Chronic Graft Versus Host Disease

Background:

- Stem cell transplants are a complex treatment that can have serious side effects. Having a caregiver to help with the recovery is critical. Serving in this role, however, can cause high stress levels and negative health effects. Researchers want to better understand the health effects of caring for a family member or friend after a stem cell transplant. Understanding these effects can help develop programs that support caregivers coping with the demands of post-transplant care.

Objectives:

- To study the physical and emotional health of caregivers for a family member or friend who has had a stem cell transplant.

Eligibility:

* Individuals at least 18 years of age who are planning to be a caregiver for a person having a stem cell transplant.
* Healthy non-caregiver volunteers for comparison studies.

Design:

* A caregiver will be caring for a transplant recipient just before transplant admission and up through 6 weeks after hospital discharge. The caregiver will usually live with or spend at least 6 hours each day with the recipient.
* All participants will have three study visits. The first visit will happen before the transplant. The second and third visits will happen at the recipient s discharge, and 6 weeks after discharge. The following tests will be done at each visit:
* A health assessment with a physical exam and review of life events and current health problems
* Blood, hair, and saliva samples
* Questionnaires about stress levels, personal health, and the caregiving process.
* Caregiver participants may have an optional fourth study visit within 72 hours after an ICU admission. This visit will include the same tests as the other visits....

There are approximately 65.7 million unpaid caregivers in the United States with an estimated 8 percent providing care to someone with cancer. Although benefit-finding has been reported, providing care to a spouse or loved one with cancer is stressful and can have negative consequences for an individual s psychological and physical health. In cancer caregivers, studies have documented negative outcomes including symptoms of fatigue, impaired sleep quality, poor quality of life, anxiety and depression. These outcomes are of particular concern when cancer patients receive intense treatment such as hematopoietic stem cell transplantation (HSCT) where caregivers are embedded in a treatment trajectory that can extend 4 12 months. In dementia caregivers, additional outcomes have been reported including poor health habits and impaired immunity. In addition, longitudinal studies have also reported caregivers have an increased risk of morbidity and mortality, particularly from cardiovascular disease.

The stress response is initiated in the brain, which determines both the physiological and behavioral responses to an event. The normal physiologic response is complex and dynamic process by which the body responds to daily events in an effort to maintain equilibrium. If the stress becomes overwhelming for the individual, either due to the number or magnitude of stressors, the burden or overload can lead to dysregulation of biologic mediators and behavioral changes (poor sleep, eating or drinking too much, smoking, lack of physical activity) that can exacerbate disease (e.g. cardiovascular disease). A recent study in cancer caregivers demonstrated marked changes in neurohormonal and inflammatory processes in the year following a loved one s cancer diagnosis which may place the caregiver at risk for morbidity and mortality from disease. The relationships among the physiologic and psychological responses as well as behavioral changes to stress have not been explored in HSCT caregivers.

What is clear from the literature is that caregiving is stressful, and it appears to increase one s risk for morbidity and mortality, particularly by increasing cardiovascular risk. What is less clear is what behavioral, psychological, physiological and clinical changes are associated with the process of caregiver for individuals undergoing allogeneic HSCT, a particularly long and stressful experience. The purpose of this exploratory, pilot study is threefold: to longitudinally examine physiological, behavioral and clinical factors in HSCT caregivers during the acute transplant recovery period, to compare those factors in HSCT caregivers to non-caregivers, and to explore the associations among physiological, psychological, behavioral and clinical factors in HSCT caregivers.

Subjects will be accrued to this protocol if they are adult caregivers for a transplant recipient participating in their first allogeneic HSCT protocol at the Clinical Center. An equal sample of healthy volunteers that are non-caregivers will be recruited to serve as control subjects. A sample of 40 subjects (20 caregiver and 20 non-caregiver volunteers) will be recruited to capture the essence of the experience and adequately explore this population.

Each caregiver participant will have data collected prior to the recipient s HSCT (day 0), during the first week of outpatient visits following the recipient s initial discharge from the inpatient setting, and finally 6 weeks post the transplant recipients initial discharge from the hospital. Questionnaires will capture the psychological and behavioral outcomes and include: Caregiver Reaction Assessment (caregivers only), Health-Promoting Lifestyle Profile II, Perceived Stress Scale, The UCLA Loneliness Scale (Version 3), General Self-Efficacy Scale, and PROMIS Short Forms for Anxiety, Depression, Sleep disturbance and Fatigue. Clinical variables (e.g. vital signs) and physiological variables (e.g. cortisol) will be collected along with the questionnaires during the clinic visit and following history and physical exam by a Licensed Independent Practitioner (LIP). A sample of non-caregivers will be minimally matched for age, gender, and race/ethnicity, and complete all study procedures except the questionnaires that are specific to the caregiver s experience (e.g. caregiver burden). There will be no long term follow-up after the study participation period. Exploratory, hypothesis generating analyses will be performed using parametric and non-parametric techniques.

Background:

- People who have an allogeneic hematopoietic stem cell transplant (HSCT) need help as they recover. Being a caregiver can be stressful to the body and mind. Researchers want to find ways to reduce this stress.

Objective:

- To see how a stress reduction intervention affects the stress levels and health of an HSCT caregiver.

Eligibility:

- Adults age 18 and older who plan to be an active caregiver for a person having their first allogeneic HSCT at the NIH Clinical Center. An active caregiver is someone who will be caring for the person from just before admission for the HSCT until at least 6 weeks after.

Design:

* Participants will be put in either the intervention group or the control group.
* All participants will get the usual caregiver education given to all patients and caregivers having a stem cell transplant at the NIH Clinical Center.
* All participants will have 2 study visits. They will give blood samples. They will have health assessments and brief physical exams. They will fill out study questionnaires and have an exit interview.
* The intervention group will get an MP3 player. It will have an audio file with a stress reduction intervention on it. Participants will do this intervention daily. It takes 20 minutes. They will do gentle stretches and breathing exercises. They will get a diary to track their practice.
* The intervention group also will have follow-up phone calls 2 and 4 weeks after their first clinic visit.
* The person getting the HSCT will not be actively involved in the study.
* The study will last about 2 months....

Millions of Americans provide unpaid care for aging or ill family members and friends. Caregiving for an individual with cancer who is undergoing stem cell transplantation is particularly stressful. The stress of caregiving is associated with many disorders, including sleep disturbances, depression, and anxiety. Although there is good evidence that cancer caregivers experience high levels of stress and stress-related symptoms, few interventions studies have been explored to address this concern in this population.

Stress reduction techniques are important skills for individuals to cope with the stress of cancer caregiving. Mindfulness techniques such as yoga and meditation reduce levels of stress and stress-related symptoms in caregivers. However, many caregivers are reluctant to take time away from the patient to attend to their own health and well-being. Few studies have explored yoga-based stress reduction interventions that can be performed at home or at the patient s bedside.

Based upon our past research examining stress and stress-related symptoms in stem cell transplant caregivers, this study is designed to determine preliminary effectiveness of a yoga-based stress reduction intervention in reducing levels of stress in caregivers during allogeneic hematopoietic stem cell transplant (HSCT). In addition, this study will explorefactors associated with change in caregiver stress and symptoms such as demographics, clinical variables, cardiometabolic and inflammatory markers, and health behaviors.

This study will use a prospective randomized control group design to examine the effects of a six-week yoga-based stress reduction intervention on perceived stress in caregivers of allogeneic HSCT patients. Subjects will be accrued to this protocol if they are a caregiver of an individual undergoing allogeneic HSCT at the Clinical Center, NIH, are greater than or equal to 18 years old, able to read English, stand and sit unassisted, raise arms over head without pain, and able to comprehend the investigational nature of the study. A sample of at least 78 caregivers is needed to adequately evaluate the effectiveness of the intervention.

All caregivers will attend the usual care group education provided at the Clinical Center for transplant caregivers. Caregivers randomized to the intervention group will be scheduled for one session with study personnel to receive a 20 minute audio file of gentle chair yoga poses and guided breath awareness, as well as instructions for performing them, and they will be asked to practice daily. Data, in the form of web-based questionnaires, as well as a physical assessment, demographic interview and blood work will be collected from all study participants at baseline (at or near the time of HSCT) and again at the end of the six-week intervention. There will be no long-term follow-up after the intervention period. Questionnaires include: Caregiver Reaction Assessment, Health-Promoting Lifestyle Profile II, Pittsburgh Sleep Quality Index, Freiburg Mindfulness Scale, PROMIS measures of anxiety, applied cognition, depression, positive affect and well-being, Multidimensional Fatigue Symptom Inventory-Short Form and NIH Toolbox measures of loneliness, self-efficacy, and perceived stress. Subjects will participate in an exit interview at the end of the study and the interventionist will complete a log, based on subject diaries, that tracks subject practice, and discrepancy between planned and actual session date and time. Quantitative analysis techniques will be used in this study.

To examine whether HSCT caregivers participating in a stress reduction intervention demonstrate improvement in certain individual factors (self-efficacy, burden, and mindfulness), psychosocial factors (stress, anxiety, depression, loneliness, positive affect and well-being), behaviors (health behaviors) and symptoms (sleep, fatigue, depression, and cognitive impairment) compared with HSCT caregivers in the control group.

Background:

Cancer causes changes in the lives of patients and in the lives of their caregivers. Patients and caregivers feel stress and they affect each other as they deal with these changes. They each cope with cancer diagnosis and treatment on their own. They also cope together. Researchers want to explore this shared change of roles and responsibilities. They will do this by gaining insight into the social experience of cancer caregiving. They also want to better understand the ways caregiver/patient teams manage the cancer diagnosis and experience. This can help find new ways to support caregivers and their patients.

Objective:

To explore the shared change of roles and responsibilities of patients and caregivers by studying the social experience of cancer caregiving.

Eligibility:

English speakers ages 18 and older who are one of the following:

A cancer patients at the NIH Clinical Center (CC)

A caregiver of a CC cancer patient

Design:

Participants will have been screened by the CC.

Patients and caregivers will be interviewed separately. This will last up to an hour and a half.

Participants will complete online surveys.

Participants may have a follow-up phone interview. This will last up to 15 minutes.

Family caregiving is recognized as a significant public health problem, with approximately 66 million adult family caregivers in the United States providing services valued at an estimated $450 billion per year. These individuals are considered informal caregivers, in contrast to formal, paid caregivers such as nurses. Family caregivers suffer role strain and burden of caring that increases their risk for morbidity and mortality, which is the reason why they have been the focus of much caregiver research in recent decades. Improving health outcomes for family caregivers of cancer patients has involved developing approaches to reduce stress by boosting caregiver preparedness, mastery, and self-efficacy; however, in recent years there has been a progressive shift in the way the cancer caregiving experience has been characterized. This characterization has moved away from a focus on the individual to a focus on the caregiver-patient dyad. This shift has occurred, in part, because research has shown that each person in the dyad impacts the way in which the other person adjusts to his/her new role and responsibilities. Thus, the way this occurs is important since these adjustments not only influence the health of the dyadic relationship but also impact the couple s overall well-being.

Current focus on mutual or dyadic coping with cancer diagnosis and treatment stems from the understanding that caregivers and patients work individually, but also together to manage their shared stress and to make meaning out of their mutual experience. Because of this, mutuality and interdependence in dyadic coping extend beyond social support. Dyads collaborate, negotiate, and problem-solve to jointly manage stress and make decisions related to a diagnosis such as cancer. However, there has been little systematic inquiry into the dynamics of shared role adjustment in the cancer family caregiving dyad even though the stress of chronic illness appears to affect both the patient and the caregiver s well-being. The social experience of managing an illness such as cancer and the adjustments made to accommodate the new life situation are the first steps in what is often a prolonged disease trajectory that will require many other changes and transitions along the way.

Therefore, the primary objective for this study conducted in phase one is to explore family caregiver and care recipient mutual negotiation of roles and responsibilities by gaining insight into the social experience of cancer family caregiving that will have application to practice and guide further study. Phase two is contingent upon sample size, with the goal to further explore quantitative measures of individual caregiver and patient as well as dyadic well-being with regard to role adjustment and mutuality. A particular focus will be on dyads who are flourishing and those who are collaborating, to determine what they are doing differently.

A convergent parallel mixed methods study will be conducted using a grounded theory approach to data collection and analysis. A purposive sample of cancer patient participants and their designated family caregiver will be recruited from the National Institutes of Health Clinical Center. Final sample size will depend on conceptual and theoretical saturation, with an anticipated sample to include approximately 20 caregiver-patient dyads. A focused interview guide with open ended questions and probes will be used. During data collection and analysis, data will be compared to previous interviews and to the literature so that gaps in the developing framework can be identified and questions for subsequent interviews adjusted. To capture the experiences and perceptions of the interviewee, interviews with caregivers and care recipient participants will be conducted separately and by the same researcher. During analysis, both interviews will be analyzed on the level of the patient, of the family caregiver, and of the dyadic interaction (e.g. of categories and themes) between the two. Participants will also complete a socio-demographic questionnaire, Neuro QoL Ability to Participate in Social Roles and Activities, and Satisfaction with Social Roles and Activities-Short Forms, the Family Caregiving Inventory for Mutuality Scale, and the Mental Health Continuum-Short Form. This quantitative data will be triangulated and merged with conceptual categories from the caregiver and the care recipient interviews and will be used to compare subgroups to further illuminate qualitative findings.

Sacrifices caregivers make can lead to burden and stress, or to positive adjustment and growth. Through a better understanding of role adjustment and dyadic mutuality, we believe it will be possible to identify novel ways to support caregivers and their patients as they cope day to day and look toward the future.

Background:

- Caring for a loved one is often stressful. Some studies show it may affect health and emotions. Researchers want to know more about this but do not know the best way to gather data. They want to know if an online survey is a good way to gather data from caregivers of people receiving cancer treatment at the NIH Clinical Center. They want to know if there are differences among caregivers depending on the type of cancer treatment the family member or friend is receiving. They want to use this data to improve support programs for caregivers.

Objectives:

- To see if an online study is a good way to get data from caregivers of people with cancer.

Eligibility:

- Adults 18 years old and older who are taking care of a cancer patient at the NIHCC.

Design:

* The study period begins when the family member or friend begins a new cancer treatment and continues for 6 months afterward.
* Participants will complete an online survey 3-4 times. This will include questions about caregiving, stress, emotions, and spirituality. The survey can be taken anywhere on a computer. It will take about 30 45 minutes each time.
* Participants will also answer general questions about themselves and their health. This will be done over the phone with the study team. It will take 5 minutes.

An estimated 65.7 million adults in America provide unpaid care to an individual who is disabled or ill; 7% (an estimated 4.6 million) are caring for a cancer patient. A body of evidence is accumulating that suggests the burden of caring for a family member across a variety of cancer populations can be extremely stressful. The stress of caregiving can have physical and psychological repercussions that can ultimately lead to increased morbidity and mortality. Spousal caregivers of hospitalized individuals are at increased risk of dying within a year of an acute hospitalization, and this risk appears to vary based upon diagnosis. While some evidence suggests caregivers may place the patient s needs above their own and neglect their own health in the process, the evidence regarding whether the stress of caregiving is associated with unhealthy behaviors such as excessive use of alcohol or increased use of tobacco is unclear.

While the evidence clearly shows that caregiving is stressful and may be deleterious to one s health, caregivers are not typically screened for physical and psychological health issues. Most research regarding the stress of caregiving has focused on psychological outcomes including caregiver burden and most research was conducted primarily during the survivorship phase (treatment complete). Because no large studies have examined burden and health outcomes in caregivers over time and across cancer diagnoses during and after the active treatment period, the primary purpose of this study is to assess the feasibility of collecting web-based patient-reported outcomes (PROs) from unpaid informal caregivers of cancer patients that examine the relationships between psychological, health behaviors and health outcomes over time during the active treatment period. A number of secondary objectives will also be addressed including: to explore differences among caregivers across cancer populations and caregiver categories (for example, spousal caregivers versus parent or child caregivers; single caregivers versus multiple caregivers), and to examine whether healthy behaviors ameliorate the relationship between burden and health outcomes. By using a large variety of measures and including a sample of caregivers from multiple cancer patient populations, we hope to identify those populations most at risk as well as those areas (health outcomes and behaviors) where caregivers might be most at risk for negative outcomes.

Subjects will be eligible for this protocol if they are adult informal caregivers to a patient enrolled in a cancer treatment study at the NIH Clinical Center. Once informed consent is obtained, the participant will be provided with information to access the PROMIS assessment center to complete the secure web-based survey. Subjects will complete questionnaires collecting information about caregiver burden and health outcomes at three time points 1) Enrollment, which will coincide with start of treatment for cancer, 2) 3 months after Enrollment, and 3) 6 months after Enrollment. An optionoal "Off-Study" questionnaire completion time point would be offered in the event that the caregiver or the study team requests removal from study participation before the final time point. Questionnaires will capture psychological and behavioral outcomes including: Caregiver Reaction Assessment, NIH Toolbox measures on Loneliness, Self-Efficacy, Meaning \& Purpose, and Perceived Stress, Family Caregiver Inventory Mutuality, Spiritual Perspective Scale, Health-Promoting Lifestyle Profile-II, and PROMIS measures for Global Health, Anxiety, Depression, Sleep Disturbance, Fatigue, and Applied Cognition. A clinician interview will assess basic demographic, health issues, use of alcohol and tobacco and the impact of caregiving at each time point. A brief exit interview will be conducted at the conclusion of participation; there will be no long term follow-up after the study participation period. Exploratory, hypothesis-generating analyses will be performed using parametric and non-parametric statistical techniques.

All participants will be involved in group exercise classes, twice a week for 12 weeks.

The exercise program to be implemented for rural cancer survivors will combine aerobic, resistance, balance, and flexibility exercises delivered in a circuit-type class setting or group personal training format, twice weekly for a 12-week period. If public health restrictions require closure of fitness facilities due to COVID-19, the program will be delivered online. If facilities are open, the sessions will be integrated in the community, delivered in-person. Whether delivered in-class or remotely, the program follows exercise progression principles (ie, frequency, intensity, time, type, overload and progression) over the 12-weeks, with tailoring of any exercise to meet individual participant needs as required, in order to promote fitness and wellness benefits. The exercise intervention is based on the Alberta Cancer Exercise (ACE) program.

Project EXCEL will provide community or online exercise programs to rural and remote and under-served cancer survivors, as well as encourage participants to become life-long exercisers. Exercise is an evidence-based self-management strategy that benefits all cancer survivors. However, most cancer survivors who live in remote or rural places don't have adequate opportunities to be involved in exercise programs that are tailored to their needs.

As cancer survivorship numbers grow, a focus on positive health in survivorship is essential. Exercise is an evidence-based effective self-management strategy that benefits all cancer survivors (CS). However, the majority of research and the limited development of evidence-based clinical or community programs in exercise and cancer has been conducted almost exclusively in urban academic and clinical settings, with very limited implementation outreach to "hard to reach" CS.

In this proposed cancer exercise hybrid implementation effectiveness research, over 5 years we will use an integrated knowledge translation approach to move the current evidence-base that clearly supports the role of exercise in cancer survivorship, into sustainable and effective community-based or online settings that will optimize the delivery of exercise to rural/remote and underserved CS (RCS). Implementing and evaluating the proposed Exercise Oncology Survivorship (EOS) partnership model will result in a sustainable resource to support the implementation and delivery of exercise oncology programs in rural Canadian communities.

Investigators will work to develop lasting referral pathways between cancer care clinics and local fitness professionals. These fitness professionals will have additional cancer specific training to ensure they can effectively and safely deliver the program. Eligible and consenting participants will be screened for exercise safety, and following fitness testing will be triaged to appropriate exercise programming. If, due to COVID-19 public health restrictions, community based programming cannot occur, the exercise sessions will be delivered online by fitness professionals.
