Polycystic ovarian syndrome (PCOS) is the most common cause of anovulatory infertility and causes menstrual disruption in 6.6-6.8% of women in reproductive age and is characterized by insulin resistance, hyperinsulinemia, hyperandrogenism and anovulation. The gaol of this study was to assess the effects of metformin on menstrual disorders and lipid profile in women with PCOS in bandarabbas.

Poly cystic ovarian syndrome (PCOS) is a common hormonal disorder in women that affecting of 6.6-6.8% of women o f reproductive age. PCOS associated with a broad range of clinical, hormonal and metabolic disorders consist of hirsutism, Obesity, Acne and elevated male hormones, anovulatory cycles, dyslipidemia and infertility.

Metformin is an oral hyperglycemic agent witch by decrease insulin resistance and improving serum glucose level in diabetic patients and anovulatory cyle in women with PCOS. The goal of this study is to evaluate the efficacy of different dosage of metformin on menstruation and lipid profile in women with PCOS in banda-abbas.

Haematological malignancies constitute the most common neoplastic disease in child population, with acute leukemia occupying the number one spot with a percentage of 32.8%. In children, leukaemia is primarily encountered in its acute form (97%) and in the majority of the cases it is presented as Acute Lymphoblastic Leukaemia - ALL (80%). Acute Non-Lymphoblastic Leukemia - ANLL is encountered less frequently (17%) and it includes Acute Myelogenous Leukaemia - AML (15%) and some other rare forms (2%), while the remainder 3% corresponds to chronic leukaemia.

L-Asparaginase (L-ASP) is a fundamental component during the loading phase with regards to achieving remission of the disease and, likewise, during the maintenance phase with the intention of establishing that remission in both children and adults suffering from ALL. The cytotoxic effect of the exogenous administration of Asparaginase is caused by the depletion of the reserve of asparagine in the blood. Asparaginase (ASP) acts as a catalyst for the hydrolysis of asparagine to aspartic acid and ammonia. Asparagine is vital for protein and cell synthesis and, therefore, for their survival. The normal cells of the human body have the ability to produce asparagine from aspartic acid, with the assistance of the enzyme asparagine synthetase. However, the neoplastic cells either lack the enzyme completely or contain minute amounts of it resulting in their inability to synthesize asparagine de novo. The survival of these cells and their ability to synthesize proteins depends entirely on receiving asparagine from the blood. Thus, the administration of ASP leads to the inhibition of DNA, RNA and protein synthesis which, in turn, results in the apoptosis of these cells.

Despite L-ASP's paramount importance in the chemotherapy treatment of leukaemia, it is responsible for a plethora of toxic adverse effects that sometimes even require the termination of its administration. A critical adverse event of ASP is a disorder in the metabolism of lipids. Specifically, it appears that the activation of the endogenous pathway that produces triglycerides through hepatic synthesis leads to hypertriglyceridaemia. The liver is capable of synthesizing VLDL (Very Low Density Lipoproteins) that are rich in triglycerides. Utilising the effect of the enzyme Lipoprotein Lipase (LpL), located on the vascular endothelium, the triglycerides detach from the VLDL causing the latter to transform into IDL (Intermediate Density Lipoproteins) and afterwards into LDL (Low Density Lipoproteins). The triglycerides are later extracted from the blood circulatory system and stored in the adipose tissue, while the LDL particles connect with tissue receptors or macrophage receptors. The final products of the breakdown (coming from the peripheral hydrolysis of triglycerides with the help of LpL) of chylomicrons, VLDL, the remnants of lipoproteins, will eventually be removed by hepatic receptors. Apolipoprotein E (Apo-E) plays an important role in this procedure, it binds these remnants in the presence of LpL and hepatic lipase. Along the duration of the treatment with ASP, reduced LpL functionality is recorded, resulting in impaired plasma clearance of triglycerides and an increase in their levels, while L-ASP appears to cause disorders in other lipid factors, such as cholesterol, HDL and apolipoprotein A. Disorders of lipid metabolism have been found to be associated with polymorphisms of the LpL and Apo-E genes, sometimes with positive and sometimes with negative effects on the lipid profile and more likely participation in cardiovascular complications. The current study will evaluate, the lipid profile of children with ALL, the effect of L-ASP on the lipid profile of the aforementioned patients, as well as the correlation between the polymorphisms of Lipoprotein Lipase (LpL) and Apolipoprotein E (ApoE) with the values of the lipids during chemotherapy. Both the universal and national bibliography that pertain to the effect of ASP on the potency of LpL and App E and to the values of the lipids in children that suffer from ALL during chemotherapy with L-ASP is limited, while there exists no bibliographic reference correlating the genetic background to LpL and Apo E and the relation of the lipid profile. The current study will examine for the first time gene polymorphisms of LpL and Apo E in children with ALL during treatment with ASP.

Blood will be collected and placed in a 4ml tube without anticoagulants. The tube will be centrifuged and the blood's serum will be isolated in order to examine the biochemical parameters of the study with the cooperation of the Biochemistry Lab of University Hospital AHEPA. Furthermore, 2ml of whole blood will be collected in a tube containing Ethylenediaminetetraacetic acid (EDTA) as the anticoagulant for use in molecular techniques. The whole blood and the serum will be stored in a temperature of -80 degree Celsius.

Correlation of Polymorphisms of Lipoprotein Lipase (LpL) and Apolipoprotein E (Apo E) With Lipid Profile of Children With Acute Lymphoblastic Eukaemia During Therapy With L - Asparaginase.

The genotypes of children with ALL will be recorded and it might constitute an early indicative factor concerning the treatment's outcome. Thusly, essential information will be extracted about the possible contribution of genotype of children under treatment with L-ASP to the lipid disorder as shown in the lab results, to better monitoring of each unique phase of the therapy for clinical occurrences and complication and to faster therapeutic intervention.

During the disease's diagnosis, the lipid profile of the patients' will be determined by measuring the changes of the following parameters compared to the baseline measures: cholesterol (mg/dl), triglycerides(mg/dl), HDL-cholesterol(mg/dl), LDL-cholesterol(mg/dl), apolipoprotein A1(mg/dl), apolipoprotein B100(g/L), lipoprotein α \[Lp(α)\](nmol/l), glucose (mg/dl), SGOT (U/I), SGPT (U/I), TSH (mU/l) FT4 (pmol/l) amylase (U/I) and lipase (U/I).

The study was conducted to determine the therapeutic role of flax seed for Poly cystic ovary syndrome. A sample of 100 subjects were collected. The subjects were from various age group ranging from 18 to 45 years. Subjects showing the signs, symptoms, biochemical, and clinical parameters of the diseases were included in the study. Three different quantities of flax seed were introduced to the subjects for 90 days.

Objectives:

1. To assess the nutritional status of Poly cystic Ovary Syndrome patients
2. To determine the therapeutic role of flax seed for PCOS patients

Material and method: The level of follicle stimulating hormone, leutinizing hormone, sex hormone binding globulin, serum prolactin, ultrasound were analyzed. To determine the side effects of the flax seed, the liver-enzymatic, lipid profile, and renal tests were conducted.

Material and methods

Study Population:

The data were collected from the Allied Hospital. All Females of ages 18 to 45 years, having PCOS attending the OPD of Allied Hospital was taken as the study population. Efficacy trials were conducted on the patients of PCOS, including females of age 18-45 years. They were divided into different groups (as per no. of selected treatments along with the control). Ten subjects left the study and were replaced. The project was approved by the ethical review committee.The feeding study was conducted with the cooperation of Allied Hospital. The study protocol was discussed with medical professionals and finalized after through technical suggestions.

Selection of subjects and Study Design

The patients were assorted randomly into four groups, 25 in each, namely control (T0),

Inclusion Criteria:

All females of age 18 to 45 having PCOS

Women showing the presence of two out of three features (Oligo- or anovulation, hyper-androgenism, or poly cystic ovaries (Ovarian volume \>10cc, 12 or more peripherally located follicles 2-9 mm in diameter or hypo-echoic enlarged ovaries with hyper-echoic central stroma in USG). Patients on Metformin (used for insulin resistance)

Exclusion Criteria:

* Females under 18 and above 45 years of age
* Those on any other medical therapy
* Pregnant and lactating women
* Those with a history of allergy to flax seed
* Women on anti-platelet and fibrinolytic drugs
* Women showing signs of PCOS but showing normal ovaries on ultrasound

Study Design

It was literature review based intervention study. The selected population for intervention purposes was studied twice, i.e. before and after the intervention of the selected nutraceutical. The same subjects were studied once again after one month of stopping the intake of selected nutraceutical. The change in results has confirmed the effects of the intervention.

This has been divided into four phases.

1. Phase-1:Assessment of the nutritional Status in PCOS
2. Phase -2:Development of nutraceutical (flaxseed sachets)
3. Phase -3:Intervention to study the effect of selected nutraceutical
4. Phase-4:Blood Analysis

   The procedure adopted in each of the above phases is explained as follows:

   Phase-1: Assessment of the nutritional status of selected patients

   This was done in Allied Hospital, patients attending OPD of gynecology ward was assessed for nutritional status. Following parameters were studied for this purpose
   * Family History
   * Medical history
   * Physical signs (thyroid gland, height, weight, skin, hair, breasts, belly, and BMI)
   * Menstrual cycle detail (menarche, frequency, duration, and severity)
   * History of dysmenorrhea
   * Ferriman-Gall way scoring for hirsutism
   * Marital status, parity, and difficulty in conception

   Phase -2: Development of Nutraceutical (flaxseed sachets)

   Phase-3: Intervention

   An acceptable number of human volunteers were selected keeping in view all ethical considerations. (25 in each group)

   3. Flaxseed (6.6 g) x TTS = 20 g/day (T2)
   4. Flaxseed (8.3 g) xTTS = 25 g/day (T3)

   The flaxseed powder from the selected treatments was given daily for a period of 12 weeks. The dietary history of the subjects was monitored with the help of a Food Diary on a weekly basis. Education/counselling were provided to the patients for necessary compliance with the restricted diet for PCOS.

   Phase-4: Blood Analysis and Ultrasound

   After three months of intervention blood tests: follicle stimulating hormone (FSH), Luteinizing hormone (LH) thyroid stimulating hormone (TSH), serum prolactin, and ultrasound will be repeated.

   Biochemical assays

   Follicles stimulating hormone

   The blood sample was taken to determine the quantitative level of serum follicles stimulating hormone (FSH) in patients fed with flaxseed powder by following the Immuno-enzymatic calorimetric method (ELISA).

   Luteinizing hormone

   The blood sample was taken to determine the quantitative level of serum luteinizing hormone (LH) in patients fed with flaxseed powder by following the Immuno-enzymatic calorimetric method.

   Sex hormone binding globulin

   The serum sex hormone binding globulin hormone (SHBG) in patients fed with flaxseed powder was determined by following the Rosner's method.

   Prolactin

   The serum sex prolactin level in patients fed with flaxseed powder was determined by following the radio-immunological method.

   Thyroid stimulating hormone

   The thyroid stimulating hormone (TSH) level in patients fed with flaxseed powder was performed to determine the thyroid gland functioning by following the third-generation non-isotopic method.

   Liver Function Test

   Alkaline Phosphatase (ALP)

   The activity of alanine phosphatase in the plasma was determined by using enzyme-linked immune-sorbent assay kit method.

   Aspartate Aminotransferase (AST)

   The activity alanine transferase in the plasma was determined by using di-nitrophenyl hydrazine (DNPH) method using Sigma kits 59-50 and 58-50 under the conditions.

   Alanine Aminotransferase (ALT)

   The activity alanine aminotransferase in the plasma was determined by using enzyme-linked immune-sorbent assay kit method.

   Lipid Profile

   Low-density lipoprotein (LDL)

   Serum low-density lipoprotein (LDL) was estimated by following the protocol of Vinson.

   High-density lipoprotein (HDL)

   The high-density lipoprotein (HDL) was assessed by HDL-Cholesterol precipitation.

   Triglyceride

   The triglyceride in the collected serum was determined by liquid triglyceride (GPO-PAP) method.

   Renal Function Tests

   Urea

   The flax seed fed patients were diagnosed with urea content by GLDH method with the help of commercial kit.

   Creatinine

   The blood creatinine of the patients fed on flax seed powder was determined by the Jaffe method using creatinine commercial kit.

   Follicles present in ovaries

   Ultrasound of the ovaries was carried out to check the number of follicles present in the ovaries.

   Size of the ovaries

   The size of the ovaries of patients fed with flax seed powder was also checked in ultrasound.

   Fasting blood glucose level

   The blood sample was taken to check the fasting blood glucose level. The sample was taken eight hours after eating. Fasting blood glucose level as determined by glucose dye oxido-reductase mediator reaction which is specific for glucose by glucometer Accuchek active.

   Research instruments
   1. Questionnaire: For screening/nutritional assessment of PCOS patients
   2. Hormonal analysis: Follicle stimulating hormone (FSH), luteinizing hormone (LH) thyroid stimulating hormone (TSH), Sex Hormone binding globulin (SHBG), and serum prolactin.
   3. Biochemical assay: ALP, ALT, AST, urea, Creatinine, Fasting blood glucose level, LDL, HDL, and triglyceride.

   Ethical Considerations:Written ethical approval was taken from the management of the concerned hospital and HOD of Gynecology department of the selected hospital. The consent was taken from the subjects after explaining the purpose of the study and after getting their consent for their menstrual cycle history, blood tests and ultrasound.

   Statistical analysis

   The data were subjected to statistical analysis by using the SPSS statistical package, analysis of variance (ANOVA) and least significant difference (LSD) was used to determine any significant difference among the treatments at p\<0.05.

Late subclinical cardiovascular disease in testicular cancer survivors exposed to cisplatin-based chemotherapy and bone marrow transplant

Testicular cancer (TC) is diagnosed in young adult males between 18-39 years old. There are late (≥10 years after treatment) atherosclerotic cardiovascular disease (ASCVD) events after cisplatin-based chemotherapy (CBCT) treatment in testicular cancer survivors (TCS), along with heightened rates of hypertension, dyslipidemia, diabetes mellitus, and metabolic syndrome. Early detection of ASCVD to direct preventive measures in young TCS is an unmet need as these patients typically fall short of traditional 40-45-year age-cut offs for ASCVD screening. ASCVD risk will be evaluated in TCS ≥ 10 years after treatment in three groups: 1)TCS exposed to CBCT, 2)TCS exposed to CBCT and bone marrow transplant (BMT), and 3)TCS cured with surgical resection/surveillance. The focus will be on detecting subclinical atherosclerosis in TCS using blood lipid biomarkers and advanced cardiac CT imaging.

Nutritional status including changes of body composition is one of the most important clinical determinants of outcome after gastrectomy for gastric cancer. Various type of gastric operations are widely used with favorable outcome in South Korea. It was reported that several advantages of laparoscopic gastrectomy are the prevention of overt weight loss and enhanced recovery of muscle mass at 6 months after surgery. But there have been no longitudinal studies evaluating changes in the body composition according to the different type of anastomosis of laparoscopic gastrectomy.

The purpose of this prospective study was to investigate changes in lipid indices associated with whole body composition during 1 year of follow-up after laparoscopic gastrectomy. Gastrectomy resulted in improved lipid indices and a reduction in body weight, fat and LBM. The HDL-Csignificantly increased in the non-obese group for 1 year after gastrectomy and the reduction of TG level was positively correlated with fat, especially with trunk fat.

* All patients will undergo a laparoscopic gastrectomy for their gastric cancer (proved to be stage I or II) or benign gastric neoplasm by preoperative screening diagnostic procedure. They will receive laparoscopic gastrectomy as follows: (1) wedge resection, subtotal or total gastrectomy with or without lymph node dissection; (2) R0 resection; (3) laparoscopic gastrectomy; (4 gastroduodenal anastomosis or duodenal bypass, including Roux-en-Y or gastrojejunostomy.
* Pre-operative, 6 and 12 months post-operative body composition measurement will be performed on each patient. The whole body composition measurement will be assessed by Hologic 4500/A dual energy X-ray absorptiometry system. The software performs calculations of the differential attenuations of the two photon energies and present data of percentage of fat, fat mass (g), lean mass (g), bone mineral content (g), bone mineral density (g/cm2), and total mass (g).
* Laboratory analysis - There is a battery of labs that are ordered in both the pre-operative, 6 and 12 months post-operative time frame. These labs are ordered independent of the patient's participation in the study. These labs are ordered to evaluate and to assist management of the patient's medical comorbidities as well as to assess for adequate nutritional intake and balance.
* Queries on nutritional intake and exercise will be made at each post-operative appointment.
