This is a prospective, exploratory, randomised clinical trial. Patients with diagnosed cancer that are to be treated with 5-fluorouracil (5-FU) will be randomised into standard oncological treatment or a cardiological assessment prior to the 5-FU treatment. The investigators hypothesize that aggressive management of ischemic risk factors in asymptomatic patients will reduce the number of hospitalisations and investigations for acute coronary syndrome during and after 5-FU treatment and that patients with high coronary artery calcium scores are more likely to experience chest pain during the treatment with 5-FU.

Registry of patients with Tako-Tsubo cardiomyopathy

Prospective, multicenter, observational registry collecting data from subjects with Tako-Tsubo (stress-induced) cardiomyopathy (TTC). Uniform, complete, and accurate data will be collected on the subject's medical history, during index hospitalization for TTC, and during follow-up.

The objectives are to evaluate the prevalence and incidence of cancer in patients with TTC, to document the underlying causes of death during hospital stay and during follow-up, to determine the long-term prognosis, and to identify possible predictors of short and long-term mortality.

The prognosis of patients with TTC is believed to be favorable. This assumption is mainly based on the observations of a rapid recovery of global left ventricular function and low cardiac in-hospital mortality. In fact, the clinical course following TTC events after hospital discharge is largely unknown due to a lack of large prospective long-term follow-up studies and inconsistent and limited findings from available studies to date.

The TTAC registry represents the first attempt to initiate an international registry with special focus on comorbidities, particularly malignancies, in patients with TTC to further elucidate the relationship between these diseases. In addition, the TTAC registry will enable documenting the exact underlying causes of death and long-term prognosis of patients with TTC and to identify possible predictors of short and long-term mortality in these patients.

The first objective of this prospective, multicenter, observational registry is to evaluate the prevalence and incidence of cancer in patients with TTC.

The second objective is to document the underlying causes of death during hospital stay and follow-up and to determine the long-term prognosis of patients with TTC.

The third objective is to identify possible predictors of short and long-term mortality in patients with TTC.

Available information is collected from subjects being treated according to routine clinical practice. In addition, during the index hospitalization, each subject is screened for occult cancer disease. During follow-up, cancer screening will be performed according to appropriate legal preventive examinations.

Subjects will be followed at 6 months and annually after the index event for up to 5 years by phone call, office visit, or by contacts with primary physicians or referring cardiologists, according to each investigator's preference.

As regulated by data protection and privacy laws and in accordance with local ethics committee requirements, subjects will be informed and requested to grant their approval after the index procedure (= cardiac catheterization for suspected acute coronary syndrome) to review their medical records and collect and analyze personal medical information. The confidentiality of the records will be maintained at all times. Subjects will be asked to agree to perform a cancer screening during index hospitalization and will also be asked to agree to be contacted during the 5-year follow-up period.

Patients in whom TTC was diagnosed will be informed and requested to grant their approval to review their medical records, to collect and analyze personal medical information, while maintaining the confidentiality of the records at all times. Subjects will be asked to agree to perform a cancer screening during index hospitalization and will also be asked to agree to be contacted during the 5-year follow-up period. Informed consent will be requested after the index procedure (= cardiac catheterization for suspected acute coronary syndrome proving presence of TTC) and the informed consent form (ICF) has to be signed prior to entering subject's data in the database or prior to any follow-up contact, whichever occurs first.

The investigator and/or designee must clearly document the process of obtaining informed consent in the subject's source documents. The voluntary process of obtaining informed consent confirms the subject's willingness to participate in the registry. It is the investigator's responsibility to ensure that the informed consent process is performed in accordance with Good Epidemiological Practice (GEP) - IEA Guidelines for proper conduct of epidemiological research and where applicable country regulations.

Subjects will be followed at 6 months and annually after the index event for up to 5 years by phone calls, office visits, or by contacts with general practitioners or referring cardiologists, according to each investigator's preference.

* Faecal occult blood test
* Complete blood count including erythrocyte sedimentation rate

* Prostate-specific antigen (PSA) testing in men

The human gut microbiome has been associated with many health factors but variability between studies limits the exploration of effects between them. This study aims to systematically characterize the gut microbiota of various critical chronic diseases, compare the similarities and differences of the microbiome signatures linked to different regions and diseases, and further investigate their impacts on microbiota-based diagnostic models.

Many studies demonstrate that microbial dysbiosis has been linked to many human pathologies. However, the current understanding of the identification of the disease-associated microbiome signatures remains limited, largely owing to the heterogeneity of microbial community structures which are shaped by the host. Undoubtedly, profiles of microbial biomarkers require validation in large, independent, population-based cohorts from different districts. Based on these, the investigator plan to organize a multicentric cross-sectional cohort, not only to systematically characterize the gut microbiota of various critical chronic diseases, such as liver cancer, gastric cancer, pancreatic cancer, lung cancer, nasopharyngeal cancer, hypertension, acute coronary syndrome, primary aldosteronism, epilepsy, chronic kidney disease, and subclinical hypothyroidism but also to compare the similarities and differences of the microbiome signatures linked to different regions and different diseases and to further investigate their impacts on microbiota-based diagnostic models. In this study, for each kind of disease, the investigators expect to recruit 500 patients with a confirmed diagnosis and 500 sex- and age-matched controls, to record their information related to demography, body measurement, lifestyle, diet, medication, diseases, and biochemistry, and to collect their feces, saliva, urine and blood samples.

Primary Objective:

To determine the risk of bleeding from ASA therapy in thrombocytopenic patients who develop Acute Coronary Syndrome (ACS), and assess its effect on the overall morbidity and mortality in these patients as well as platelet functions.

Aspirin is known to decrease death rate by as much as 50% in patients that suffer from heart attacks. Patients with low platelet count are not given aspirin for fear of an increased risk of bleeding. Researchers want to compare the risks versus the benefits of using aspirin in this patient population.

Participants in this study who suffer chest pain will be treated with a single enteric coated aspirin 325 mg instead of the current treatment without aspirin. Participants will then be tested to confirm that they had a heart attack by EKG (a test to measure the electrical activity of the heart) and blood tests (5ml of blood) will be drawn every 8 hours to detect enzymes that are released from the heart due to the heart attack. Blood samples will also be examined for platelet number.

Participants who are found to have had a heart attack and have a platelet count of between 100,000 and 20,000 will be continued on aspirin (160 mg per day). All other standard medications for heart attacks will also be given.

Participants who are found to have had a heart attack but whose platelet number is more than 100,000 will be given the standard therapy for heart attack, including enteric coated aspirin 325 mg per day, and will no longer take part in this study. Participants who are found to have had a heart attack but whose platelet number is less than 20,000 will be not be included in the study and will be treated as deemed appropriate by their primary physician.

Participants will be examined daily and evaluated for bleeding. Blood samples (30 ml of blood) will also be drawn before or after aspirin is given and 24 hours, 72 hours and 7 days after aspirin treatment to study platelet function. Participants will be followed up on the study for 7 days. Participants will be followed up in the cardiology clinic within 1-2 weeks after discharge from the hospital, then once a month for six month. Further follow up will be every 6 month. Patients are requested to follow up with cardiology by phone at any time for any bleeding.

Participants who are not found to have had a heart attack will not receive any further aspirin treatment.

This is an investigational study. Aspirin is an FDA approved drug for treatment of heart attacks and is commercially available. Aspirin is a standard therapy for patients who have had a heart attack. Thirty patients will take part in this study. All will be enrolled at M. D. Anderson.
