Pear Bio has developed an organ-on-a-chip device together with a computer vision pipeline through which the response of an individual patient's tumor to different systemic therapy regimens can be tested simultaneously ex vivo. This study will recruit patients with advanced or metastatic triple negative breast cancer who are due to start a clinically-indicated new line of therapy.

The oncologist will be blinded to the response on the Pear Bio tool (the assay will be run in parallel with the patient's treatment). The primary objective of this study is to establish the sensitivity and specificity of Pear Bio's test against patient outcomes (response, progression-free survival, overall survival)

This is a UK-based observational study that aims to discover novel predictive biomarkers with the potential to guide treatment decision making and prolong PFS and OS in patients with advanced TNBC. Patients will undergo a mandatory, study-specific core needle biopsy or fine needle aspiration of the breast tumour or metastasis before commencing their next line of therapy. The research sample will be run on Pear Bio's test whilst the patient receives therapy as per their physician's choice. This study will not use Pear Bio's tool to inform the choice of treatment, with the treating oncologist being blinded to the test results. Treatment response data will be collected at multiple timepoints to conduct analyses on the study's secondary and tertiary objectives.

Fresh tissue resections that arrive at Pear Bio's lab will undergo processing, cell culture and various drug dosing and omics assays (depending on extracted cell numbers). Tumour samples will be processed using a cell isolation kit to retrieve a viable single-cell suspension. A minimum of 100,000 cells (10,000 viable cells per chip) will be used for staining with live and dead cell-tracking dyes. In parallel, blood vials will be processed for PBMCs and further effector cell extraction (flow cytometry, Dynabeads, etc). The remaining cells will be used for sequencing (DNA/RNA), fixed for immunofluorescence characterisation of biomarkers/receptor status or used for further omics assays (if cell numbers allow). This may include tumour mutational burden and microsatellite instability testing.

The stained cells will be cultured in a biomimetic hydrogel within Pear Bio's organ-on-a-chip to provide a physiological 3D environment for drug dosing experiments. Using a microfluidic device, samples in each chip will be exposed to approved therapies (either as monotherapy or combination therapies, as outlined below) over multiple days.

In parallel, PBMCs will be extracted from whole blood, characterised and sorted via flow cytometry and fluorescence-activated cell sorting (FACS) or magnetic beads selection. Cells of interest (e.g. CD8+ T cells) will be used for culture in Pear Bio's chips jointly with cells isolated from the matched tumour sample. To test immunotherapies, tumour cells will be co-cultured with immune cells in a modified organ-on-a-chip architecture. Chips receiving immunotherapies may be tested for tumour mutational burden and/or microsatellite instability.

Confocal microscopy will be conducted daily to collect 3D image data of the cells and track their position and behaviour over time. At the end of the assay, the 3D cell cultures will be fixed for further 3D immunofluorescence analyses or used for embedding, sectioning and assessment of spatial transcriptomics. For targeted therapies, RNAseq, IF and other omics data will be integrated to confirm drug MoA and identify other potential therapeutic targets. Concurrently, 3D image data is processed through a computer vision pipeline to measure functional metrics of the ex vivo 3D cell cultures, including cell viability, cell culture width and cell migration, both at a bulk tumour level and at a single-cell resolution. For immunotherapies, additional metrics such as immune cell infiltration and immune cell killing will be recorded. A patient report is then generated to outline an individual patient sample's response to each therapy tested.

In our previous study (title: Expression of Major Histocompatibility Complex Molecules class II- HLA-DR in Squamous Cell Carcinoma of the Head and Neck. ID 2222)the Authors verified that the epithelial cells of squamous cell carcinoma of the head and neck acquire the ability to express HLA-DR. Although the role of the expression of these molecules on neoplastic cells still remains controversial, a positive association between HLA-DR expression and clinical outcome was observed by us in analogy to what was reported by several studies on various types of tumors : in squamous cell carcinoma of the larynx, colorectal cancer , stomach cancer and others. In these tumors the expression of HLA-DR correlates with the presence of immune cells such as CD16+/CD11c macrophage myeloid cells, associated with a good prognosis and T cells which, recalled in the damaged tissue, they determine the formation of an immunogenic microenvironment that could support an anti-tumor immune response. Oncology studies are in fact focusing on the role of the tumor microenvironment which is characterized by different cell populations, among which the most abundant population is represented by tumor-associated fibroblasts (CAF). CAFs are fibroblasts which, in a tumor context, assume a phenotype similar to that of myo-fibroblasts and are distinguishable from normal fibroblasts by a greater expression of α-sma, FAP and FSP-1, which represent their specific markers, as well as a greater expression of vimentin, fibronectin, and type XI collagen. Numerous evidences in different types of tumors have reported both the immunosuppressive role, as these cells are capable in vitro of inhibiting the proliferation of T lymphocytes, to favor their apoptosis or to induce the phenotype of regulatory T lymphocytes; and the pro-tumor role, as they are capable of promoting tumor proliferation and invasion, angiogenesis and metastasis, thus contributing to the worsening of the prognosis. Many studies are directing their research on which factors secreted by CAFs are responsible for their function. In particular, among the many factors secreted by CAFs, there are the interleukins IL-17 and IL-33 which, as it has been demonstrated, can induce the activation of HLA-DR molecules on bone marrow-derived mesenchymal cells . It therefore seems interesting to investigate the role of HLA-DR in relation to the presence of the tumor microenvironment represented by CAFs.

Several studies have demonstrated a correlation between the expression on tumor cells of MHC II (major histocompatibility complex class II) molecules, in particular of HLA-DR (human luekocyte antigen) molecules, and the clinical outcome of the disease. Indeed, it has been observed that tumor cells under inflammatory stimulation of IFN-γ become capable of expressing MHC II molecules functioning as non-professional APCs (antigen presenting cells). The APC cells are responsible for presenting the antigen to the T lymphocytes, thus inducing their activation, which necessarily requires the presence of two signals: the first induced by the MHC molecules (MHC class II for CD4+ lymphocytes) and the second induced by Costimulatory molecules CD80 or CD86, without which lymphocytes undergo a state of anergy.

Although the role of the expression of these molecules on neoplastic cells still remains highly controversial, a positive association between HLA-DR expression and clinical outcome has been reported by several studies on various types of tumors: in squamous cell carcinoma of the larynx , colorectal cancer , stomach cancer , and others. Also in our study "Expression of the Molecules of the Major Histocompatibility Complex class II-HLA-DR in Squamous Cell Carcinoma of the Head and Neck" (ID 2222) the investigators were able to verify that the neoplastic epithelial cells of squamous cell carcinoma of the head and neck acquire the ability to express HLA-DR (data not shown, in progress of the scientific work to be presented to journal with impact factor and of the final PhD thesis in Neuroscience of the PhD student Chiara Prampolini). In some types of tumors, the expression of HLA-DR correlates with the presence of immune cells such as CD16+/CD11c macrophage myeloid cells, associated with a good prognosis and T cells which, recalled in damaged tissue, thus determine the formation of an immunogenic microenvironment which could thus support an anti-tumor immune response . Therefore, the variability in the expression of HLA-DR molecules would explain the different immune involvement in the tumor site and would condition the host's ability to stem tumor progression.

* Isolate tumor epithelial and cancer-associated fibroblast (CAF) cell cultures from pathological and healthy tissue samples from patients with primary squamous cell carcinoma of the head and neck Secondary objectives of the study
* To investigate the expression of HLA-DR in the obtained cell cultures. Enrolled patients and enrollment methods Calculation of the sample size The calculation of the sample size was carried out using Power Analysis, deeming acceptable a first type error equal to 5% with a test power of 90%, therefore a second type error of 10%. To determine the delta, the minimum acceptable difference to be detected between the two groups being compared (group A: healthy tissue; group B: pathological tissue) and the standard deviation relating to the populations under examination, the calculations of the effect size were followed guidelines proposed by Choen .

Patients Four patients with primary squamous cell carcinoma of the head and neck admitted for surgical treatment at the Otolaryngology Clinic, Fondazione Policlinico Gemelli, IRCCS, Rome, in the year 2020-2021 will be included in the study.

Of the 4 cases considered will be available:

* Clinical data
* Informed consent
* Tumor tissue sample
* Non-tumor tissue sample taken from material surgically removed during surgery

Informed consent The subjects involved in the study will be invited to read the project information sheet and to personally sign the informed consent for the use of their stored biological material and clinical data for research purposes.

Organic material For each patient included in the study, a biopsy will be taken both from the pathological tissue and from the healthy tissue removed during the therapeutic surgery, used as a control. Both biopsies will be cultured.

5. Study design The study is a non-profit, single-centre that consists of two phases. The expected duration of the study is 12 months.

I PHASE:

Selection of cell populations

Through specific culture media will be selected:

* Epithelial cells from healthy biopsy and tumor epithelial cells from tumor biopsy maintained in culture with: DMEM High Glucose + HAM'S F12 1:1, FBS 10%, penicillin-streptomycin 1%, L-glutamine 1%, insulin 10µg /ml, EGF 20ng/ml, Hydrocortisone 0.5µg/ml;
* Fibroblasts from healthy biopsy and tumor associated fibroblasts (CAF) from tumor biopsy maintained in culture with: DMEM High Glucose, FBS 10%, penicillin-streptomycin 1%, L-glutamine 1%. Both fibroblasts and CAFs will be isolated from epithelial and tumor cell cultures, respectively, with a series of passages in trypsin-EDTA 0.25%

Cell populations from the healthy biopsy (healthy epithelial cells and fibroblasts) will be used as negative control.

PHASE II: Evaluation of HLA-DR expression in tumor epithelial cells and tumor CAFs HLA-DR positivity will be evaluated by double immunofluorescence analysis with the addition of an epithelial marker (cytokeratin), in addition to the evaluation of the presence of classical costimulatory molecules (CD80, CD86).

HLA-DR+ tumor cells that do not express the costimulatory molecules will be stimulated in culture with the administration of IFN-ɣ and LPS, in order to induce their expression .

Statistic analysis Comparisons between categorical data (healthy and pathological tissue) will be made by chi-square test or Fisher's exact test, where appropriate. All tests will be two-tailed and an alpha \<0.05 will be considered statistically significant.

Digestive cancers (liver, colonic, pancreatic) have a high incidence and high mortality, their population prevalence is also increasing. Given that the anesthesia techniques and the agents used act directly and indirectly on the immune system during the perioperative period, influencing both the treatment and the prognosis of patients with colorectal cancer who undergo elective interventions, a series of perianesthetic interventions have been proposed in order to reduce morbidity-mortality perioperative.

Contributions to the Elucidation of the Mechanisms and Effects by Which Certain Perianesthetic Interventions Modify Long-term Evolution of Patients With Digestive Cancers Subjected to Surgery

The investigators aim to correlate the anticancer effects of two anesthetic techniques with tumor markers (p53;p38) and cell proteins involved in proliferation or apoptosis (IGFR;Bcl-2;Bcl-6).

The investigators will report if the anesthetic agent used in cancer surgery influences the serologic values of these markers.

The investigators will investigate the serum of the patients who received different types of anesthesia (elective colorectal cancer surgery) by incubating it with colon cell lines (HCT116). The investigators will concentrate on cell proliferation assay.

The investigators aim to discover how the growth of HCT116 will be influenced by patients' serum in terms of rate of proliferation. The measurements which will be used are Inhibitory Concentration (IC50) measured at 0 , 24 and 48 hours after incubation.

The concentrations will be measured in mcg/ml.

The investigators aim to determine de plasmatic concentration of lidocaine in the patients serum , which have received lidocaine perioperatively.

The investigators will do a Mass-Spec analysis of lidocaine

The investigators will do a follow up for the patients in all the 3 groups. The investigators want to investigate the survival ratio for the patients that had a elective colorectal cancer surgery.

This study will recruit patients with early TNBC who are planned for standard of care neoadjuvant chemotherapy followed by surgery. The oncologist will be blinded to the response on the Pear Bio tool (the assay will be run in parallel with the patient's neoadjuvant chemotherapy). The primary objective of this study is to establish the sensitivity and specificity of Pear Bio's test against patient outcomes (pathological complete response).

This is a multicenter, UK-based, observational pilot study that aims to determine the accuracy of a new assay, the Pear Bio tool, in predicting pathological complete response (pCR) in patients receiving neoadjuvant chemotherapy for early TNBC. Patients will undergo an additional, mandatory biopsy of the breast tumor before commencing neoadjuvant chemotherapy. The biopsy sample will be run on the Pear Bio tool while the patient receives their standard of care neoadjuvant chemotherapy. As such, for this study, the result from the Pear Bio tool will not be used to inform the choice of neoadjuvant chemotherapy and the treating oncologist will be blinded to the assay results. The pathological outcome from surgery (pCR vs non-pCR) will be collected and used to calculate the specificity of the assay as the primary endpoint of the study. Sensitivity, positive predictive value and negative predictive value will also be measured.

Pear Bio has developed a 3D microtumor assay and computer vision pipeline through which the response of an individual patient's tumor to different anti-cancer regimens can be tested simultaneously ex vivo. This study will recruit patients with primary brain tumors who are due to undergo surgery.

Oncologists will be blinded to treatment response on the Pear Bio tool (the assay will be run in parallel with the patient's treatment). The primary objective of this study is to establish the ex vivo model and confirm whether approved therapies exhibit their intended mechanism of action in the model. Secondary objectives include correlating test results to patient outcomes, where available.

This is a multicenter, observational pilot study that aims to determine the feasibility of using the Pear Bio tool in patients with primary solid brain tumors.

Patients who are due to undergo clinically-mandated surgery will provide informed consent to donate excess tissue and a 40mL blood sample.

The samples will be tested on the Pear Bio tool while the patient receives their standard of care treatment. As such, for this study, the result from the Pear Bio tool will not be used to inform the choice of treatment and the treating oncologist will be blinded to the assay results.

The investigators will examine drug mechanism of action (MoA), differentiated response, and correlate that with outcomes seen in the patients treated in routine clinical practice.
