Primary objectives:

- To determine the time to progression in subjects with breast cancer metastatic to bone only treated with 153Sm-EDTMP and Autologous Peripheral Blood Stem Cell Transplantation (PBSCT).

Secondary objectives:

* To determine the overall survival at one year.
* To determine the efficacy of 153Sm-EDTMP at 6 months.
* To assess pain in subjects treated with 153Sm-EDTMP.
* To determine the safety of 153Sm-EDTMP.

153 Sm-EDTMP is designed to provide radiation to the bone and bone marrow. The radiation may kill cancer cells.

Before you can start treatment on this study, you will have "screening tests." These tests will help the doctor decide if you are eligible to take part in this study. Your complete medical history will be recorded. You will have a physical exam. Blood (about 2 teaspoons) and urine will be collected for routine tests. You will have a breast exam, mammogram, and computed tomography (CT) scan. You will have a bone marrow aspirate/biopsy. To collect a bone marrow aspirate/biopsy, an area of the hip or chest bone is numbed with anesthetic, and a small amount of bone marrow/bone is withdrawn through a large needle. Blood (about 2 teaspoons) will be drawn to check the status of the disease. You will have x-rays, a bone scan, a positron emissions tomography (PET) scan, and a bone density test. Blood (about 2 teaspoons) will be drawn to check for HIV (the virus that causes AIDS), CMV (cytomegalovirus), Hepatitis B, and Hepatitis C (viruses that can infect the liver). You will have an electrocardiogram (ECG -- a test that measures the electrical activity of the heart). You will have an echocardiogram or multiple-gated acquisition scan (MUGA). An echocardiogram uses sound waves to make pictures of your heart, which helps show how well your heart pumps blood. You will be asked to lie on your left side while a technician places a probe with gel on your chest to create images of your heart to determine the function and size. You will have a lung function test to make sure your heart and lungs are healthy. You will complete a questionnaire about the pain you are experiencing. The questionnaire will take about 15 minutes to complete. Women who are able to have children must have a negative blood (about 2 teaspoons) pregnancy test.

If you are found to be eligible to take part in this study, on Day 1 you will receive a tracer dose. A tracer dose is a small amount of the study drug used to measure how the study drug is affecting the body. Your weight will be measured. Before the tracer dose is injected, you will be given fluids through a needle in your veins starting from 2 hours before the tracer dose up to 6 hours afterwards. You will receive a small amount of 153 Sm-EDTMP, called a tracer dose, through a needle in your vein.

After the tracer dose, you will have pictures taken of your body with a gamma camera 3 times that day. These photographs will be taken immediately after the dose is injected, and then 2-3 hours and 4-6 hours later. You will need to lie flat for these imaging sessions for about 10-30 minutes each time.

On Day 2, you will have 1 picture taken of your body with the gamma camera.

If, based on the photographs taken, doctors find that the drug does not go to your bones as expected, you will be removed from the study. If you leave the study at this point, you will have return to the clinic 2 times a week for the next 4 weeks. At these visits you will have a complete physical exam, and you will be asked about any side effects you may be experiencing.

If enough of the study drug does go to your bones, you will receive a higher dose of 153 Sm-EDTMP, called a therapy dose, 7-14 days after the tracer dose.

Before the therapy dose begins, you will have a physical exam, including measurement of your vital signs. Fluids will be given through a needle in your vein beginning 2 hours before the therapy dose and continuing up to 6 hours after.

You will receive the injection of 153 Sm-EDTMP through your central line. The injection will take about 15 minutes. Afterwards, your vital signs will be measured. Blood (about 2 teaspoons) will be drawn for routine tests.

The therapy dose of 153 Sm-EDTMP is expected to destroy your body's ability to make blood cells. To restore your blood-making ability, you will be given back stem cells (blood-forming cells) that will have already been taken from your body and stored for this treatment. This is called an autologous peripheral blood stem cell transplant. You will receive the stem cells given through a needle in your vein 14-21 days after the therapy dose usually as an outpatient procedure.

Family members may not be allowed with you until radiation subsides. This will reduce their exposure to radiation. You will be given instructions on how to follow radiation safety practices.

Once the radiation subsides you will be released from the hospital, at least 2 times a week you will have a complete physical exam, and you will be asked about any side effects you are experiencing. Blood (about 2 teaspoons) will be drawn up to 3 times a week until your stem cell transplant. Your vitals signs will also be checked 2 times a week.

After the transplant, at least 2 times a week for up to two (2) weeks your blood (about 2 teaspoons) will be drawn for routine tests. You will have a physical exam and your vital signs will be checked. You will receive additional care, drugs, or blood transfusions, as needed.

You will return for follow-up visits every 3 months until the end of Year 2 and then every 6 months in Years 3 and 4. Tests may occur more often if a side effect is noticed. Any side effects that you develop will be monitored until it goes away. Some of these exams and tests may take place with your home doctor, and these records will need to be sent to the study doctor so he/she can review them for careful monitoring.

In order to check the response of the cancer to the study therapy, you will have chest x-rays, CT scans, and bone scans every 3 months for the first 2 years after transplant. These will be repeated every 6 months in years 3-4 and once a year after that, as long as your cancer does not get worse. If the breast cancer has spread to your bone marrow at the time you enter this study, bone marrow biopsies will also be done at these times to see whether the cancer is responding. You will also have a PET scan at 6 months after transplant, and more often if your physician believes it will be helpful. Mammograms and bone density exams will be done once a year.

Once a year for the 10 years after you leave the study, you will have a study visit or you will be contacted by telephone. You will be asked about your general health, hospitalization, and cancer treatments.

This is an investigational study. 153 Sm-EDTMP is FDA-approved and commercially available for bone pain control. Up to 40 patients will take part in this study. All will be enrolled at M. D. Anderson.

Primary Objectives:

1. To determine the efficacy of administering multiple doses of intravenous (i.v.) busulfan at a dose of 130 mg/m2, to yield a systemic plasma drug exposure represented by a daily area under the plasma concentration versus time curve (AUC) of approximately 5,000 mMol-min for 4 days, followed by i.v. melphalan at a dose of 70 mg/m2 for 2 days in adult patients receiving autologous or allogeneic transplantation for lymphoid malignancies or myeloma.
2. To describe the plasma pharmacokinetic (PK) profiles of busulfan and melphalan in this regimen.
3. To determine the disease-free and overall survival of patients receiving this preparative regimen.
4. To determine the treatment-related morbidity and mortality of this combination of drugs.

Busulfan and melphalan are both traditional alkylating agents that are designed to interfere with the production of cancer cells at the DNA (deoxyribonucleic acid) and RNA (ribonucleic acid) level.

Before you can start treatment on this study, you will have what are called "screening tests". These tests will help the doctor decide if you are eligible to take part in the study. You will have a complete physical exam, including routine blood (2-3 teaspoons) and urine tests. Patients will have a chest x-ray, heart scan, lung function test, and a bone marrow biopsy. Women who are able to have children must have a negative blood pregnancy test.

If you are found to be eligible to take part in this study, you will be able to start receiving chemotherapy treatment with busulfan and melphalan. Participants who agree to the optional blood draws described above will at first receive a therapeutic "trial dose" of busulfan by vein to test the blood levels over time. This therapeutic trial dose of busulfan is about 25% (1 fourth) of the full therapeutic dose of the drug. This information will be used to decide what the future high-dose busulfan treatments you receive will be. If you do not agree to the optional blood draw, you will receive a fixed amount of high-dose busulfan from the start.

On the 1st day of hospitalization, you will receive fluids by vein through a central venous catheter. If you choose the optional busulfan dose for pharmacokinetic-based busulfan dosing, you will receive the optional busulfan dose 9 days before stem cell infusion (Day 1), followed by a rest day on Day 2. If you choose to receive a fixed dose of busulfan, busulfan will be injected through a central venous catheter over 3 hours, once a day, for the next 4 days (Days 3-6, ending 3 days before the stem cell infusion day).

Patients receiving pharmacokinetic-based dosing of busulfan will also continue to receive busulfan on Days 3-6. This will be followed by melphalan for all patients, given through your central venous catheter over 30 minutes, once a day, for 2 days, on days 8 and 9. Your stem cell infusion day will be on Day 10 of treatment.

Patients receiving 5 out of 6 antigen matched-related allogeneic stem cell transplants or unrelated allogeneic stem cell transplants will also receive antithymocyte globulin (ATG), by vein, on Days 7-9, up to one day before the stem cell infusion. This is given to decrease the risk of GVHD and graft rejection in mismatched transplants.

On Day 10, healthy blood stem cells or bone marrow from the donor will be given through the central catheter. This is your transplant date. You will also receive several other medications to help the treatment work and to help prevent infections while your immune system is weak. Tacrolimus and methotrexate will be given to decrease the risk of graft-versus-host-disease (GVHD). GVHD occurs when the donor's immune cells fight the patient's body. The tacrolimus will be started on the day before the transplant and will continue for up to six months. Tacrolimus is given by vein at first and then by mouth when you are able to eat. Methotrexate is given by vein on Days 11, 13, 16, and possibly on Day 21, up to 11 days after the transplant.

Please note that the treatment dates listed above were used to help explain your general treatment plan. By standard medical convention, the day of stem cell infusion is always listed as day zero. Therefore, the days listed above are different from the treatment plan described in the protocol and abstract.

Sulfamethoxazole (Bactrim) or pentamidine will be given to fight bacteria. Bactrim is given by mouth when the counts are good. Pentamidine is given by vein when the counts are low. Acyclovir will be given at first by vein and then Valtrex (valacyclovir) will be given by pill to decrease the risk of viral infections. Granulocyte colony-stimulating factor (G-CSF) will be given to help the new bone marrow grow. It is given as an injection under the skin after the transplant. It will continue until the white blood cells reach an acceptable level. Overall, some of these drugs will be given for as long as 6 months or possibly longer. Other medications may be necessary. If you are allergic to some of these drugs, changes will be made.

You will be in the hospital for about 3-4 weeks. You will have checkups every day until discharged from the hospital. You will then be seen in the outpatient clinic at least 3 times a week until your blood counts improve. You will be seen by your doctor at least every week until 100 days after the bone marrow transplant. You must stay in Houston during this time. After 100 days, you will return to the clinic according to your individual physician's recommendations.

Some patients may need to receive spinal taps with instillation of chemotherapy several times over the year after transplantation. This is only for patients with a previous clinical history of leukemic involvement of the brain or high risk of developing leukemia relapse in the brain. The spinal tap is performed in the clinic. You are given local anesthetic at the lower back site, a small needle is inserted in the space between 2 spinal bones, a small amount of fluid that bathes the brain (cerebrospinal fluid) is removed for testing, and a small amount of chemotherapy is given.

Bone marrow samples will be taken at about 1 month and 3 months after the transplant. You will also have a lung function test at 3 months after the transplant.

This is an investigational study. The FDA has approved all of the drugs used in this study for use in stem cell transplantation. Up to 168 patients will take part in this study. All will be enrolled at M. D. Anderson.

The purpose of this study is to evaluate the difference in relapse rates and long term event free survival in patients with intermediate grade or immunoblastic non-Hodgkin's lymphoma (NHL) whose marrow is not obviously involved with NHL who are randomized to receive either an autologous bone marrow (ABMT) or peripheral stem cell transplant (PSCT). All patients with intermediate grade NHL with histologic negative bone marrow who would otherwise meet all eligibility criteria for high-dose therapy and ABMT are eligible for this study. Patients who are eligible will be randomized to either PSCT or ABMT at the time of enrollment into our transplant program.

These patients would be enrolled in a high-dose protocol using carmustine, etoposide, cytarabine, and cytoxan (BEAC) with autologous hematopoietic rescue. The patients will be stratified according to good and poor prognosis category and relapsed vs. first partial response categories. The patients will have bone marrow or peripheral stem cells collected according to standard protocols. A standardized hematopoietic growth factor will be used for mobilization and post-transplant. The patients' PSC or bone marrow (BM) product will be assayed via invitro-culture techniques for occult tumor, and by molecular biologic assays. The patients' outcome for transplantation will be evaluated with response to transplantation, relapse rates and event free survival being the measured end points.

Patients will be treated on the most medically appropriate regimen with a preference for Flu/TBI Arm followed by an infusion at Day 0 of Alpha/Beta T Cell-Depleted Hematopoietic Stem Cells.

Flu/Bu/Mel will the preference for patients with JMML or infants with leukemia.

Phase II, Open-Label, Prospective Study of T Cell Receptor Alpha/Beta Depletion (A/B TCD) Peripheral Blood Stem Cell (PBSC) Transplantation for Children and Adults With Hematological Malignancies

For the purposes of data and safety monitoring, this study is classified as high risk (investigator initiated under an IND). Therefore the following requirements will be fulfilled:

* The Masonic Cancer Center Data and Safety Monitoring Council (DSMC) will review the study's progress at least quarterly.
* The PI will comply with at least twice yearly monitoring of the project by the Masonic Cancer Center monitoring services.
* The PI will oversee the submission of all reportable adverse events per the definition of reportable in Section 11.5 to the Masonic Cancer Center's SAE Coordinator, the University of Minnesota IRB, and the FDA.

Monitor median rate of engraftment by 42 days.

Determine the rate of graft failure by day 100 (defined as lack of achievement of an ANC \>=500/mL with associated pancytopenia)

Determine the incidence of non-relapse mortality (NRM) at 100 days and 1 year
