The continuous score (MET minutes/week) used as primary physical activity outcome, measured at baseline and follow-up. Compliance measured using exercise logs, which will record resistance exercise and pedometer steps.

Blood (about 1½ tablespoons) collected at baseline and 4 months visits following a 12-hour fast. Blood will be processed and stored for later biomarker analysis - body adiposity, sex hormone binding globulin, estradiol, adipokines (adiponectin, leptin, resistin), cytokines (TNF, CRP, IL-6).

The main aim of this study is to determine if persons can increase fruit and vegetable intakes if they make a specific plan to do so.

The conduct of cancer prevention trials through community settings and/or national networks of clinical sites requires the development of appropriate interventions. One promising behavioral strategy that is very simple to administer is that of the formulation of implementation intentions. The premise of this intervention is that once persons formulate a plan of action, automatic behavior follows. Use of this intervention in primary care is an innovative approach to eliciting preventive behaviors with minimal staff time. The goal in this study will be to increase fruit and vegetable consumption by 2 servings/day without a change in overall energy intakes by decreasing consumption of a less nutritious food. Substitution of foods is critical for prevention of weight gain. This feasibility study aims conduct a randomized, controlled clinical trial to test whether formulation of implementation intentions, either alone or with telephone support, can increase fruit and vegetable consumption without an increase in overall energy intakes. Recruitment success in a family medicine clinic in Ypsilanti Michigan will be documented with emphasis on the extent of minority participation. Dietary assessment will be the main outcome variable. Other assessments will include demographics, self-assessment of behaviors targeted by the intervention and levels of carotenoids in plasma. We will enroll 105 subjects to retain 28/arm, and power is good to detect small differences in fruit and vegetable intakes among the three study arms. This will generate useful data for the design of larger dietary intervention trials that are cost-effective and which will utilize multiple clinical sites to optimize enrollment.

This is a Two-part Pilot Study: Part 1 is descriptive and Part 2 is a pilot randomized trial.

Part 1 will be a formative study in which individual interviews are conducted with 20 patients previously diagnosed with colorectal cancer. During open-ended interviews, researchers will collect data on QOL issues colorectal cancer patients face and will elicit feedback regarding development of and participation in a pilot expressive disclosure intervention. Patients also will pilot test an assessment procedures including completing the written questionnaires and wearing the Electronically Activated Recorder (EAR).

Part 2 will include a small randomized pilot test in which 44 patients (two cohorts of 22 patients) will be randomly assigned to the Expressive Disclosure Group Program or a Standard Care Control Group.

Aims of the study include:

* To conduct a descriptive study of colorectal cancer patients, through qualitative interviews and standardized questionnaires, in an effort to assess their QOL, specific health and emotional problems, issues related to social functioning, and preferences regarding intervention format and logistics.
* To use the information from the descriptive study to develop an Expressive Disclosure Group Program for colorectal cancer patients.
* To pilot test a novel technology called the Electronically Activated Recorder (EAR) for assessing cognitive processing and social support in colorectal cancer patients and compare these data to those obtained with traditional self-report measures.
* To pilot test the Expressive Disclosure Group Program and conduct process evaluation including rates of recruitment and retention, attendance, satisfaction, barriers to participation, and feasibility of randomization.
* To explore the effects of the Expressive Disclosure Group Program on outcome variables of QOL and psychological functioning and mediating variables of cognitive processing, coping skills, and social support.

Colon and rectal cancer patients will be asked to participate in this study.

Part 1:

Participation in this study will include three tasks:

First, you will participate in individual interviews during which questions about quality of life will be asked. You also will be provided information about a new expressive disclosure group program for colon and rectal cancer patients, after which you will be asked your opinions about the program. The entire individual interview will last 30 minutes to 1 hour. The interviews will be audiotaped, and then the study staff will make a written copy of the answers given on the tapes to be used in analysis.

Second, you will be asked to complete questionnaires about your medical history, quality of life, mood, thought processes, coping style, and social support. It will take about 1 hour to complete the questionnaires.

Third, you will be asked to wear an Electronically Activated Recorder (EAR) for two days in a row. The EAR is a recording system consisting of a digital tape-recorder and a small external microphone. The EAR is programmed to record for 30 seconds at a time, and then is off for 12 minutes. It repeats this 12.5 minute cycle the entire time while worn for two days. The recordings from the EAR will provide information about your daily activities. The study staff will make a written copy of what is said on the recordings for analysis. Before returning the EAR device, you will be given the opportunity to schedule an appointment where you can listen to the recordings and erase any portion that you think is embarrassing or private. If participants have no concerns about the EAR recordings, the EAR can be returned in a prepaid mailing packet. After wearing the EAR, you will complete an evaluation questionnaire over the phone, which will take about 5 minutes.

This is an investigational study. A total of 25 colon and rectal cancer patients will be asked to participate in this study. Participants will be from M.D. Anderson Cancer Center and Kelsey-Seybold Clinics.

Part 2:

You will first be asked to complete a brief questionnaire about your mood. If the answers to the questionnaire indicate that you are distressed, you will be eligible for the remainder of the study. Only those participants who score as distressed will stay in the study. Regardless of your answers, you will be informed of your test results and provided with a list of community referrals. If your responses to the questionnaire indicate potential harm to yourself, a licensed psychologist will follow-up with you and make recommendations, which may include mental health referrals at M.D. Anderson and/or in the community.

If the first questionnaire indicates that you are distressed, you will be asked to complete additional questionnaires about your medical history, quality of life, mood, thought processes, coping style and social support. It will take about 1 hour to complete the questionnaires.

You also will be asked to wear an Electronically Activated Recorder (EAR) for two days in a row. The EAR is a recording system consisting of a digital tape-recorder and a small external microphone. The EAR is programmed to record for 30 seconds at a time, and then is off for 12 minutes. It repeats this 12.5 minute cycle the entire time while worn for two days. You will complete the questionnaires when you enter the study, 2 months after entering the study, and 4 months after entering the study. You also will be asked to wear the EAR for 2 days when you enter the study, and for 2 days at 4 months after entering the study.

You will be assigned randomly (as in the toss of a coin) to one of two programs: the Expressive Disclosure Group Program or the Standard Care Program.

Patients in the Expressive Disclosure Group Program will attend groups with about 11 members. The program will include 12 group meetings over the course of 4months: 9 weekly meetings, 2 semi-monthly (twice a month) meetings, and a final meeting in month 4. Each meeting will last 1 hour. This program will involve talking and writing about several topics that are important for individuals facing colon and rectal cancer. Participants in this program also will receive written educational material and information about community resources. Group meetings will be audio-taped. Copies of tapes will be provided to participants when they miss a session. Tapes will sometimes be reviewed by project investigators to make sure the groups are covering the program content.

Patients in the Standard Care Program will not attend any formal group meetings. They will receive by mail written educational material and information about community resources.

This is an investigational study. A total of 149 individuals diagnosed with colon or rectal cancer will take part in this study: 131 from M. D. Anderson and 18 from Kelsey-Seybold Clinics.

OUTLINE: This is a randomized, multicenter study. Patients are selected according to age ( 25 to 49 vs 50 to 85), time since initial completion of therapy (3 to 18 months), and are separated in two groups.

Arm I: Quality of life is assessed at baseline and at 3 and 6 months.

Rationale: Determining quality of life in breast cancer patient after completing chemotherapy or radiation therapy or surgical procedures or any combination therapy.

Intervention includes supportive care in terms of psychological support and moral boosting efforts by counseling breast cancer survivors which may enhance the well-being and quality of life of women who are treated with chemotherapy or radiation therapy or surgical procedures or any given combination therapy.

PURPOSE: Randomized clinical trial study to compare the effectiveness of support system and counseling of breast cancer survivors on the well-being of women, who have completed various modes of treatment of breast cancer with the subjects who have no support system.

Study Type: Quality of life and survival

Study Design: Two arm randomized controlled clinical trial to study quality of life and survival of breast cancer patients after completion of chemotherapy or radiation therapy or surgery or any combination therapy. The stages from I to IV (metastases to other sites of the body) are included.

OBJECTIVES:

* Determine the quality of life in breast cancer patient after completing chemotherapy or radiation therapy or surgical procedures or any combination therapy in both Arms.
* Determine the effect of providing standard written materials in Arm I.
* Determine the effect of psychological support and moral boosting efforts by counseling breast cancer survivors which may enhance the well-being and quality of life of women who are treated with chemotherapy or radiation therapy or surgical procedures or any combination therapy in Arm II.
* Determine the effect of socioeconomic status on all available demographic area which is not a criteria for exclusion in both Arms.
* Determine psychosocial and mental state to measure wellness and good quality of life in both Arms.
* Determine the periodic medical care for related or unrelated problems of the disease which have a positive impact on the survival and quality of life in both Arms.
