Patients receive ibrutinib at standard dose and schedule through an ongoing MD Anderson clinical trial. Patients that respond to ibrutinib but experience relapse or disease progression receive treatment based on the results of the PDX models as in Cohort 1 if they are available. Patients who experience relapse after treatment with ibrutinib are moved to Cohort I if the PDX models are not ready.

This early phase I pilot trial studies how well patient-derived xenografts work in personalizing treatment for patients with mantle cell lymphoma that has come back (relapsed) or that isn't responding to treatment (refractory). Xenograft models involve taking a piece of tissue from a tumor that was previously collected and putting that tissue inside of a mouse in the laboratory. This allows the tumor to grow in the mouse so that researchers can test the effects of certain drugs. If the drugs have an effect on the tumor(s) in the mice, patients may receive that treatment for mantle cell lymphoma.

PRIMARY OBJECTIVES:

I. To determine the feasibility in the patients treated with the patient-derived xenografts (PDX)-directed therapies in both cohorts (cohort 1 and cohort 2).

II. To establish mouse xenografts from the mantle cell lymphoma (MCL) tissue samples of patients with relapsed MCL.

III. To determine the activity of a panel of anticancer drugs consistent with the patient's clinical history against these MCL cells in vitro.

IV. To test the best selected in vitro options in the PDX model to create a profile, in rank order based on the efficacy of best 3-5 options, of individualized patient treatment options.

V. To determine the feasibility of predicting the patient's response to therapy using a PDX-based strategy.

VI. To define susceptibility and resistance determinants to the drugs in xenografted tumors.

SECONDARY OBJECTIVES:

I. To determine the objective response (OR) rate (complete + partial responses).

II. To determine safety and toxicity. III. To determine progression-free survival (PFS).

EXPLORATORY OBJECTIVES:

I. To correlate detected gene mutations and changes in ribonucleic acid (RNA) and/or protein expression with treatment responses.

OUTLINE: patients are assigned to 1 of 2 cohorts.

COHORT 1 (TRADITIONAL COHORT): Patients who have previously received ibrutinib, acalabrutinib, PI3K inhibitor ACP-319, or BTK inhibitor BGB-3111 receive treatment through ongoing clinical trials at MD Anderson or standard of care. At the same time, previously collected tissue is used to develop PDX models and suitable drugs/regimens are tested in the PDX models. Patients then receive treatment based on the results of the PDX models through another clinical trial or standard of care.

COHORT 2 (CO-TRIAL COHORT): Patients receive ibrutinib at standard dose and schedule through an ongoing MD Anderson clinical trial. Patient-Derived Xenografts in Personalizing Treatment for Patients With Relapsed/Refractory Mantle Cell Lymphoma

A Pilot Study Evaluating Xenografts to Personalize Therapies (PDX) in Relapsed Mantle Cell Lymphoma to Optimize Response: The EXPLORE Trial

This will be established if both PDX model is built successfully based on the primary tumor samples, and the results of the PDX results are available before disease relapse. We consider the study feasible if the posterior probability of feasibility rate above 30% is larger than 80% at the end of the study. The feasibility rate in each cohort will be summarized by frequency and 95% posterior credible interval.

Logistic regression will be utilized to assess the effect of patient prognostic factors on the response rate. Will be estimated by a Bayesian posterior credible interval, and summarized by frequency and 95% posterior credible interval.

Toxicity data by type and severity will be summarized by frequency tables. Will be estimated by a Bayesian posterior credible interval, and summarized by frequency and 95% posterior credible interval.

The distribution of PFS will be estimated using the method of Kaplan and Meier. Cox proportional hazard regression will be employed for multivariate analysis.

This is a multicentre prospective study of the feasibility and clinical value of a diagnostic service for identifying therapeutic targets and recommending personalised treatment for children and adolescents with high-risk cancer.

This is a multicentre study conducted under the Zero Childhood Cancer Program. The study will be enrolling patients under the age of 21 with high-risk cancer over 3 years from cancer centres in Australia. Patient's cancer cells will be tested for genetic abnormalities (mutations) and undergoing drug testing in highly specialised laboratories. A Multidisciplinary Tumour Board comprising of oncologists, clinical geneticists and scientists will then discuss the results of each case and determine whether a personalised medicine recommendation can be made. A report describing the results and Tumour Board recommendation (if any) will be provided to the patient's treating doctor. It is always at the discretion of the treating doctor whether to alter the patient's management based on the information arising from this research project.

Preclinical models of urogenital carcinoma have been emerging as a way to pre-determine drug resistance before therapy is targeted. The implantation of tumor specimens in the chorioallantoic membrane (CAM) of the chicken embryo results in a high-efficiency graft, thus allowing large-scale studies of "tumor avatar". The aim of the study is to develop a tumor culture platform for treatment evaluation. Biopsies will be collected from primary tumors of patients and grafted onto the chorioallantoic membrane of chicken embryos. After tumor implantation at the CAM, tumor growth will be accompanied by imaging that will quantify tumor vascularization, tumor volume, and tumor blood flow. Following tumor growth, "avatars" will be divided into different treatments. Using the "tumor avatar" model together with patient tumors, the investigators will be able to observe the individualized tumor response for each patient in a treatment context, as well as determine the potential drug to be used in each case. These results may support a phenotype-based reading within 7-10 days.

Introduction

1. Phenotypic Evaluation Model

   In 1874, William Roberts and John Tyndall observed the inhibition of cell growth on the agar plate. In the 1940s, Heatley suggested the use of filter paper discs containing antimicrobial solutions and introduced the "radial disc method".

   WHO released a report on the methodology and the reference method in 1975, which support the international standard of Clinical Laboratory Standards. The phenotype response of microorganisms to antibiotic treatment, comparing the proliferation of colonies might offer a rationale to the herein proposed tumor treatment response phenotype platform (patient-derived tumor avatar).
2. Cancer Treatment on Patient-Derived Xenografts (PDX)

   To propose an individualized treatment, it is necessary to know the tumor treatment response phenotype, which varies with the host. Studies using xenografts are being used to predict a response to certain drugs.
3. Chorioallantoic Membrane

The chorioallantoic membrane (CAM) assay has been used as a fast, reproducible method for antitumor drug testing in vivo. This assay has been widely used for the study of angiogenesis and has also been successfully developed in a xenograft tumor model, including urogenital tumors, due to the total non-development of the lymphoid system of the embryo. The new techniques have been studied, generating consequently new protocols. The major challenge for the xenograft model in CAM is a relatively high occurrence of embryonic death after egg removal, with a mortality rate between 25 and 50%.

The chick eggs will be used because they present a favorable vascularization environment. After the growth of the CAM, which is after 8 days of egg fertilization, access to the blood vessels is highly facilitated, causing them to be manipulated and observed. This makes it able to sustain systems and cells in the way that complex cancer studies can be done.

Blood vessels in addition to nourishing the development of -allo and xenografts, this redefined a supportive environment unique to intravasation and dissemination of tumor cells.

The CAM is highly vascularized, which makes it a rich medium for tumor implantation, in addition to other factors such as proteins. This support ends up being exclusive to the invasion, dissemination, and vascularization of tumor cells.

Treatment of genitourinary tumors is not yet individualized, due to the high heterogeneity of the tumors. The choice of treatment that the patient will get is defined by population studies that present inadequacies when transferred to the context of the individualized treatment.

The cell culture and organoids are a simple model to be applied, having a low cost and fast results. However, when the method is applied to the study of tumors, the results do not represent tumor environment conditions, forcing the cells to an adaptation that interferes with their growth and response to treatments patterns. Thus the xenografts models overcome some restrictions of the culture so that the framework is solid for its development. In contrast, the creation of a xenografts protocol becomes more complex, but it is a more representative method of cancer in its natural environment. CAM was studied with two strands, in-ovo, and ex-ovo. For in-ovo studies, the protocol was exposed by cutting eggshell.

The ex-ovo studies focus on improving the accessibility of the CAM and the embryo, enabling its documentation and manipulation.

Recently it has been described a correlation between renal tumors and drug resistance using exogenous CAM xenograft model. This causes other aspects to be monitored, studied and improved, especially regarding angiogenesis and research with tumors.

Hypothesis

There is increasing evidence that the diversity of the patient and cancer represent the main challenge in their treatment, which justifies the search for a clinical tool capable of providing information on the phenotype of response to treatment using the chorioallantoic tumor (CTA) platform/avatar.

Just as the antibiogram already established in clinical practice anticipates the susceptibility of bacteria to antibiotics, there is room for individualized treatment with potential benefit in cancer outcomes, quality of life and cost-effectiveness.

To define and anticipate the best treatment ("tumor chemogram") for each patient with urogenital cancer using treatment response phenotype platform, as the tumor avatar in the chorioallantoic membrane can transform the clinical practice.

Objectives

* To verify the efficiency of the sowing of fresh (immediate) and frozen tissue -80 degrees (in 24 and 48h) as a strategy in cases of failure of primary sowing;
* To evaluate the stability of the neoplastic tissue and the maintenance of its characteristics (genomic, proteomic, metabolic and phenotypic) in the proposed platform;
* To evaluate the response phenotype to the treatment of urogenital carcinomas in xenograft.
* To explore the impact of the proposed platform implementation on individualized treatment, its cost and results (oncological and functional) in patients with urogenital carcinomas.

Proposed Methodology

A prospective observational study including 30 biopsies of urogenital tumors from patients of the respective hospitals. Each biopsy will generate up to 12 tumor explants of 1 to 2 mm2, which will be implanted individually on the chorioallantoic membrane of 1 chicken embryo to test the 10 proposed drugs, plus 2 control eggs.

Therefore, 12 embryos will be needed for each biopsy. Biopsies of urogenital primary tumors of patients after consent will be collected, provided by the "Hospital das Clínicas da Unicamp" and "Hospital Municipal de Paulínia".

The material will be collected and placed in culture medium (McCoy) and kept on ice within 24 hours after resection. Part of the tissue fragments will be rapidly frozen and stored at -80oC for further studies.

It will be reserved a safety sample in ideal conditions of refrigeration for a sowing of rescue in cases of failure of the previous attempt. Samples will be divided where possible in:

* Fresh tumor: mitochondrial assay, culture, western blot
* Tumor formol: histopathology, immunohistochemistry
* Tumor culture: chorioallantoic membrane, frozen For the tumor biopsy graft, the CAM will be used when the embryos are 8 to 10 days old.

First, the CAM layer will be gently ruptured with the aid of a forceps, then the biopsy of the tumor will be placed near a Y-shaped bifurcation of a blood vessel. After placement of the tumor, 200ul of PBS will be placed to prevent contamination. After the graft comes into contact with the vascularization of the CAM, tumor growth will be monitored in a period of 3 to 4 days.

This process will be evaluated and if there is tumor growth the system will go to the treatment phase. Since there was tumor growth, the treatments will be performed from 17 days after the embryo. The treatments used will be doses of the following medicines: cisplatin, gemcitabine, paclitaxel, doxorubicin, carboplatin, pazopanib, sunitinib, sorafenib, methotrexate, and vinblastine. For the concentration of the drugs, they will be placed next to the filter paper in the tumor regions. This treatment will take from 3 to 4 days, after which analyzes will be carried out to determine the potentiality of each drug.

Euthanasia of the embryo will occur by decapitation (according to the protocol approved CEUA 4874-1 / 2018) since it already has a developed pain system.

Scissor cut through the CAM around the attached graft, transferred to a Petri dish with PBS. The excess of CAM will then be removed, leaving only the graft to be analyzed by histopathology, immunohistochemistry, western blotting and mitochondrial evaluation.

Risks

The risks involved in the participation of this study are minimal, as there is no interference in the treatment of the subject of the research, only the use of a small part of material withdrawn in the surgery that will be filed to be used in the laboratory in order to know even more about the disease, specifically identify future treatments with the best chances of results if necessary.

Benefits

This study will allow a greater knowledge of the disease with a potential benefit in the choice of future treatments, without altering any aspect of the current treatment that the research subject is receiving (the best standard treatment available at the time).

Data Analysis Methodology

Comparisons between primary tumor groups versus cultivated in xenograft will be performed using one-way ANOVA with the post hoc Bonferroni test. Data will be analyzed using Student t-tests or ANOVA repeated measures (Bonferroni post hoc test). The level of significance will be defined as less than 0.05 using Prism 5 (GraphPad software).

Mini patient derived xenograft (MiniPDX) is a drug sensitivity test model established by transplanting primary human tumor cells into immunodeficient mice by special methods. The investigators intend to use the Second-generation sequencing（NGS）and MiniPDX drug sensitivity models to guide the treatment decision-making for patients who were resistant to abiraterone, enzalutamide or other new second-generation anti-androgenic drugs. In order to develop precise personalized treatment plans for patients and extent their lifetimes.

Most patients with metastatic prostate cancer are effective in endocrine therapy at the beginning, but after a median survival of 12 to 18 months, almost all patients develop castration-resistant prostate cancer (CRPC). Since the pathogenesis of CRPC is still unknown, the clinical lack of precise treatment for the cause is a difficult and hot topic in current research and treatment. Mini patient derived xenograft (MiniPDX) is a drug sensitivity test model established by transplanting primary human tumor cells into immunodeficient mice by special methods. This efficient drug sensitivity test can provide sensitivities of single drug or drug combination in order to screen out the optimal individualized regimens for each patient. Personalized Mini-PDX for Metastatic CRPC

A Single-center, Open, Real World and Prospective Trial of Personalized Mini Patient-Derived Xenograft (MiniPDX ) Modeling in Adult Patients With Metastatic Castration Resistant Prostate Cancer

The ratio of number of participants with evidence of a confirmed complete response (CR) or partial response (PR) to all participants is objective response rate (ORR) by using Response Evaluation Criteria In Solid Tumors (RECIST) Version 1.1 to evaluate.

Progression-free survival (PFS) is defined as the time from the date of the first administration of patients medication plan based on MiniPDX drug sensitivity test to the date of the first documentation of disease progression or death due to any cause, whichever comes first, censored at the last date at which the participant was determined to be progression-free.

Overall survival is defined as time from initiation to death of any cause.

Adverse Drug Reaction：Adverse events determined according to CTCAE (version 4.03) and attribution to study treatment.

Overall clinical consistency（accuracy） as assessed by evaluating Response Evaluation Criteria In Solid Tumors (RECIST) criteria in patient tumor and correlating to tumor regression in Mini-PDX model for same drug treatment.

All women eligible for this protocol will fall into this group. These women will have histologically confirmed triple negative breast cancer and be eligible for neoadjuvant chemotherapy or have evidence of metastatic disease.

Patients are treated with neoadjuvant chemotherapy as according to the standard of care guidelines. Patients with metastatic disease are treated according to standard of care guidelines.

This study will determine if patient-derived tumor xenograft (PDX) mouse models can serve as a reliable model for treatment response for individual patients with triple negative breast cancer. The collection of patient tumor tissue will also provide insight into the mechanisms of therapeutic resistance for those individuals. Ultimately, this study will enhance our understanding of the genomic basis for treatment response for triple negative cancer on an individual basis, while having the potential to suggest new therapeutic options for high-risk triple negative breast cancer patients with residual disease post neoadjuvant.

Triple negative breast cancer (TNBC) represents an aggressive, genomically heterogeneous, subtype of breast cancer with limited therapeutic options. Many patients with TNBC receive standard neoadjuvant chemotherapy (NAC) pre-operatively; response is directly correlated with long-term outcome. Patients with residual disease after NAC are at higher risk for recurrent or metastatic disease, but additional adjuvant therapies are not currently part of standard care. Personalized treatment for patients with TNBC requires an improved understanding of the genomic landscape of individual TN breast tumors, as well as improved predictive models of response to specific therapeutic agents. This pilot study will determine if patient-derived tumor xenograft (PDX) mouse models can serve as a reliable model for treatment response for individual patients with TNBC. The tumor tissue collected (paired pre-and post NAC) will also provide insight into mechanisms of therapeutic resistance for individual patients. This study will enhance our understanding of the genomic basis for treatment response for TNBC on an individual patient basis, and lead to potential new therapeutic options for high-risk TNBC patients with post-NAC residual disease.

Objectives:

1. To collect and bank tissue specimens from patients with operable triple negative breast cancer at: a) at time of diagnosis, prior to the start of neoadjuvant therapy and b) at time of definitive surgery, after the completion of neoadjuvant therapy.
2. To collect and bank tissue specimen for patients with advanced metastatic triple negative disease prior to start of any line therapy for metastatic disease.
3. To create patient-derived xenograft (PDX) mouse models utilizing specimens collected pre- and post- neoadjuvant treatment
4. To determine if response of PDX tumors to cytotoxic agents in the Champions Oncology Tumor GraftTM (COTG) assay correlates with responses observed in patients.

Participants with HNSCC who will undergo curative surgery will be included in this group. This involves PDX development only, no drug testing will be done on the PDX.

Participants with RMHNSCC who are under palliative treatment will be included in this group. Drug testing on PDX per Investigator's choice (upto 4): PDX will be developed and upto four Chemotherapeutics (that are funded in Ontario) will be tested on the PDX. Chemotherapeutics will be selected at the discretion of the treating Medical Oncologists. Result of the drug testing will be provided to the responsible physician and can be utilized in patient care.

Primary objectives:

1. Evaluate the feasibility of rapidly accruing 30 participants with recurrent metastatic head and neck squamous cell carcinoma for the development of patient derived xenographs (PDX) from fresh, real time biopsies in which sensitivity to \< 4 Ontario funded chemotherapeutic regimen will be tested. Written feedback to the primary oncologist will be provided.
2. There is also a curative intent cohort of 30 participants undergoing surgical resection with curative intent. These PDX models will undergo exome sequencing with written feedback.
3. Feasibility in both surgical and recurrent cohorts will be a measure of i) engraftment rate, ii) patient status at the time of drug testing completion and iii) rate of accrual.

Tumour samples from 30 participants with HNSCC that undergone curative surgery will be used to establish patient-derived xenografts (PDXs). These tumours will also undergo exome sequencing. This will provide a biobank of PDX models with available genomic information for future research projects. Another 30 participants with recurrent or metastatic HNSCC (RMHNSCC) will be recruited to this study. PDX models will be developed from these patient tumours, followed by genomic sequencing. PDX models are developed by transplanting small tumour pieces into immunocompromised mice. These mice are then treated with different available drugs for RMHNSCC at the discretion of their medical oncologist. These mice will be then followed up to examine the tumour response to treatments. When studied in clinic, PDX models have shown high correlation with patient response to the treatment. The PDX drug testing results will be provided to the treating medical oncologist to guide care at the oncologists discretion. Investigators' hope is that improved chemotherapy responses are observed with this strategy.
