The main objective of this pilot study is to evaluate the acceptability of clinical data collection using a semi-automated e-solution based on the use of text messaging among older adults undergoing chemotherapy treatment at home.

Most cancer diagnosis are made among the 70 years and over population. In this population, negative consequences of cancer treatments are of great concern, especially for those who are socially isolated or live in rural areas. International oncology societies recommend personalized treatments to reduce adverse events. However, few tools have been validated in this population to monitor compliance and risks of outpatient treatments. In addition, health professionals continue to manage unplanned patient calls which could disrupt effectiveness of their work. Our hypothesis is that the semi-automated collection of health data (text message questionnaires) during outpatient chemotherapy secures the patient's care pathway and optimizes care time, in particular for patients' education and support. Therefore, patients (and/or their caregivers) who benefit from calls follow-up (usual care), would have the possibility of receiving an additional weekly remote questionnaire (by email or SMS) via a CHATBOT on any terminal. A minimum score of 0 and a maximum total score of 27 can be obtained from the scale. The scale does not have any cut-off point. The higher the score obtained from the scale, the higher the level of knowledge about suicide.

This scale consists of 26 items. The lowest score that can be obtained from this scale is 26 and the highest score is 130, and the scale has no cut-off point. An increase in the score obtained from the scale indicates that nurses' self-perception is becoming more positive in relation to their knowledge and skills in the recognition and management of suicide risk.

There are 55 items in total in the scale. The scale has three subscales: "stigma", "isolation/depression" and "glorification/normalization". Total score is not calculated in the scale. High scores from the "stigma" sub-dimension of the scale indicate high stigma towards suicide. High scores from the "isolation/depression" sub-dimension of the scale indicate that suicide is more associated with depression and isolation, while high scores from the "normalization" sub-dimension indicate that individuals normalize suicide rather than stigmatizing people who commit suicide.

Chemotherapy, the main treatment for childhood leukemia, has side effects on healthy cells. One of the most important of these side effects is the risk of infection due to neutropenia. In clinics, blood culture is the gold standard for the detection of possible infection risk (bacteremia, fungemia, etc.). During the diagnosis and treatment process in children with leukemia, a large number of peripheral or catheter blood cultures are performed. When the culture results are positive, it is accepted that the infection has grown. In some cases, a positive peripheral blood culture result may not be clinically significant. An agent belonging to the skin flora of the patient or the healthcare professional taking the field culture may also cause the blood culture result to be positive. This is called contamination or false positive blood culture. Contaminated cultures may cause prolonged hospitalization, additional medical interventions, unnecessary initiation of antibiotic treatment and related antibiotic resistance, toxicity due to additional drugs and an increase in hospital costs. The peripheral blood culture contamination rate, which is accepted as a quality indicator in some countries, should be below 3%.

In this context, this study aimed to determine the prevalence of false positive peripheral blood cultures by examining the peripheral blood culture results obtained during routine follow-ups from children hospitalized with leukemia in the pediatric hematology clinic. The data will be collected retrospectively covering three years before March 2024 when the ethics committee approval was obtained. Since the relevant clinic cares for approximately 25 new children diagnosed with leukemia annually, the study sample is planned as 75 cases.

A positive peripheral blood culture does not always mean that the individual has a systemic infection. In some cases, a positive blood culture result may also be seen in the event of contamination with the skin flora of the person receiving/receiving the culture. This is called a false positive blood culture. This situation is characterised as a situation that reduces the reliability of the test. Contaminated false positive blood culture is a single culture bottle containing microorganisms belonging to the skin flora (Coagulase Negative Staphylococcus \[Staphylococcus epidermidis, Staphylococcus hominis, Staphylococcus capitis, Staphylococcus haemolyticus\], Viridans streptococci, Aerococcus, Micrococcus, Propionibacterium spp, Bacillus spp., \[except B. Anthracis\], Corynebacterium spp. and alpha-gamma haemolytic streptococci, Dermabacter spp., Rothia spp. and Kocuria spp.)

In cases where bacteria grow in blood culture media, it is necessary to distinguish between local skin flora and true pathogenic bacteria. Contaminated culture results in prolonged hospital stay, additional tests, unnecessary exposure of the patient to antimicrobial agents and increased risk of other complications (qualitative change in microorganisms, increased colonisation rates with drug-resistant organisms such as VRE and MRSA, increased Clostridium difficile infection rates). Inappropriate antibiotherapies initiated due to contaminated blood culture results may cause toxic side effects in the patient. One of the most serious negative effects of inappropriately used antibiotics is the development of antibiotic resistance. Due to the large-scale use of inappropriate antibiotics in hospitals, nosocomial flora undergoes a change. This change leads to the disappearance of susceptible microorganisms in the flora and the increase in resistant strains.

In this context, the aim of this study was to determine the prevalence of false positive peripheral blood cultures by retrospectively examining the peripheral blood culture results obtained during routine follow-up from children hospitalised with leukaemia in the paediatric haematology clinic.

Population and Sample The population of the research will consist of children who received chemotherapy due to leukemia treatment and whose peripheral blood culture was taken during routine treatment. The sample will consist of pediatric patients who were hospitalized in the Hematology Department Clinic of Ege University Faculty of Medicine Children's Hospital between March 2021 and March 2024 (after the approval of the Ethics Committee, retrospectively from the month of approval (3 years)). Since the number of inpatient cases diagnosed with leukemia in the Pediatric Hematology Clinic is approximately 25 cases per year, it is planned to examine the peripheral blood culture results of 75 different cases in three years.

Following and implementing innovations and current developments in care practices for children diagnosed with leukemia and their parents will allow the quality of care to increase. The purpose of this research is to evaluate the effect of web-based education given to parents of children diagnosed with leukemia on their knowledge level, satisfaction and self-efficacy.

The research is a single-blind randomized controlled experimental research type. The research will be conducted in a single center, Ankara Bilkent City Hospital MH4 Children's Hospital Hematology-Oncology Clinics and Leukemia Polyclinic. The population of the research consists of parents of children diagnosed with leukemia between the ages of 2-18 who are followed and treated in the specified places. The sample of the research consists of 72 participants, 36 in the experimental group (the group that received web-supported training) and 36 in the control group (the group that continued routine treatment).

The consent of the parents who meet the research criteria and voluntarily agree to participate in the study will be obtained with the "Informed Voluntary Consent Form" and the parents in both the experimental group and the control group will be given the "Child and Parent Introductory Information Form", "Child with Leukemia Parent Information Level Pre-Test Evaluation Form". ", "Generalized Perceived Self-Efficacy Scale" will be applied. Once the sufficient number is reached, web-based training will be provided to the parents in the experimental group. "Parents of Children with Leukemia Knowledge Level Post-Test Evaluation Form", "Generalized Perceived Self-Efficacy Scale" and "Web-Based Parents of Children with Leukemia Training Satisfaction Evaluation Form" will be applied to the parents who complete the training. No intervention will be made to the parents in the control group, and the children will continue their routine care and treatment process. The "Child with Leukemia Parent Knowledge Level Posttest Evaluation Form" and the "Generalized Perceived Self-Efficacy Scale" will also be applied to the parents in the control group.

In childhood cancers, parent education is one of the important nursing practices that must be carried out so that parents can feel competent in care practices and the child can be given the most appropriate care in line with their needs. The content of these trainings should focus on parents gaining skills by participating in care. It is important to provide quality education to parents with methods that suit their learning preferences and styles. As the use of technology has gained importance around the world, technology-based interventions for children and parents have also become important. Developments in the field of digital health can make it easier to transfer many aspects of cancer treatment into families' lifestyles with new options. Technology-based interventions contribute to children and parents as they provide opportunities such as appealing to different senses and providing continuous access to information. One of these interventions is the creation of web-based programs. Carrying out training via web bases provides the opportunity to easily access training content without any time or space limitation. It also allows the individual to determine his own learning pace and examine the materials easily.

Patients and their relatives often use the internet to access cancer-related information. However, some of the information on the internet, which is an easy and fast way to access information, may be outdated or inaccurate. Although the use of the internet as an alternative method of accessing information and the fact that it is a fast and easy method of accessing information is seen as a great advantage in terms of health services, inaccurate and outdated information on the website poses a major problem. Therefore, it is important to create websites where parents can access accurate and up-to-date information.Following and implementing innovations and current developments in care practices for children diagnosed with leukemia and their parents will allow to increase the quality of care. The purpose of this research is to evaluate the effect of web-based education given to parents of children diagnosed with leukemia on their knowledge level, satisfaction and self-efficacy.

The hypotheses of the research are presented below; HOa: There is no difference between the knowledge levels of parents who received web-based child care training with a diagnosis of leukemia and the knowledge levels of parents who did not receive training.

HOb: There is no difference between the self-efficacy scale scores of parents who received web-based leukemia child care training and the self-efficacy scale scores of parents who did not receive training.

H1a: There is a difference between the knowledge levels of parents who received web-based child care training with a diagnosis of leukemia and the knowledge levels of parents who did not receive training.

H1b: There is a difference between the self-efficacy scale scores of parents who received web-based leukemia child care training and the self-efficacy scale scores of parents who did not receive training.

The research is a single-blind randomized controlled experimental research type, and "pretest-posttest randomized control group design", one of the experimental research types, will be used in the research. The research will be conducted in a single center, Ankara Bilkent City Hospital MH4 Children's Hospital Hematology-Oncology Clinics and Leukemia Polyclinic. The population of the research consists of parents of children diagnosed with leukemia between the ages of 2-18 who are followed and treated in the specified places. "Simple random sampling method" will be used in sample selection. Before starting the research, the sample size was determined with the Power and Sample size analysis G Power Program. Medium effect size f = 0.25, Type I error: 0.05, Type II error: 80% (power), and the number of samples calculated was 17 parents for the group receiving web-supported education (experimental group), 17 parents for the group receiving routine care. For the group (control group), 17 parents were determined as a total of 34 parents. However, it was planned to conduct the research with a total of 60 parents, 30 parents for both groups, in order to avoid problems in population generalization, to fulfill the parametric test assumptions of the research, and to reveal the relationships between variables more clearly. Considering that there may be losses due to the research requiring long-term follow-up, 72 parents will be included in the research by taking 20% more than the determined sample number. In the randomization to be carried out, the "simple randomization method" will be used in order to provide equal samples for both groups (experiment-control).

In order to collect data in the research, "Child and Parent Introductory Information Form", "Child with Leukemia Parent Information Level Pre-Test-Post-Test Evaluation Form", "Web-Based Child with Leukemia Parent Education Satisfaction Evaluation Form", "Generalized Perceived Self-Efficacy Scale" will be used. The consent of the parents who meet the research criteria and voluntarily agree to participate in the study will be obtained with the "Informed Voluntary Consent Form" and the parents in both the experimental group and the control group will be given the "Child and Parent Introductory Information Form", "Child with Leukemia Parent Information Level Pre-Test Evaluation Form". ", "Generalized Perceived Self-Efficacy Scale" will be applied. Parents who meet the research criteria and voluntarily agree to participate in the study will be included in the experimental and/or control group according to the randomization list. Once the sufficient number is reached, the parents in the experimental group will be contacted via mobile phone and necessary explanations (name of the site, membership to the site, how to log in to the site, menus on the site and how to use these menus, link of the site, training duration) will be made. In order for users to learn according to their own learning levels, without space and time constraints, 5 modules prepared in line with the objectives and achievements, 5 video lectures related to the modules, and videos showing the applications in the video lectures will be uploaded to the site. Before the modules are uploaded to the website, expert opinion will be obtained and necessary arrangements will be made. Training achievements and objectives will be given in the introduction part of the modules, and brief information on the subject will be given at the beginning of the video.

Users will be asked to review the relevant modules and videos, and after reviewing the modules, they will be asked to tick the "I have read and understood" box. Parents in the experimental group will be given web-based training for 5 weeks. "Parents of Children with Leukemia Knowledge Level Post-Test Evaluation Form", "Generalized Perceived Self-Efficacy Scale" and "Web-Based Parents of Children with Leukemia Training Satisfaction Evaluation Form" will be applied to the parents who complete the training. No intervention will be made to the parents in the control group, and the children will continue their routine care and treatment process. At the end of 5 weeks, "Child with Leukemia Parent Knowledge Level Posttest Evaluation Form" and "Generalized Perceived Self-Efficacy Scale" will also be applied to the parents in the control group. At the end of the study, the modules will be shared with the control group.

To conduct the research, the necessary written permissions were obtained from the Ankara Provincial Health Directorate with the permission of the ethics committee.

Web-based training will be provided to parents in the experimental group, and parents in the control group will continue their routine treatment and care process.

Participant blinding was done because the parents who participated in the study voluntarily did not know which group they were in until the end of the study.

Providing Web-based Training to Parents Of Children With Leukemia

Evaluation of the Effect of Web-Based Education Provided to Parents of Children With Leukemia on Parents' Knowledge Level, Satisfaction and Self-Efficacy

This form aims to evaluate the parents' knowledge levels before the training and the effectiveness of the training given in the post-training period. The form consists of "19" closed-ended questions about common symptoms in children with leukemia and the management of these symptoms. The evaluation form before the training (pre-test) and after the training (post-test) consists of the same questions. All of the items in questions 1, 2, 3, 6, 7, 8, 9, 11, 13, 14, 15, 16, 17, 18, 19 are correct, and questions 4, 5, 10 and 10 are correct. There is only one correct answer to question 12. Each of the items in questions 1, 2, 3, 6, 7, 8, 9, 11, 13, 14, 15, 16, 17, 18, 19 is worth 1 point, and "water and soap" in question 4 is worth 1 point. 5 points. The total knowledge score for the correct answers to the questions "once a week", 25,000 mm3 in the 10th question and "4-6 times" in the 12th question will be calculated as 1 point each.

The study, investigates the current physical activity promotion practices among physicians and nurses, as well as social norms toward physical activity during treatment among cancer patients and among physicians and nurses who treat or counsel cancer patients.

For this purpose, four large surveys with health care professionals (specialized practitioners, N=300; general practitioners, N=300; oncology nurses, N=300) and cancer patients (N=1200, 400 patients each with breast, prostate or colorectal cancer) with a cross-sectional research design are conducted.

A research recruitment tool will be available in paper and online formats and will include information on finding out about LC clinical trials, the role of research teams, embedding research into multidisciplinary team meetings and health needs assessments, communication pathways, signposting LCPs, practical considerations and reaching underrepresented groups. The tool will be used by lung cancer nurses in their daily practice as an aid to support their discussions with lung cancer patients about clinical trial opportunities. There are \~85,000 lung cancer patients (LCPs) in the UK; yet only around 8% were recruited into clinical trials in 2021/22. LCPs need opportunities to take part in clinical trials to access new treatments, increasing their quality of life, treatment choices and life expectancy.

Discussions with nurses can help patients make better treatment decisions, improving experiences of care. However, research has shown that lung cancer nurses (LCNs) often feel unable to discuss participation in trials with patients due to lack of knowledge, confidence, time and training.

This study aims to develop and test a research recruitment tool for LCNs, to support LCPs to enter clinical trials.

Objectives include to:

* Explore reasons for low uptake of LCPs into clinical trials
* Develop a tool for LCNs to talk to patients about clinical trials
* Test whether the tool improves the number and quality of discussions nurses have with LCPs about clinical trials

The study has four phases:

Phase 1: A literature review will identify problems that make clinical trial uptake difficult for LCPs, carers and clinicians

Phase 2: Six group discussions with LCNs, patients and carers will explore issues that create potential barriers for patients taking part in clinical trials. The groups will take place online, last approximately one hour and be recorded.

Phase 3: Part 1\&2 findings will help us develop a LCN research recruitment tool. The tool will contain information on how nurses obtain information about LC clinical trials, research teams, communication issues, practical issues and how to reach LCPs.

Phase 4: The research recruitment tool will be tested in four UK NHS hospitals. A survey will identify any changes in nurses' clinical trials awareness and confidence before and after using the tool. Interviews with LCNs, patients and carers will explore their views on the tool, clinical trials participation and experiences of care.

Background: Patients who discuss taking part in research and those that go on to participate have better outcomes and experiences. Reported trial benefits for patients include greater access to new drugs; disease control; improved survival; enhanced quality of life; more intensive follow up and monitoring; better symptom management; enhanced care and support; and patient empowerment. In cancer care, the rate at which targeted treatments are translated into clinical practice for use by LCPs is reliant on accessible clinical trial opportunities; however, trial uptake remains low. Recruitment to research within the UK National Health Service (NHS) is challenging and COVID-19 saw a 59% reduction in cancer patients entering clinical trials in England, from 67,057 in 2017/18 to 27,734 in 2020/2021 \[7,8\]. Resultingly, the Institute of Cancer have recommended that trial information is made accessible and understandable to patients and that clinical staff have conversations about trials early in the treatment pathway.

Being assigned a LC clinical nurse specialist is associated with better experiences of care and studies have found that discussions nurses have with patients about clinical trial enrolment play a key role in decision-making and recruitment. However, LCNs often feel unable to discuss clinical trials with patients due to a lack of knowledge and confidence, time, expertise and training. Key barriers to trial recruitment include screening complexities; lack of resources, staff, skills and equipment; time; limited clinician awareness; perceived administrative burdens, concerns over trial suitability; inadequate information communicated to patients and clinicians and complex trial terminology. It is recommended that clinical trials are more closely integrated into clinical practice and that staff are adequately trained to support clinical research.

Study design: This mixed methods study consists of the following phases:

Phase 1: A systematic review of the best scientific literature will identify challenges and barriers to clinical trial recruitment amongst LCPs, carers and clinicians. PRISMA reporting guidelines will be followed.

Phase 2: Six qualitative focus groups with lung cancer patients, carers, lung cancer nurses and other members of the multidisciplinary team (MDT) will explore challenges and facilitators to LCPs' clinical trial entry. Focus group participants will be recruited through the participating sites lung cancer nursing teams.

Eligible clinician participants (nurses and other MDT staff) will be contacted through lead LCNs at participating sites (see phase 4), who will share invitation letters and participant information leaflets with their teams. If they are happy to proceed a researcher will organise a time, date and place with participants to proceed with the focus group. Participants will also be recruited through LCNUK and the National Institute for Health Research (NIHR) Local Clinical Research Networks.

Patients and carers will be recruited through the NCRI consumer forum and other cancer patient/carer networks, including RCLCF and Mesothelioma UK. LCNs at participating NHS sites will also share invitation letters and information leaflets with LCPs, inviting them to take part. If they are happy to proceed the researcher will organise a time, date and place with participants to proceed with the focus group. The study will also be promoted via social media channels such as Twitter, Healthcare Unlocked and closed Facebook groups. We intend to recruit \~48 participants to focus groups (eight per group); three focus groups will be with LCNs, research nurses and multidisciplinary team members from district general hospitals (DGHs) and tertiary centres, and two with LCPs and carers. A final group will include clinicians from the Christie and Royal Marsden cancer centres, where recruitment to trials is higher than in DGHs and tertiary centres, to identify transferable elements of best practice. Focus groups will be held remotely, via the online Zoom platform.

Focus groups facilitated by two researchers, will last one hour, and will be recorded and transcribed. Data will be thematically analysed using the Framework Method, to enable comparisons across groups to be identified and themes generated.

Phase 3: Phase 1\&2 findings will inform development of the LCN research recruitment tool. The tool, available in paper and online formats, will include information on finding out about LC clinical trials, the role of research teams, embedding research into multidisciplinary team meetings and health needs assessments, communication pathways, signposting LCPs, practical considerations and reaching underrepresented groups. Tool development will be guided by the Trial Steering Group (TSG), with representation from LC patient and public involvement (PPI) members, LCNs, multidisciplinary research and clinical teams. The tool will be tested for face and content validity by \~10 LCNs who participated in Phase 2, and \~4 PPI representatives. Relevant feedback will be used to make modifications to the tool.

Phase 4 pilot: Participants in the pilot study will pilot the research recruitment tool for proof of concept across four UK NHS sites, including tertiary centres and DGHs: Oxford, Derby\&Burton, Lanarkshire, Nottinghamshire. An additional two NHS sites, Surrey and Birmingham, will act as controls.

Each site's LCNs (n=36) will be invited to participate through their senior nurse manager who will provide eligible nurses with an online invitation letter and participant information sheet (PIS). If they are happy to proceed the researcher will organise a time, date and place with participants for the pilot phase of the study. Nurse participants will then have a briefing session, where the pilot study purpose will be explained. LCNs at the pilot sites (n=24) will be provided with a training session on using the tool, before implementing it in their teams for six months.

Survey data will be collected from each LCN (n=36) at baseline, three and six months and LCN's will reconsent electronically, via the Qualtrics survey platform, prior to completing each online survey. The survey will collect information on the following: 1) Items from the validated General Perceived Self-Efficacy Scale will measure LCNs' self-efficacy (primary outcome measure) in relation to their research roles 2) The number of LCPs each LCN has approached to discuss clinical trial opportunities will be recorded 3) Likert style survey data on LCNs' knowledge, confidence and awareness of clinical trials will be collected. These questions will be developed from phase 1\&2 findings and tested for reliability and validity by TSG members. The repeated tests measure ANOVA will compare survey responses between baseline, three and six months. Between groups ANOVA will compare differences in scores between pilot and control sites. No formal sample size calculation is required as this is a pilot study; however, all LCNs across the six sites will be invited to take part (n\~36).

Towards the end of the pilot, eight LCNs who have been taken part in the pilot will be invited, via an invitation letter and participant information sheet provided by the research team, to attend a 30-minute interview, via the online Zoom platform, or via telephone, to explore the tool's acceptability, in terms of ease of use and impact on recruitment.

Eight patients/carers who have joined clinical trials at the pilot sites will also be interviewed remotely, via the online Zoom platform or by telephone to explore their clinical trial experience and its impact on quality of life, care satisfaction, self-efficacy and symptom control will be explored. This will provide valuable information on the tool's impact on LCP experience.

Improving Access and Recruitment to Clinical Trials for Lung Cancer Patients

IPD will not be shared with researchers outside of the study team.

Data will be collected from each lung cancer nurse (n=36) at baseline, three and six months. Items from the validated General Perceived Self-Efficacy Scale (GSE) will measure LCNs' self-efficacy in relation to their research.

The total score is calculated by finding the sum of the all items. For the GSE, the total score ranges between 10 and 40, with a higher score indicating more self-efficacy.

Data will be collected from each lung cancer nurse (n=36) at baseline, three and six months on their knowledge of clinical trials. Data will be collected via a non-validated Likert style survey with five options ranging from 'strongly agree' to 'strongly disagree'.

Data will be collected from each lung cancer nurse (n=36) at baseline, three and six months on their confidence in talking to lung cancer patients about clinical trials. Data will be collected via a non-validated Likert style survey with five options ranging from 'strongly agree' to 'strongly disagree'.
