The treatment decisions and evaluation of treatment outcomes, such as the assessment of metastases resectability and tumor response, were carried out by the multi-disciplinary team (MDT) comprising experts from each medical centers. In this cohort, initially unresectable colorectal cancer liver metastasis (CRLM) patients who were successfully converted and achieved no evidence of disease status were treated with chemotherapy plus targeted therapy, as adjuvant therapy.

The two cohorts received different adjuvant therapy regimens, with the CT group receiving chemotherapy plus targeted therapy; the CA group received chemotherapy alone.

Chemotharapy plus targeted therapy regimen, as an adjuvant therapy, can effectively reduce the rate of both intrahepatic and extrahepatic recurrence in initially unresectable CRLM patients. Those with KRAS/NRAS/BRAF mutated tumors or cycle of conversion therapy ≤ 4 can benefit more from chemotharapy plus targeted therapyrather than from chemotharapy alone, with a tolerable toxicity profile.

This multi-center study enrolled consecutive patients with initially unresectable CRLM who underwent conversion therapy or radiofrequency ablation (RFA) and achieved no NED status between June 1, 2013, and June 30, 2020, from Zhongshan Hospital of Shanghai, Zhongshan Hospital of Xiamen, Cancer Hospital of Beijing. The inclusion criteria were as follows: (1) with histologically confirmed CRC; (2) with initially unresectable synchronous liver metastases (LMs); (3) accepted conversion therapy and successfully converted into resectable status; (4) underwent R0 intestinal and hepatic resection; (5) accepted adjuvant therapy. The exclusion criteria were as follows: (1) R1/R2 resection; (2) extrahepatic metastases; (3) accepted postoperative monotherapy; (4) lack of follow-up data. According to the adjuvant therapy regimen (with or without targeted therapy), the cohorts were divided into CA cohort and CT cohort. The study was approved by the ethics committee of three medical centers and was adhered to Good Clinical Practice guidelines and the Declaration of Helsinki. Written informed consent was obtained from all participants.

Lung cancer is a common disease leading to 18 % of cancer deaths worldwide. Despite various improvement in treatment, there still remains low 5-year survival rate of 10-20 % in advanced lung cancer patients. Skeletal muscle mass and physical performance have been shown to effect overall survival and prognosis in lung cancer. This research focuses on effects of different treatment of lung cancer such as chemotherapy, targeted therapy and immunotherapy on skeletal muscle mass and physical performance.

Lung cancer is a common disease leading to 18% of all cancer deaths worldwide. Prognosis of lung cancer depends on various factors like disease stage, socioeconomic factors, body composition factors such as sarcopenia have been shown to be associated with poor prognosis and survival. Sarcopenia is defined as decline in muscle strength, muscle quantity/quality (skeletal muscle mass) and physical performance. Diagnosis of sarcopenia is confirmed by demonstrating reduced skeletal muscle mass while physical performance can be used to assess severity of sarcopenia. In past 5 years there are some studies indicating an association between treatment of lung cancer and decline in parameters of sarcopenia. Although remarkable improvements in medical treatments for non-small cell lung cancer (NSCLC) based on chemotherapy, targeted therapy and immunotherapy have been made, advanced lung cancer patients still have low five-year survival rate at 10-20 percent. Notably with a high prevalence of 43%, sarcopenia is shown to be a significant predictor of worsening overall survival and poor prognosis in NSCLC patients.

Cancer patients have expedited decline in skeletal muscle mass as compared to normal age-related decline in patients without cancer. Majority of older studies on NSCLC patients show that there is decrease in skeletal muscle mass during chemotherapy. Data on changes in skeletal muscles in NSCLC patients receiving targeted therapy and immunotherapy are limited, however studies suggest that patients experience decline in skeletal muscle mass while receiving these therapies as well. During treatment, in addition to decline in skeletal muscle mass, non-small cell lung cancer patients also exhibit decline in other parameters of sarcopenia i.e., muscle strength and physical performance as well. The impact of decline in parameters of sarcopenia leads to functional impairment with poor response to treatment, increased drug toxicities and poor quality of life. The value of measuring sequential decline in parameters of sarcopenia is not clearly understood. It is believed that interventions to treat sarcopenia can evolve if we can understand effects of different cancer therapies on sarcopenia. Early screening and treatment of sarcopenia in these patients may help to prevent functional impairment and improve overall survival.

Although there are other causes of sarcopenia in cancer patients such as impaired nutritional intake, low physical activity, causes relating to cancer therapies are not well established. There still remains a wide knowledge gap in understanding decline in different parameters of sarcopenia while patients are on different types of lung cancer treatment. Therefore, we aim to study a prospective cohort to evaluate differences in rate of decline in skeletal muscle mass and physical performance in advanced NSCLC patients receiving chemotherapy and targeted therapy/immunotherapy. The skeletal muscle mass and physical performance will be evaluated using computed tomography (CT) and short physical performance battery (SPPB) respectively. There is lack of validated cut-off values for CT-defined skeletal muscle mass in Thai population. Therefore, in Thai population, we aim to validate the cutoff values set by Japanese Society of Hepatology(JSH) based on recommendation of Asian Working Group for Sarcopenia (AWGS). As latest consensus on sarcopenia highlights importance of physical performance, if there exist an association between CT-defined skeletal muscle mass and physical performance, the former can be used in predicting physical functioning of patients. Therefore, we intend to determine baseline association between CT-defined skeletal muscle mass and physical performance as well. As far as we know there are no previous studies done to evaluate the differences in decline in skeletal muscle mass in patients who are unable to complete their first line chemotherapy and receive other line of chemotherapy (chemotherapy agents which are not considered first line), therefore we intent to study these differences too. Finally, this study also includes an evaluation of overall response rate of chemotherapy in patients with low CT-defined skeletal muscle mass.

This study is based in Songklanagarind Hospital and enrollment of participants will be as per our inclusion criteria which includes advanced lung cancer patients who are more than 18 years old treated with either chemotherapy, targeted therapy or immunotherapy. The sample size required for the study was estimated using simulation in Stata, with an estimated total N=240. According to treatment recommended by primary chest physician, participants will be divided into groups of chemotherapy (CMT) and targeted/immunotherapy (TKI/IO). Measurement of variables will take place in four different time period. Baseline characteristics will be measured before starting the treatment. The outcome variables our study i.e., computed tomography (CT)-defined skeletal muscle mass, short physical performance battery (physical performance) and health related quality of life using Thai Version of EQ-5D-5L Health Questionnaire (EQ-5D-5L) will be measured before starting treatment (T1), after 3rd, 6th and 9th months (T2, T3 and T4) of treatment. Additionally, after 3rd cycle of treatment "response evaluation criteria in solid tumors" (RECIST v1.1) will be recorded to evaluate response rate of chemotherapy. For patients who are unable to tolerate first line chemotherapy, they will be recruited for our 1st secondary objective and patients in this group will be evaluated only for CT-defined skeletal muscle mass in subsequent time period. At the end of our study, we will have three groups of patients i.e., patients receiving first line chemotherapy (CMT) patients on first line targeted/immunotherapy (TKI/IO) and patients on other line chemotherapy (OCMT).

Measuring skeletal muscle area (SMA) at L3 vertebral level using computed tomography scan is used to estimate total body skeletal muscle mass (SMM). Height adjusted SMA, also called skeletal muscle index (SMI) can be calculated by dividing SMA by patient's height square. As our study is based on Thai population, cut-off for diagnosis of CT-defined sarcopenia will be taken as L3 lumbar vertebra (L3SMI) \<42cm2/m2 for men and \<38 cm2/m2 for women. Short physical performance battery (SPPB) test will be used to evaluate physical performance. SPPB consists of set of three tests including balance test, gait test and 5 times chair stand test. Patients are asked to stand side-by-side, semi-tandem and tandem to measure balance. And then patients are asked to walk 4 meters to assess gait speed. Chair stand test is performed by asking the patient to fold their arms and stand up from seated position for five times. Points from 0 to 12 will be given according to time required to perform above tests. Health related quality of life will be measured using the Thai Version of EQ-5D-5L Health Questionnaire. This is validated tool for use in Thailand which has a self-completed questionnaire for adults consisting of 5 dimensions of mobility, self-care, usual activities, pain/discomfort and anxiety/depression The baseline characteristics will be presented with n (%) and mean (SD) or median (IQI) as appropriate. Chi-square test (or Fisher Exact test), Student t-test (or Kruskal-Wallis test) will be calculated to evaluate the differences of baseline data between CMT and TKI/IO groups. The baseline correlation between SPPB and SMI will be presented with scatter plot and LOWESS line. The change of SMI after 6th cycles of treatments and the differences of the change between CMT and TKI/IO groups will be calculated using Student t-test or Kruskal-Wallis test. Multivariable linear regression will be used to adjust the imbalance of baseline characteristics between these two-groups. Decline in SMI for CMT and OCMT will be calculated using same analysis. The rate of decline in SMI (and SPPB score) will be calculated using multilevel-linear regression (Mixed linear regression) and will be shown in two-way graph. The response rate to chemotherapy will be analyzed using logistic regression. The p-value of less the 0.05 will be considered statistically significant. The analysis and graphically figures will be shown using Stata/MP 17.0, StataCorp, 4905 Lakeway Dr, College Station, TX 77845, USA.

Differences in Rate of Decline in CT-defined Skeletal Muscle Mass and Physical Performance in Patients With Advanced Non-small Cell Lung Cancer Receiving Chemotherapy and Targeted Therapy/Immunotherapy, Before and After Treatment.

Subjects will be grouped into one of three study groups (Groups 1, 2, or 3) based on their disease status and history of prior cancer therapy. Group 1 subjects will comprise treatment naïve subjects with localized disease and (i) are eligible for neoadjuvant treatment, (ii) have unresectable disease, or (iii) are medically unfit for surgical resection.

Subjects will be grouped into one of three study groups (Groups 1, 2, or 3) based on their disease status and history of prior cancer therapy. Group 2 will comprise treatment naïve subjects with metastatic disease.

Subjects will be grouped into one of three study groups (Groups 1, 2, or 3) based on their disease status and history of prior cancer therapy. Group 3 will comprise subjects with metastatic or unresectable disease who have received at least one prior systemic therapy, whether matched or unmatched.

The Molecular Tumor Board (MTB) will suggest molecularly targeted "matched" treatment.

Subjects will receive treating physician's choice of traditional systemic therapy treatment for their malignancy, defined by National Comprehensive Cancer Network (NCCN) guidelines and/or tumor board recommendation(s).

This is a prospective, open-label navigational investigation designed to evaluate the feasibility of using molecular profile-based evidence to determine individualized cancer therapy for patients with aggressive malignancies. This is a non-randomized, histology-agnostic trial. Although there will be a case mix of histologies, the investigators now know that individual histologies are composed of a heterogeneous mix of molecular alterations. It is not clear whether one case mix is better or worse than another. Thus, the investigators are testing a strategy of molecular matching that may apply across different cancers.

Eligible and consented patients, if not already performed, will have their tumor tissues/blood molecularly profiled. Patients will be stratified into Group 1 (treatment naïve, localized/unresectable/medically unfit for surgery), Group 2 (treatment naïve, metastatic), and Group 3 (prior treated). Based on multiomic profiling, matched therapy, if available, will be recommended by the Molecular Tumor Board. Patients who receive the recommended matched therapy are designated to Arm A. Otherwise, those that receive the unmatched therapy (i.e., treating physician's choice of therapy) or have no molecular alterations are designated to Arm B. The study feasibility will be measured by the ability to enroll patients, the acceptable turnaround time and the actionable information obtained from multiomic profiling, and the viability of identifying and delivering the matched therapy.

Background:

- Sunitinib is drug that is approved for treating various types of cancers, including kidney cancers. However, it has not been approved to treat cancers of the thymus. Sunitinib works by blocking proteins that are responsible for cell division and growth. Some of these proteins can be found on thymus cancer cells. Researchers want to see if sunitinib can be used to treat advanced thymus cancer. It will be given to people who have had at least one earlier chemotherapy treatment containing platinum.

Objectives:

- To see if sunitinib is a safe and effective treatment for advanced thymus cancer that has not responded to earlier treatments.

Eligibility:

* Individuals at least 18 years of age who have advanced thymus cancer that has not responded to earlier treatments.
* At least one previous cancer treatment must have been chemotherapy treatment containing platinum.

Design:

* Participants will be screened with a physical exam and medical history. Blood and urine samples will be collected. Imaging studies and tumor biopsies will be used to check the severity of the cancer.
* Participants will take sunitinib tablets once a day, in the morning. They will take the tablets daily for 4 weeks, followed by 2 weeks of rest with no sunitinib. This 6-week period is called a cycle.
* Treatment will be monitored with frequent blood tests and imaging studies.
* Treatment cycles may be repeated as long as the tumor does not continue to grow and there are no severe side effects....

BACKGROUND:

Platinum-based chemotherapy is the standard of care for advanced unresectable thymoma and thymic carcinoma. However over 50% of these patients may fail initial therapy and therefore require second-line therapy. New therapeutic options are needed for patients who have disease progression on or after platinum-containing therapy. Results obtained from protocol 12-C-0118 so far have shown impressive clinical activity of sunitinib in patients with recurrent thymic carcinoma with an objective response rate of 23% and disease control rate of 91% which is unprecedented for this histology. Treatment at a dose of 50 mg once daily for four weeks followed by 2 weeks off was poorly tolerated. Twenty five out of 41 patients needed dose reductions due to development of intolerable adverse events.

OBJECTIVES:

Primary objective:

- To evaluate the objective response rate (Partial Response (PR)+Complete Response (CR) for sunitinib in patients with relapsed or refractory thymoma or thymic carcinoma

MAIN ELIGIBILITY:

* Patients with histologically confirmed thymoma (Group 1 only) or thymic carcinoma who have previously been treated with at least one platinum-containing chemotherapy regimen with progressive disease prior to study entry
* Measurable disease by Response Evaluation Criteria in Solid Tumors (RECIST) 1.1 criteria
* Adequate renal, hepatic and hematopoietic function
* No major surgery, radiotherapy, chemotherapy or biologic therapy within 28 days of sunitinib

DESIGN:

* In the first group (Group 1), sunitinib will be administered orally using a continuous schedule at 50 mg per day for 4 weeks with 2 weeks off to constitute a 6-week cycle (Schedule 4/2) until disease progression or development of intolerable side-effects.
* In the second group (Group 2), sunitinib will be administered orally using a continuous schedule at 50 mg per day for 2 weeks with 1 week off to constitute a 3-week cycle (Schedule 2/1) until disease progression or development of intolerable side-effects.
* Toxicity will be assessed every cycle by Common Terminology Criteria for Adverse Events (CTCAE) Version 5.0
* Tumor response assessments by RECIST 1.1 criteria will be performed every cycle for Group 1 and every other cycle for Group 2 (every 6 weeks) for patients receiving treatment for less than one year, and every two cycles for Group 1 and every four cycles for Group 2 (every 12 weeks) for patients who have been receiving treatment one year or longer.
* Exploratory studies include evaluation of serum vascular endothelial growth factor (VEGF), placental growth factor (PlGF), interleukin 4 (IL-4), interleukin 12 (IL-12), hepatocyte growth factor (HGF), and basic fibroblast growth factor (bFGF) (Group 1 only); and circulating tumor cells, endothelial progenitors, and mature apoptotic endothelial cells (both groups). In Group 2, regulatory T cells (Tregs), exhausted cluster of differentiation 8 (CD8) T cells, myeloid-derived suppressor cells (MDSCs), and Type 1 T helper (Th1)/Type 2 T helper (Th1) T cell populations will also be evaluated. Where tumor samples are available, intra-tumoral immune infiltrate will be assessed (both groups). Exploratory studies apply to National Cancer Institute (NCI) only.

Sunitinib for Advanced Thymus Cancer Following Earlier Treatment

A Phase II Study of Sunitinib in Patients With Advanced Relapsed or Refractory Thymoma or Thymic Carcinoma With at Least One Prior Line of Platinum-Based Systemic Chemotherapy

All individual participant data (IPD) recorded in the medical record will be shared with study investigators upon request.

Here is the number of participants with serious and/or non-serious adverse events assessed by the Common Terminology Criteria for Adverse Events (CTCAE v5.0). A non-serious adverse event is any untoward medical occurrence. A serious adverse event is an adverse event or suspected adverse reaction that results in death, a life-threatening adverse drug experience, hospitalization, disruption of the ability to conduct normal life functions, congenital anomaly/birth defect or important medical events that jeopardize the patient or subject and may require medical or surgical intervention to prevent one of the previous outcomes mentioned.

Objective response rate (CR + PR) will be measured according to the Response Evaluation Criteria in Solid Tumors (RECIST) 1.1 criteria for sunitinib monotherapy in participants with advanced thymic malignancies. Complete response is disappearance of all target lesions. Partial response is at least a 30% decrease in the sum of the diameters of target lesions, taking as reference the baseline sum diameters.

Progression free survival is defined as the duration of time from start of treatment to time of progression or death, whichever occurs first. Progression was measured by the Response Evaluation Criteria in Solid Tumors (RECIST) v1.1 and is defined as at least a 20% increase in the sum of the diameters of target lesion, taking as reference the smallest sum on study; and the appearance of one or more new lesions.

Number of participants alive at 1 year after treatment with sunitinib.

Adverse events assessed by the Common Terminology Criteria for Adverse Events (CTCAE v5.0). Grade 3 is severe. Grade 4 is life-threatening. Grade 5 is death related to adverse event.
