Mohs micrographic surgery followed by sentinel lymph node biopsy

Preoperative Lymphoscintigraphy: A standard injection of 2 mCi of 99m-Technetium sulfur colloid. Sequential immediate and delayed images will be obtained using SPECT/CT using a gamma probe. Ten-second counts will be done in areas of high activity to identify location of sentinel lymph node (SLN). After removal of the lymph node, a 10-second count will be done of the lymph node ex-vivo followed by 10-second count of the lymph node resection bed to ensure removal of SLN (less than 10% activity of LN).

Surgical resection: Mohs micrographic surgery followed by SLNB. Intraoperative margins around the tumor will be confirmed through frozen section analysis. Patients with planned free flap reconstruction, will undergo SLNB with intraoperative, frozen section analysis. If lymph node is positive, immediate completion neck dissection will occur.

The purpose of this study is to research if a type of biopsy known as sentinel lymph node biopsy (SLNB) can help in determining the rate of tumor deposits that are hard to detect and identify in node-negative cutaneous squamous cell carcinoma of the head or neck.

Nonmelanoma skin cancer is the most commonly diagnosed malignancy in the United States, with cutaneous squamous cell carcinoma (cSCC) representing about 20% of those cases. It is estimated that there are 700,000 patients diagnosed each year in the United States and the incidence has been increasing worldwide. Most tumors are found within the sun-exposed areas of the head and neck. The vast majority require only local treatment, however there is a subset of patients who present with regionally metastatic disease. Retrospective reviews have determined factors associated with recurrence, metastasis, and disease-specific death include tumor size ≥ 2 cm, location on lip and ear, perineural and lymphovascular invasion, poorly differentiated histology, tumor extension beyond fat and immunosuppressed patients.

There have been multiple proposed staging schemes, including the American Joint Committee on Cancer (AJCC) and recently the Brigham and Women's Hospital (BWH) tumor staging. AJCC staging utilizes the size for early stage tumors, T1 \< 2 cm and T2 ≥ 2 cm and less than 4 cm. Advanced tumors, T3 and T4 are stratified by aggressive features, including invasion beyond fat, perineural invasion (PNI), or bone/skull base erosion. BWH aimed to better stratify tumors by the number of aggressive features, T1 tumors with 0 features, T2a tumors with 1 feature, T2b with 2-3 features, and T3 with 4 or more features. High-risk features include tumor diameter ≥ 2 cm, poorly differentiated histology, PNI, tumor invasion beyond fat, or bone erosion. In a recent study, and found BWH provided superior prognostication for patients with localized cSCC as compared to AJCC7. However, BWH does not address regional or distant metastasis and therefore current treatment guidelines are based on BWH staging scheme. AAJCC systemCurrently, there is a general paucity of prospective data that can provide a consensus on risk-stratification of cutaneous squamous cell carcinomas and subsequent treatment algorithm. A consensus on risk-stratification for high-risk cSCC is lacking; consequently, treatment algorithm for primary tumors and nodal disease is not clear.

There is a subset of tumors with increased risk of local recurrence, nodal and distant metastasis which has yet to be elucidated. Data has shown metastasis to regional lymph nodes is the strongest predictor of recurrence and survival, and risk of nodal metastasis is noted to be 6% for all-comers and upwards of 20% for high-risk tumors. Therefore, detecting subclinical metastatic disease is extremely important for staging aggressive skin cancers and optimizing treatment.Imaging, including CT, MRI and PET, are considered for high-risk tumor. Data has demonstrated a higher risk of developing local recurrence, nodal metastasis and death from disease in patients that did not receive pre-treatment imaging as compared those who underwent imaging.

Guidelines recommend surgical excision of the primary tumor as first line treatment for cutaneous squamous cell carcinoma. Both Moh's micrographic surgery (MMS) and standard excision are both considered feasible surgical options. Available literature on management of lymph node metastasis largely limited to retrospective reviews and case series. Therefore, optimal management of regional lymph nodes for high-risk tumors remains unclear.

Sentinel lymph node biopsy (SNLB) is a common procedure utilized in multiple oncologic surgeries, especially dermatology. SLNB is recommended for aggressive skin cancers including melanoma and Merkel cell carcinoma. There is limited data on the utility of SLNB in nonmelanoma skin cancers, including cutaneous SCC. To date, there have been no prospective trials and current data comes from small retrospective studies. Therefore, the impact of SLNB on management and outcomes with high-risk cSCC remains unknown. Therefore, the impact of SLNB on cSCC remains unclear. The purpose of this study is to evaluate the rate of occult metastasis utilizing sentinel lymph node biopsy in clinically node-negative intermediate and high-risk cSCC.

Sentinel Lymph Node Biopsy for Cutaneous Squamous Cell Carcinoma of the Head and Neck

Incidence of occult metastasis in cT2-4N0 cutaneous squamous cell carcinoma (cSCC) of the head and neck by measuring the number of patients with positive sentinel lymph node biopsies.

Evaluate the tumor variables associated with occult metastasis using Fisher's exact tests for characteristics with categorical data and using logistic regression for characteristics with continuous data. (Tumor characteristics will be evaluated for their association with metastasis. Analyses will evaluate each of the characteristics individually using Fisher's exact tests for characteristics with categorical data and using logistic regression for characteristics with continuous data. No multivariable models will be assessed due to the small number of expected patients with metastasis. A 5% significance level will be used for all tests.)

Regional recurrence and distant metastasis confirmed by clinical, pathologic and radiographic evidence of metastasis.

Evaluate the gene profile via validated 40-gene expression profile (40-GEP) to identify common mutations associated with high-risk tumors characteristics.

This is a retrospective-prospective study design. Patients who completed treatment approximately 3 years (range of 2-4 years) at time of IRB approval of this study will be identified and any existing data in the patient's record will be collected in addition to conducting office visits for long-term follow-up.

The study will include 2 parts:

1. Retrospective chart review: Collect data from patient records who completed treatment a minimum of 2 years prior to study onset and determine the feasibility of doing a matched pair cohort study comparison of eBx versus Mohs surgery. The history, demographic and treatment data will be retrospectively collected from up to 320 subjects previously treated with eBx for the treatment of NMSC and up to 320 subjects previously treated with Mohs surgery for the treatment of NMSC.
2. Prospective Follow-up: Patients will return for long-term follow-up visits for the investigators to assess the lesion site, document absence of recurrence/ absence of recurrence, toxicities, and collect patient reported outcomes.

This study examines melanoma and nonmelanoma skin cancer in people diagnosed with vitiligo compared to matched controls.

For the primary analysis, this study examines the risk of melanoma and nonmelanoma skin cancers: squamous cell carcinoma and basal cell carcinoma in people diagnosed with vitiligo. It also examines actinic keratoses. In secondary analysis, this study examines the prevalence of skin cancer in people diagnosed with vitiligo compared to matched controls.

This study also examines whether the susceptibility to melanoma and nonmelanoma skin cancer may vary between different patient subgroups, e.g. ethnicity, those who undergo phototherapy or treatment with the new targeted biologic immunosuppressive therapies.

One-third of all malignancies in the United States (approximately one million cases diagnosed annually) are nonmelanoma skin cancer (NMSC). NMSC causes considerable morbidity, economic burden, facial deformity and at least 1,000 deaths annually. Prevention of these malignancies with a topical agent free of serious side effects would confer substantial public health benefit. Three hundred fifty thousand veterans were expected to develop NMSC in 1994. NMSC is one of the most common conditions requiring dermatologic care in the VA system. Topical tretinoin has been used extensively to treat photoaged skin. Retinoids administered orally in high doses appear to be effective in chemoprevention of nonmelanoma skin cancer but have unacceptable toxicity. In this study, 1131 patients with a recent history of squamous cell and/or basal cell carcinoma were enrolled at six participating centers over a four-year period and were randomly assigned to either 0.1% tretinoin cream or placebo. They were followed for a minimum of two years to determine if topical tretinoin is effective in reducing the risk of new occurrences.

Primary Hypothesis: To determine the efficacy of topical tretinoin cream for the prevention of nonmelanoma skin cancer (NMSC) among high risk individuals (at least 2 NMSC?S in last 5 years).

Secondary Hypothesis: Secondary objectives are: (a) to determine the long-term effect of topical tretinoin on the prevalence of premalignant actinic keratoses, and (b) to distinguish subpopulations in which topical tretinoin is particularly effective or ineffective, compared to the overall study population.

Intervention: Apply Tretinoin 0.1% cream or placebo cream to face and ears twice a day.

Primary Outcomes: New NMSC lesions on the face and ears. Number of actinic keratoses on the face and ears.

Placebo, vehicle control

The main purpose of this study is to see if 5-fluorouracil (5-FU) skin cream can prevent the growth of new skin cancers on the face and ears. The cost of trying to prevent skin cancer will be compared to the usual cost of treating skin cancer. Participants are being asked to be a part of this study because the participants have been treated for two or more skin cancers within the past five (5) years. At least one of these cancers occurred on the face or ears. Having had two or more skins cancers in the past 5 years makes it likely that participants will develop additional skin cancers in the future.

Exposure to ultraviolet radiation from the sun or artificial sources such as tanning beds is a major cause of basal cell and squamous cell carcinoma of the skin. Using lotions, creams, or gels that contain sunscreens can help protect the skin from premature aging and damage that may lead to skin cancer.

The 5-FU skin cream used in this study is FDA-approved to treat some types of skin cancers and spots that might become skin cancer. However, 5-FU skin cream has never been studied to see if it can prevent skin cancer. This drug is not approved by the FDA for how it will be used in this study.

In this study, one half of the patients will use the 5-FU cream and the other half will use a skin cream that looks identical to the 5-FU cream but does not have 5-FU or any other active drug in it.

Approximately twelve VA medical centers will work together in this study. About one thousand (1000) patients will be in this study. The study is sponsored by the U.S. Department of Veterans Affairs Cooperative Studies Program.

Basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) of the skin, both of which are keratinocyte carcinomas (KCs), account for a half of all cancers in the United States, and over 100,000 diagnoses per year in the VA. The standard treatment for these KCs is excision of the lesion, and they cost the US health care system some $2.5 billion annually and about $100 million annually in the VA. There is no proven means to prevent KCs (except perhaps for a modest benefit of intensive daily sunscreen use). An effective prevention strategy would dramatically change the way high-risk patients are managed and could substantially reduce the costs of care. The investigators' preliminary analysis indicates that the savings will be $116 per high-risk patient and will account for a total national savings of over $11 million. These findings imply that the study would pay for itself by the end of 4 years. The investigators hypothesize that topical 5-fluorouracil (5-FU) chemoprevention will prevent skin cancer surgeries and will be cost-saving. To test this the investigators propose a randomized controlled trial of 5-FU compared to a vehicle control to the face and ears in a high-risk population.

In the study, 1000 Veterans at high-risk of skin cancer defined as at least 2 KCs in the prior 5 years, at least one of which was on the face or ears, will be randomized to 5-FU or a vehicle control cream, and followed for 2 to 4 years. The primary endpoint will be surgery for any KC on the face and ears. The investigators will also assess the cost of care, quality of life, the side effects associated with treatment, and the prevalence and number of actinic keratoses, a skin cancer precursor and itself a cause of morbidity and cost. By targeting patients at high-risk, the study focuses on high-cost patients for whom this treatment could both improve outcomes (cancers and quality of life) and reduce costs.

CSP #562 - The VA Keratinocyte Carcinoma Chemoprevention Trial

Adverse events collected were those related to the study intervention. Data on adverse events were collected spontaneously through patient reports, actively elicited at each clinic visit through open ended questionings and examination, and gathered at the time of telephone contact during the therapy period. All serious adverse events were collected, including those considered related and unrelated to the study interventions.Subjects were monitored for SAE at each study visit and telephone call.
